    Notes: Summary The aim of this investigation is to evaluate the accuracy of the diagnostic parameters of the radiological features, especially in conventional lumbar myelography, since these will be used in the future as a gold standard for new developing diagnostic methods like ultrasonography (US) and magnetic resonance imaging (MRI). A short review of the clinical symptoms and diagnostic radiology of the tethered spinal cord (TSC) is given. The radiological features of the TSC syndrome are compared with the peroperative findings in 30 patients investigated and operated in our hospital. All patients showed lumbar or sacral osseous malformations on the plain X-ray films. This means a high diagnostic sensitivity although the specificity for TSC is low. Conventional myelography had a sensitivity of 0.82 and positive predictive value of 0.94, while the specificity and negative predictive value are respectively 0.96 and 0.85. The positive predictive value of the total diagnostic procedure of plain X-ray films and lumbar myelography for TSC may be estimated at 1.

    Notes: Summary The radiodiagnostic signs of congenital basal encephalocele are well known. The case history described here shows that a long existing, acquired, traumatic transethmoidal encephalocele can cause the same roentgen appearance. The differences between the congenital and recent posttraumatic forms are indicated.

    Description / Table of Contents: Résumé Les auterus décrivent la technique d'angiographie vertébrale intra-opératoire dans le traitement des malformations artérioveineuses alimentées exclusivement par les artères vertébrales. Le risque d'interférence avec l'équilibre hémodynamique est plus important dans l'artère vertébrale que dans le système carotidien et les facteurs pouvant influencer cet équilibre sont discutés. Les buts de l'angiographie vertébrale peropératoire sont: la localisation de la sonde à l'intérieur de la malformation en cryochirurgie, la détection rapide de la malformation au cours de l'intervention et la vérification immédiate du résultat aprés exérèse chirurgicale de la malformation.

    Notes: Summary A description is given of the technique of vertebral angiography during operative procedures used in the treatment of arteriovenous malformations fed exclusively by the vertebral arteries. The risk of interference with the haemodynamic equilibrium is greater in the vertebral artery than in the carotid tree, and factors which can influence this equilibrium are discussed. The purpose of intraoperative vertebral angiography is: localization of the cryoprobe within the malformation in cryosurgery; rapid detection of the malformation during the operation; and immediate verification of the result after operative removal of the malformation.

    Description / Table of Contents: Zusammenfassung Die Ergebnisse dieser Untersuchung bestätigen die Hypothese, daß der einfache, harmlose Doppler-LP-Test ein nützliches Verfahren sein kann, um Information zu bekommen von dem Zusammenhang des Liquordrucks und der zerebralen Blutdurchströmung. Ferner wurde der vorhersehende Wert des Tests untersucht bezüglich des klinischen Erfolges nach Liquordrainage bei Patienten mit Hydrocephalus mit normalem Liquordruck. Es wurden vierzehn Patienten untersucht mit Hydrocephalus mit normalem Liquordruck; acht Patienten zeigten einen positiven und sechs Patienten einen negativen Doppler-LP-Test. Sieben der acht Patienten mit dem positiven Test zeigten eine deutliche Verbesserung nach der Operation, doch keiner der sechs Patienten mit dem negativen Test. Die Schlußfolgerung dieser Untersuchung könnte sein, daß der Doppler-LP-Test ein neuer und brauchbarer preoperativer Selektionstest wäre bei Patienten mit Hydrocephalus mit normalem Liquordruck. Im übrigen unterstützt dieses Studium die Hypothese, daß die Autoregulation der zerebralen Blutdurchströmung gestört sein kann bei Patienten mit Hydrocephalus mit normalem Liquordruck.
    Notes: Summary The results of this study confirm the hypothesis that the simple innocuous Doppler LP test may be useful in obtaining information about CSFP-CBF (cerebrospinal fluid pressure-cerebral blood flow) relationships. The value of this test in predicting the clinical outcome of CSF shunting in patients with normal pressure hydrocephalus (NPH) was studied. Fourteen patients with NPH were examined; eight patients showed a positive and six patients a negative Doppler LP test. Seven of the eight patients with a positive test improved following the shunting procedure, but none of the six patients with a negative test improved after operation. This study suggests that the Doppler LP test is a useful preoperative selection test in patients with NPH. In addition this study supports the hypothesis that CBF autoregulation may be impaired with NPH.
