Notes: Abstract Juvenile sardine, Sardina pilchardus Walbaum, 1792, were collected off Mallorca, Balearic Islands, during April–July 1989, and were aged by means of daily otolith increments. Growth rates were in the range of values reported for other clupeoids, although lower than those for larvae of the Mediterranean anchovy Engraulis encrasicolus. Hatching dates were back-calculated from the age in days and the date of capture. The data indicated that hatching was continuous, but revealed a low peak in the last half of November 1988 and a second, higher peak in the first half of March of the following year, in accordance with the general cycle of secondary production in this oligotrophic area.

Notes: Abstract Daily deposition of growth increments in sagittal otoliths of reared and wild Sardina pilchardus (Walbaum, 1792) larvae from hatching to complete yolk-sac absorption is reported. Increments laid down prior to hatching were observed. A distinct growth increment, ≃ 1 μm wide and located 5 to 7 μm from the focus is laid down on the day of hatching. Subsequently, narrower increments (〈1 μm) are laid down daily throughout the yolk-sac stage. Measurements were made on the maximum radius of the sagittal otoliths of two groups of larvae: 83 larvae reared from fertilized eggs sampled from Ría de Muros (NW Spain), and 239 wild individuals sampled along the northern coasts of the Iberian peninsula. Counting of daily ring increments can provide a direct and valid estimation of larval age without the necessity for later correction.

Notes: Conclusions With logistic regression the mortality of the SCAP is related with characteristics of the patient at admission to the ICU (Previous state of health, APACHE II 〉 15) and the developement of complications (MOF and Radiographic spread of pneumonia).

Notes: Abstract Objectives To review the pathophysiology, presentation and treatment of isoniazid (INH) intoxication. Data sources Human, animal and modeling studies published since 1940 identified through MEDLINE and a review of the bibliographies of relevant articles. Study selection and data extraction The studies identified were reviewed with emphasis on the most recent. Earlier studies were selected for their historical value and relevance to the clinical setting. Data synthesis Isoniazid overdose is a potentially fatal intoxication. The incidence of tuberculosis has recently increased in the United States and therefore the frequency of INH overdose may also increase. Patients with INH overdose may present with nausea, vomiting, ataxia, symptoms reminiscent of atropine intoxication, coma and grand mal seizures. Lactic acidosis is revealed by laboratory evaluation. Treatment requires admission to the ICU for ventilatory support, and management of seizures and acidbase abnormalities. Pyridoxine, in a dose equivalent to the amount of INH ingested is the only effective antidote. Conclusions INH overdose should be suspected in any patient presenting with seizures and metabolic acidosis. Prognosis is good when treatment is instituted early.

Notes: Abstract In a five-year-old girl with congenital hepatic fibrosis raised erythrocyte sedimentation rate and serum gammaglobulins were noted for 3 years. Needle liver biopsy showed histological features of cholangitis andKlebsiella oxytoca was cultured from a liver biopsy specimen. Antibiotic treatment resulted in normalization of erythrocyte sedimentation rate and serum gammaglobulins. This suggest that cholangitis can be clinically latent in congenital hepatic fibrosis and should be investigated and treated when unexplained inflammatory signs are present.

Notes: Abstract Most studies on the prevalence of asialoglycoprotein antibodies have involved adults. In this study the prevalence of antibodies to the human asialoglycoprotein receptor (ASGP-R) was determined in 63 children with auto-immune hepatitis. It was shown that 75% of those with auto-immune hepatitis type 1 and 40% of those with auto-immune hepatitis type 2 were positive and the presence of anti-ASGP-R auto-antibodies, at a mean titre of 1:1,600 and 1:1,000 respectively. No statistical significance was found between anti-ASGP-R positive and negative patients with respect to the median age at onset, proportion of females and ALT levels. However, the titre of anti-ASGP-R antibodies correlated significantly with hypergammaglobulinaemia. Conclusion Anti-asialoglycoprotein receptor antibodies are frequently present in the sera of children with auto-immune hepatitis, particularly type 1, The presence of these antibodies is an indirect marker of inflammatory activity.

Notes: Abstract 131I Rose Bengal (131IRB) studies were performed in 73 infants with extrahepatic biliary atresia (EHBA) and in 37 with intrahepatic cholestasis of various origins. Fecal 131IRB excretion of less than 10% (“complete” cholestasis) was observed in EHBA but also in some patients with either paucity of intrahepatic bile ducts (syndromatic type) or with α-1-antitrypsin deficiency. Seventy one 131IRB tests were also performed 3 to 8 weeks postoperatively in children operated on for EHBA. Fecal 131IRB excretion more than 15% was present in 27 out of 34 cases who were later completely jaundice free and in only one out of 37 cases where no bile flow restoration occurred. These results indicate that complete cholestasis in infants can be observed in some types of intrahepatic cholestasis, as well as in EHBA, and show that a postoperative 131IRB test is a reliable means of predicting complete restoration of bile flow in EHBA.

Notes: Abstract A 2-year-old boy with a Kasai portoenterostomy had systemic features suggestive of cholangitis and acute portal vein obstruction. The rapid rise in portal pressure caused a transient, severe decompensation of hepatic function. A superior mesenteric arterial angiogram confirmed the presence of diffuse intrahepatic portal vein thrombosis. Conclusion This case provides clinical and radiological evidence supporting an association between ascending cholangitis and acute intrahepatic portal vein thrombosis.
