    Description / Table of Contents: Sommario In 6 centri facenti parte del Gruppo Italiano per lo Studio dei Disturbi del Movimento è stata valutata l'efficacia della somministrazione di tossina botulinica A in 251 pazienti affetti da distonia focale e da spasmo del facciale. Nei pazienti con blefarospasmo, la percentuale media di miglioramento osservata è compresa tra il 66 e l'81%, mentre nei pazienti con torcicollo varia tra il 40% e il 51%. Nei pazienti affetti da spasmo del facciale la percentuale media di miglioramento è compresa tra il 73% e l'81%. Buoni risultati sono stati ottenuti anche nella terapia di distonie focali meno frequenti, come la distonia oromandibolare e laringea e il crampo dello scrivano. Gli effetti collaterali osservati sono risultati generalmente lievi, locali e transitori. Lo studio conferma quindi l'utilità della tossina botulinica nella terapia sintomatica delle distonie focali e nello spasmo del facciale.
    Notes: Abstract In six Centers belonging to the Italian Movement Disorder Study Group, the efficacy of botulinum toxin treatment was evaluated in an open collaborative study in 251 patients with focal dystonia and hemifacial spasm. The percentage of functional improvement ranged from 66% to 81% in patients with blepharospasm, from 40% to 51% in patients with spasmodic torticollis and from 73% to 81% in those with hemifacial spasm. Good results were also obtained in patients with oromandibular dystonia, laryngeal dystonia and writer's cramp. Side effects were mild and transient. Local botulinum toxin injection is the first choice symptomatic treatment in focal dystonia and hemifacial spasm.

    Description / Table of Contents: Sommario Vengono descritti tre casi di neuropatia da “intrappolamento” del ramo interosseo del nervo radiale. Le osservazioni relative a questi tre casi confermano la tesi che questo tipo di neuropatia è causata dall'interazione di fattori esogeni di varia natura (occupazione, gravidanza, compressione durante il sonno, ecc.) con anomalie anatomiche a livello del muscolo supinatore breve.
    Notes: Abstract Three unusual cases of posterior interosseous nerve palsy involving an entrapment mechanism are described. The findings in these three cases seem to support the view that PIN entrapment usually results from interaction of exogenous factors (occupation, pregnancy, compression during sleep, etc.) with anatomical anomalies at the level of the supinator brevis muscle.

    Description / Table of Contents: Sommario Riportiamo i risultati del trattamento con plasmaferesi (PE) e immunoglobuline (IVIg) in 36 su 50 pazienti con sindrome di Guillain-Barré (GBS) reclutati attraverso uno studio di incidenza condotto in Emilia-Romagna. Il confronto fra PE e IVIg non ha evidenziato differenze significative per quanto concerne la capacità di migliorare il decorso della malattia. Complessivamente, nei trattati con IVIg e PE il 25% e l'11,1% rispettivamente erano guariti ad un mese, mentre il 58,3% e il 55,5% erano migliorati di almeno un grado. Questi risultati sono in accordo con le conclusioni del Dutch GBS Trial per ciò che concerne l'outcome a un mese. Nessuna ricaduta è stata osservata nei due gruppi. Inoltre, non è stata osservata nessuna differenza nell'outcome clinico a 1 e 3 mesi tra i pazienti che ricevettero una sola o due terapie. Un secondo ciclo di terapia non sembra migliorare significativamente il decorso clinico della malattia. PE e IVIg sono entrambe sicure ed efficaci nella terapia della GBS.
    Notes: Abstract We report the effects of treatment with plasma-exchange (PE) and intravenous immune globulins (IVIg) in 36 out of 50 patients with Guillain-Barré syndrome (GBS) recruited by an incidence study in the Emilia-Romagna region of Italy. Comparison of the patients treated with PE and IVIg showed no significant differences in terms of effectiveness in improving the clinical course of GBS: at one month, respectively 11.1% and 25% had recovered, and 55.5% and 58.3% had improved by at least one grade. These results are in agreement with those of the Dutch GBS trial. No relapses were observed in either group. Moreover, our results showed no difference in clinical outcome at 1 and 3 months between the patients receiving only one therapy and those receiving two; a second cycle of therapy did not seem to improve the clinical course of the disease significantly. We conclude that PE and IVIg are both safe and effective therapies for GBS.

