Zusammenfassung Die chirurgische Vorbehandlung zur Nierentransplantation umfaßt prophylaktische Eingriffe, beispielsweise solche, die eine spätere Magenulcusblutung durch Vagotomie, Pyloroplastik oder2/3-Resektion oder das Fortschreiten eines renalen Hochdrucks durch bilaterale Nephrektomie verhindern. Nicht selten (3 Fälle bei 43 Transplantationen) ist die Komplikation des sekundären Hyperparathyreoidismus mit seinen Folgeerscheinungen am Gefäßsystem, die durch subtotale Nebenschilddrüsenresektion zu behandeln ist. Im Einzelfall lassensich genaue Indikationen für die jeweilige Maßnahme angeben. Während der Transplantation ist besondere Sorgfalt auf die Anastomosierung von Arterien und Venen, auf eine subtile Blutstillung und auf die Vereinigung der ableitenden Harnwege zu verwenden. Bei 43 Transplantationen beobachteten wir 2 Arterienstenosen, 1 völligen Verschluß, 1 Venenthrombose, 4 postoperative Nachblutungen, 6 Ureterinsuffizienzen und 1 Harnleiterstenose. Diepostoperative Aufgabe des Chirurgen erstreckt sich hauptsächlich auf die Behandlung von Komplikationen, am häufigsten von solchen infektiöser Natur. Trotz immunsuppressiver Behandlung sollte in diesen Fällen rechtzeitig operiert werden.

Summary The surgical preoperative treatment before kidney transplantation embraces prophylactic surgery, e.g. such as prevents a later gastric ulcer haemorrhage through vagotomy, pyloroplasty or a 2/a resection or the further development of renal hypertension through bilateral nephrectomy. A not uncomomn occurrence (3 cases in 43 transplantations) is the complication of a secondary hyper-parathyroidism with its consequent vascular manifestations, which should be treated with subtotal parathyroid resection. In the individual case, exact indications can be determined for the measures to be taken. Duringthe transplantation, special attention should be given to the anastomosing of arteries and veins, subtile haemostasis and the suturing of the efferent ureters. In 43 transplantations, we observed 2 cases of arterial stenosis, i complete contusion, 1 veinous thrombosis, 4 postoperative secondary haemorrhages, 6 cases of ureteral insufficiency and 1 case of ureteral stenosis. Thepostoperative task of the surgeon embraces mainly the treatment of complications, most often those of an infectious nature. In spite of immunosuppressive therapy, operative intervention should be timely in these cases.
