Notes: Postoperative ulnar neuropathy as a result of mechanical trauma at the elbow was observed in eight patients undergoing renal transplantation. In five cases this occurred on arms that were adducted during the operation and in which an arteriovenous shunt was present. In four cases (one patient had the lesion bilaterally) the other, abducted arm was affected. Analysis of possible causes revealed no single factor responsible for the condition. Factors that most likely contributed were: pressure on the adducted arm by the combined weights of patient and surgeon, blood pressure monitoring with a cuff compressing the cubital fossa, venous congestion by the arteriovenous shunt, and subclinical uraemic polyneuropathy. Although no single factor could be identified protection of the adducted arms with a hard plastic cover and placement of the blood pressure cuff as proximally as possible on the abducted arms provided a successful solution to the problem.

Notes: Summary The aim of this investigation is to compare EEG power-density spectra between dyslexic and normal children. The children have been selected on the basis of specific reading and writing tasks; those with severe organic diseases and mental disorders were excluded. Groups aged 8, 9 and 10 years have been compared. In the 8-year-old group the power in the alpha band is higher in normals, in the 9-year-old group there was a higher power of the mu rhythm in normals and in the 10-year-old group the power in the temporal theta band is lower in normals. A possible relation between these findings and psychomotor development is discussed.

Notes: Summary In a group of normal children aged 8, 9 and 10 years power density spectra of a 100-second EEG period were made over four different brain regions. All children had to undergo medical and psychological examinations prior to this EEG and the criteria for normality were based on these examinations. Reference values of the power density distribution for children between 8 and 10.5 years are given.

Notes: Summary In this study is described the quantitative structure of one circumscribed area of the cerebral cortex 2 months after a series of electro-consulsions. The results indicate a persistent change in the nuclear volume of the cerebral neurons in this area. There was no loss of neurons in the cortex. The changes described are diffuse in the cerebral cortex and not restricted to particular architectonic layers.

Notes: Abstract Myophosphorylase deficiency (McArdle's syndrome) is an uncommon conditon characterized by exercise intolerance, muscle cramps and myoglobinuria. The present report describes the clinical, histochemical, electronmicroscopic and biochemical findings in a 12-year-old boy with myophosphorylase deficiency. The diagnosis should have been suspected when the boy was 6 years old. Most index cases have not been diagnosed until adult life, but this syndrome has to be considered in the differential diagnosis of exercise intolerance in childhood.

Notes: Abstract The results of needle biopsies of skeletal muscle in 95 infants (age-range 2/12-17 1/12 year) are presented. The biopsies were carried out using the Vondra-needle. In all but one case an amount of material was obtained which was sufficient for histopathological examination. The needle biopsy technique has the advantage that it can be performed by the paediatrician in the out-patient department. In our experience the procedure entails no risk.

Notes: Abstract In an add-on pilot study, a group of 15 children with cryptogenic and intractable West syndrome (3) and Lennox-Gastaut syndrome (12) received intravenous immunoglobulin (IVIg, 0.4g/kg body weight per day for 5 consecutive days, followed by the same dose once every 2 weeks for 3 months). Five patients had been treated previously with ACTH without success. The reduction in clinical seizures averaged 70%. Electroencephalographic (EEG) recordings revealed a mean reduction in epileptic discharges of 40%. In all 15 patients, acceleration of EEG background activity occurred, and psychomotor development improved. Prior to IVIg administration, CSF examinations were normal. After IVIg administration, the serum total IgG concentration increased by an average of 76%, and the CSF IgG concentration by 44%. According to our data, IVIg crosses the blood-CSF barrier, and might be effective in the treatment of West syndrome and Lennox-Gastaut syndrome. We suggest it should be considered when other treatments, such as ACTH, have failed.

Notes: Summary The results of this study confirm the hypothesis that the simple innocuous Doppler LP test may be useful in obtaining information about CSFP-CBF (cerebrospinal fluid pressure-cerebral blood flow) relationships. The value of this test in predicting the clinical outcome of CSF shunting in patients with normal pressure hydrocephalus (NPH) was studied. Fourteen patients with NPH were examined; eight patients showed a positive and six patients a negative Doppler LP test. Seven of the eight patients with a positive test improved following the shunting procedure, but none of the six patients with a negative test improved after operation. This study suggests that the Doppler LP test is a useful preoperative selection test in patients with NPH. In addition this study supports the hypothesis that CBF autoregulation may be impaired with NPH.

Notes: Summary We describe a method for obtaining cortical evoked potentials after stimulation of the lumbosacral and cervical dermatomes in healthy volunteers. Such dermatomal evoked potentials are expected to contribute to the diagnosis of lumbar and cervical root entrapment. Normative data are presented for each dermatome including left-right differences. A significant correlation was found between absolute latencies and body length for the lumbosacral segments. This correlation was virtually absent in the cervical segments. The latency difference between the same cervical or lumbosacral dermatome left and right was also independent of body length for all segments.

Notes: Summary In this survey we describe the uses of somatosensory, visual, and auditory evoked potentials (EPs), adding some critical comments on the values and pitfalls of these methods. It must be stressed that the application of EPs is most valuable when combined with a thorough neurological examination. There is general agreement that EP measurement is one of the best techniques for objective, noninvasive study of brain function in humans.