    Notes: Abstract: To evaluate nervous system involvement in a cohort of Italian patients with primary Sjögren’s syndrome (pSS), 87 unselected patients (83 female, and four male) observed consecutively at our institution over a period of 5 years were screened by clinical and instrumental (MRI, SPECT, electrophysiological testing, CSF analysis) investigations for peripheral and central neurological abnormalities. Seroimmunological parameters and extraglandular features other than neurological manifestations were also evaluated. Seven patients had central nervous system (CNS) disease (8%), mostly non-focal dysfunction, and 12 had peripheral nervous system (PNS) disease (13.8%), mostly mild or severe sensory or sensory-motor polyneuropathies. One patient had concomitant CNS and PNS involvement. Compared with CNS disease, PNS involvement occurred in older patients (450 years), independent of the disease duration. Patients with and without neurological abnormalities did not differ for seroimmunological parameters (including antiphospholipid antibodies) or extraglandular manifestations. From a statistical point of view, the only relevant finding was the detection of a slight increase in serum IgA and IgM levels (p50.05) in patients with an intact nervous system. Neurological involvement in pSS, be it central or peripheral, is not a rare finding. A careful clinical neurological evaluation, combined with a multiplicity of instrumental investigations, is recommended in the global assessment of pSS patients.

    Notes: 2 P2O7 are usually employed for the extraction of the HS from soil and peat, little information concerning the interaction between extractants and humic acids is available. In this study, the results on the molecular structure of humic acids (HA) extracted and separated from a Sphagnum Irish peat sample with alkaline pyrophosphate solution (0.1 M NaOH + 0.1 M Na2P2O7) are reported. The HA were redissolved in 5 mM NaCl, fractionated in six cut-off fractions (nominal molecular weight: HA 〉 300, 100-300, 50-100, 20-50, 10-20, and 5-10 kDa) using a tangential ultrafiltration system. All samples were dialyzed against distilled water and then freeze-dried. Fourier-transform infrared (FT-IR) and 31P nuclear magnetic resonance (NMR) spectroscopies were applied to characterize the incorporation of pyrophosphate (PP) in both the unfractionated HA and in the six HA fractions. Spectroscopic analyses showed that it was possible to assign a given pattern of bands to define the structure of phosphate: PP was present only in the unfractionated HA, whereas orthophosphate was present in both the high molecular weight and lower molecular weight fractions. The high concentration of phosphate found in the lower molecular weight HA fractions has been attributed to the presence of metals and the oxygenated groups of HA. The changes in the relative intensity of the bands attributed to oxygenated functional groups observed in the FT-IR spectra could be assigned to the interaction of phosphate groups with metal cations, which are involved in a coordination complex with the oxygenated groups.

    Notes: A multinuclear magnetic resonance and vibrational study on 5-bromouridine and its base pairing with guanosine and adenosine in deuterated dimethyl sulfoxide at different concentrations is reported. A dicarbonylic non-self-associated form is suggested for 5-bromouridine on the basis of the 1H-, 15N-NMR, and Raman data. When guanosine is added in equimolar amounts, a downfield shift of the (N3)H proton of 5-bromouridine and of the (N1)H and NH2 protons of guanosine is observed; these results can be interpreted, according to the Raman ones, considering that only a fraction of guanosine is “wobble base paired” with 5-bromouridine, whereas the remaining part is self-associated. When 5-bromouridine is mixed with adenosine, the proton chemical shift of the aminic NH2 of adenosine increases and the (N3)H iminic of 5-bromouridine moves downfield at a value higher than that observed for the 5-bromouridine-guanosine mixture. This behavior supports the hypothesis that 5-bromouridine interacts more with adenosine than with guanosine, but the results obtained are not able to establish which type of pairing (Watson-Crick or Hoogsteen) is present. Finally, the infrared spectrum of the solid 5-bromouridine: adenosine adduct, for which X-ray measurements of other authors suggested a Hoogsteen pairing, is reported and the observed bands are discussed. © 1997 John Wiley & Sons, Inc.
