Summary In the case of 40 patients with rheumatoid arthritis (R.A.) with different stages of activity, the serum level of the “collagen-like-protein” (C.L.P.) was determined according to the method ofLe Roy, Kaplan, Udenfriend, andSjoerdsma and compared with the level of 20 normal persons. The CLP level showed up to ten times above normal in cases of active R.A. The results to hand, proved statistically, showed a significant correlation with the stages of activity according toVoit andGamp and with the Lansbury-index. In the same manner a significant correlation could be proven with the α2-globulinfraction and the protein bound neutral sugar in serum. Accordingly the CLP level seems to be a useful parameter for the activity of R.A. One special advantage is that it is not dependent on the amount of food intake, especially on inflammation of parenchymal, only small amount of collagen containing organs, which influence quickly and permanently for instance the level of the α2-globulin, of glycoprotein and of CRP in serum. In the case of other connective tissue disturbances, Pagets's diesease, scleroderma, Marfan's syndrome and Ehlers-Danlos syndrome, the CLP level is also raised. In such cases however the disease can easily be differentiated by the R.A. The CLP level is dependent on age, but only during childhood. With the CLP determination greater reliability is possible in estimating the activity of R.A. The use of the same in the routine diagnostics is relatively limited on account of the different emthods meployed.

Zusammenfassung Bei 40 Patienten mit chronischer Polyarthritis (C. P.) verschiedener Aktivitätsstadien wurde der Serumspiegel des „collagen-like-protein“ (CLP), vergleichsweise bei 20 Gesunden, nach der Methode vonLe Roy, Kaplan, Udenfriend undSjoerdsma bestimmt. Der CLP-Spiegel fand sich bei aktiver C.P. bis zum Zehnfachen der Norm erhöht. Für die vorliegenden Werte ließ sich eine signifikante Korrelation sowohl zu den Aktivitätsstadien nachVoit undGamp wie zum Lansbury-Index statistisch sichern. In gleicher Weise konnte eine signifikante Korrelation zur α2-Globulinfraktion und zum Spiegel der eiweißgebundenen Neutralzucker im Serum nachgewiesen werden. Damit erscheint der CLP-Spiegel als brauchbarer Parameter für die Aktivität der C.P. Ein besonderer Vorteil besteht in der Unabhängigkeit von der Nahrungsaufnahme, vor allem von Entzündungen parenchymatöser, nur wenig Kollagen enthaltender Organe, die beispielsweise den Spiegel der α2-Globuline, der eiweißgebundenen Neutralzucker und des CRP rasch und nachteilig beeinflussen. Bei anderen Bindegewebserkrankungen ist der CLP-Spiegel ebenfalls erhöht, so beim Morbus Paget, bei der Sklerodermie, beim Marfan-Syndrom und Ehlers-Danlos-Syndrom. Allerdings lassen sich diese Erkrankungen von der C.P. jedoch leicht abgrenzen. Bei Kindern besteht eine Altersabhängigkeit des CLP-Spiegels. Mit der CLP-Bestimmung ist eine höhere Zuverlässigkeit in der Beurteilung der Aktivität der C.P. erreicht. Die Anwendung in der Routinediagnostik ist durch die relativ aufwendige Methode noch beschränkt.
