    Notes: Background  The most frequent side-effects due to nevirapine (Viramune®), the first non-nucleoside HIV reverse transcriptase inhibitor introduced for clinical use, are cutaneous hypersensitivity reactions. In non-serious cases, tolerance induction can be proposed.Objective  Describe the long-term safety and efficacy of tolerance induction to nevirapine.Methods  Six HIV-infected patients started a tolerance 1 to 2 months after an episode of nevirapine hypersensitivity. Increasing doses of nevirapine (2.5, 10, 25, 100, 250 and 750 μg and 2.5, 5, 10, 25, 50 and 100 mg) were given orally at half-hour intervals under strict medical surveillance and patients were followed up regularly.Results  All but two of the patients tolerated this induction well. One had a mild and transient reaction on day 1; nevirapine was not stopped. One had a moderate to severe reaction one month later; nevirapine was stopped and he recovered fully. All other patients have been taking nevirapine for more than 3 months without any adverse reaction.Conclusion  Therefore, when no alternatives are available in severely ill and hypersensitive HIV-infected patients, tolerance induction is a possible therapeutic option.

    Notes: Summary A nucleotide fragment encoding amino acids 29 to 94 in the E-protein of 28 dengue-2 isolates of diverse geographic and host origins was examined by direct sequencing of a polymerase chain reaction (PCR)-amplified product, and compared to six previously published sequences. Nucleotide divergence ranged from 0 to 19.8% corresponding to a maximum of 9% divergence in the amino acid sequence. Taking a divergence of 6% between the nucleotide sequence as a cut off for genotype classification, six groups have been established. Southeast Asian and the Jamaican 1983 genotypes show a high rate of similarity (〉95.2%). Our results suggest that virus of this group is now circulating as the dominant topotype in Brazil (1990) and in French Guyana (1986–1991). African strains fall into two groups, one endemic group (1970–1990) and one epidemic group (1986–1987). The three other groups correspond to viruses from Sri Lanka (1982) and the Seychelles (1977), from Puerto Rico (1973) and from Tahiti (1975). Our approach appears to be valuable characterizing dengue isolates, easily and rapidly.

    Notes: Abstract For long life nuclear wastes (essentially actinides) research is in progress to propose new matrices with an improvement of the chemical durability. High silica content glasses present high chemical durability, and we describe a process to prepare silica glass embedding the nuclear waste. Porous silica (gel) is used as a host matrix for nuclear waste. Neodymium oxide and cerium oxide are used to simulate the actinide oxides. The gel is soaked in a solution containing the simulant in nitrate salt form. We investigate the effect of the porous network features (pore size distribution) on the ability of the material to soak up the simulating salts. A new kind of gel (composite aerogel) is proposed owing to its large permeability. After drying and nitrate decomposition, the composite material is fully sintered trapping the nuclear waste. The glass-ceramic materials (silica glass + simulating oxide) have been synthesized with simulant content close to 20 weight percent. We show that the 2 simulant oxides behave differently because of their ability to form silicate phases. The chemical durability of the glass-ceramics is investigated. The large improvement of the chemical durability (60 times) compared to the classical borosilicate glasses shows that this containment process can be a suitable way to confine actinides and long life nuclear wastes.

    Notes: Abstract In vitro secretion of antibodies to cytomegalovirus was investigated by analysis of peripheral blood mononuclear cell culture supernatants from subjects infected with cytomegalovirus. Patients with primary or recurrent cytomegalovirus infection showed transient in vitro cytomegalovirus-specific antibody secretion. A high proportion of patients infected with human immunodeficiency virus and seropositive for cytomegalivirus showed in vitro cytomegalovirus-specific antibody secretion. All peripheral blood mononuclear cell cultures from patients with symptomatic human immunodeficiency virus infection were found to secrete in vitro antibodies to cytomegalovirus, despite the fact that isolation of cytomegalovirus from some of these patients was not achieved. In human immunodeficiency virus-infected patients, in vitro secretion of anti-cytomegalovirus antibodies appeared to be persistent. In vitro cytomegalovirus-specific antibody secretion by peripheral blood lymphocytes probably reflects an in vitro cytomegalovirus-specific B cell activation. This new assay could be considered an interesting method for detecting both acute or chronic cytomegalovirus infection, with potential use in routine laboratory practice.

    Notes: Abstract The sensitivity to fluconazole, serotype and adherence to buccal epithelial cells of 46Candida albicans isolates was studied in vitro. Strains were isolated from 18 HIV-positive patients with variable CD4+ lymphocyte counts who were receiving long-term fluconazole therapy. Results showed that sensitivity to fluconazole correlated with serotype. Of the sensitive isolates 81.5 % were serotype A and 18.5 % serotype B, whereas of the resistant isolates 37.5 % were serotype A and 62.5 % serotype B. Sensitivity to fluconazole also correlated with adherence capacity, resistant isolates showing slightly less adherence to buccal epithelial cells than sensitive isolates. Weakly adherent isolates represented 87 % of resistant isolates and only 63 % of sensitive isolates. Resistant strains were isolated in highly immunocompromised patients. All strains isolated in patients with CD4+ lymphocyte counts above 200 lymphocytes/µl were sensitive, whereas intermediate and resistant strains were isolated in patients with counts below 200 lymphocytes/µl. Statistical analyses showed no correlation between adherence of isolates to buccal epithelial cells and CD4+ lymphocyte counts.
