Summary The size and degree of variation of the Basal line (FM — CP), the height of the Thalamus, and the width of the ventricles were investigated in an collection of 100 lateral and anteroposterior pneumencephalograms as a basis to over 1200 stereotactic operations. The Basal line and the height of the Thalamus showed slight individual range of variation and were confirmed as the important intracerebral reference lines. Nevertheless the range of variation of these two reference lines and especially the variations of the ventricle size are so pronounced that the necessity for the calculation of the target point in each individual cannot be dispensed with. In our investigations the ratios of the Basis line to the height of the Thalamus, of the Basal line to the width of the ventricles, and of the height of the Thalamus to the ventricle width showed no relationships of practical value. The craniocerebral ratios of Basal line to skull length, of Thalamus height to skull height, and of ventricle width to skull width showed a range of variations which, in part, was pronounced. These variations were also apparent in an investigation (50 cases) into the relationship of the antero-internal Pallidum to the region of the sella, determined on the basis of intracerebral calculations with thorough consideration of the individual circumstances. A parallel investigation (32 cases) of the distances between these two targets and the Habenula showed that the craniocerebral variations in distances were considerably larger than the variations in distance between the two intracerebral reference points. These findings demonstrate the unreliability of craniocerebral relationships, especially for attempts in stereotactic pallidotomy to locate the medial Pallidum using the sella turcica. These findings also provide a firm indication that one of the craniocerebral topographic correlations developed method for intracerebral localisation during stereotactic operations is valueless.

Résumé La taille et le degré de variation de la ligne basale (FM — CP) la hauteur du thalamus et la largeur des ventricules furent mesurées sur une collection de 100 encéphalographies de profil et de face sur plus de 1200 opérations stéréotaxiques. La ligne basale et la hauteur du thalamus montre une légère variation individuelle et furent confirmé dans leur valeur de ligne de référence cérébrale importante. Toutefois les variations de ces deux lignes de repère et spécialement celles de la taille des ventricules furent si marquées que la nécessité de faire le point dans chaque cas ne peut pas être évité. Dans nos investigations le rapport de la ligne basale et de la hauteur du thalamus, de la ligne basale à la largeur des ventricules et de la hauteur du thalamus à la largeur des ventricules ne montrent pas de relations de valeur pratique. Ce rapport de la ligne basale à la longueur du crâne, de la hauteur du thalamus à la hauteur du crâne et de la largeur du ventricule à la largeur du crâne montre une série de variations nettes. Ces variations étaient aussi nettes dans une investigation (50 cas) du pallidum interne à la région sellaire déterminées sur la base de mesures cérébrales individuelles. Ces recherches démontrent l'absence de, relations crâniocérébrales qui puissent permettre de tenter la pallidotomie stéréotaxique et de situer le pallidum interne d'après la selle turcique. Elles apportent aussi l'indication qu'aucune des méthodes de corrélation topographique crâniocérébrale utilisées pour les localisations cortico cérébrales en vue d'opération stéréotaxique n'a de valeur.

Zusammenfassung An einem Kollektiv von 100 lateralen und antero-posterioren Pneumencephalogrammen aus den Unterlagen zu über 1200 stereotaktischen Eingriffen wurden die Größe und Größenvariation von Basislinie (FM—CP), Thalamushöhe und Ventrikelbreite untersucht. Mit ihrer geringen individuellen Schwankungsbreite bestätigen sich Basislinie und Thalamushöhe als wesentliche intracerebrale Bezugslinien. Dennoch ist die Schwankungsbreite dieser beiden Bezugslinien und im besonderen die individuelle Variation der Ventrikelbreite so erheblich, daß die Notwendigkeit einer individuellen Zielpunktberechnung nicht aufgehoben wird. Intracerebrale Relationen von Basislinie zur Thalamushöhe, von Basis-Jinie zur Ventrikelbreite und von Thalamushöhe zur Ventrikelbreite zeigen nach unseren Untersuchungen keine praktisch verwertbaren Gesetzmäßigkeiten. Die cranio-cerebralen Korrelationen von Basislinie zur Schädellänge, von Thalamushöhe zur Schädelhöhe und von Ventrikelbreite zur Schädelbreite, sowie das Lageverhalten der Basislinie zur Schädelbasis unterliegen einer zum Teil erheblichen Schwankungsbreite. Diese Schwankungen zeigen sich ebenfalls bei einer Untersuchung über das Lageverhalten des — auf Grund intracerebraler Berechnung unter weitgehender Berücksichtigung individueller Verhältnisse bestimmten — Zielpunktes im vorderen Pallidum internum zum Sellabereich (50 Fälle). Eine vergleichende intracerebrale Distanzuntersuchung zwischen diesem Zielpunkt und der Commissura habenularis (32 Fälle) zeigt, daß die craniocerebralen Distanzschwankungen erheblich größer sind als die Distanzschwankungen der beiden intracerebralen Bezugspunkte. Diese Befunde cranio-cerebraler Korrelationen belegen die Unzuverlässigkeit cranio-cerebraler Beziehungen im besonderen für die, von den Sellastrukturen ausgehende Lokalisation des Pallidum internum bei stereotaktischer Pallidotomie, und sind grundlegender Hinweis dafür, daß eine auf der craniocerebralen Korrelationstopographie aufbauende intracerebrale Lokalisation für stereotaktische Hirnoperationen keinen Gebrauchswert besitzt.
