Résumé Le volume de mésangium et le nombre des cellules glomérulaires de 30 reins de diabétiques coupés semi-finement ont été quantitativement analysés. Les résultats ont été examinés en relation: avec l'importance du pouvoir de liaison de l'insuline dans le sérum; avec le niveau du cholestérol dans ce même sérum; avec l'insuffisance rénale et avec la thérapeutique. Nous avons pu constater une augmentation considérable du volume du mésangium et ce, jusqu' au double de la normale. Nous avons observé également une véritable prolifération des cellules mésangiales et endothéliales, tandis que les cellules épithéliales diminuaient à cause de l'adaptation à la structure glomérulaire simplifiée. La prolifération s'éléve à 47% par rapport aux témoins normaux. L'augmentation du volume du mesangium et du contenu cellulaire mésangial est caractérisée par une corrélation positive avec le titre des anticorps antiinsuline. Une corrélation du volume du mésangium et du nombre des cellules mésangiales avec le niveau du cholestérol dans le sérum n'a pas pu être déterminée statistiquement. Après une longue thérapeutique avec l'insuline on trouva des volumes mésangiaux plus élevés qu'avec une médication sporadiqjue par l'insuline ou qu'avec des antidiabétiques par voie orale. Nous voyons dans la sclérose glomérulaire diabétique — grâce aux constatations quantitatives — une apparition de la maladie glomérulaire caractérisée par l'augmentation du volume du mésangium et par une véritable prolifération cellulaire, apparition qui ne se différencie pas dans sa caractéristique morphométrique d'une glomérulonéphrite intercapillaire proliférative. Il se peut qu'un processus de l'immunité dans le mésangium, occasionné par le complexe insuline-anti-insuline, soit le motif de cette variation.

Zusammenfassung In 30 Nieren von Diabetikern werden das Mesangiumvolumen und die Zahl der einzelnen glomerulären Zellen quantitativ an Semidünnschnitten bestimmt. Die Ergebnisse werden auf ihre Beziehungen zur Insulinbindungskapazität des Serums, zum Serumcholesterinspiegel, zur Niereninsuffizienz und zur Therapie untersucht. Wir können eine erhebliche Mesangiumvolumenzunahme bis auf das Doppelte gegenüber der Norm feststellen. Weiterhin wird eine echte Proliferation der Mesangium- und Endothelzellen beobachtet, während die Deckzellen als Folge der Anpassung an die vereinfachte glomeruläre Struktur abnehmen. Die Gesamtzellproliferation beträgt im Mittel 47% gegenüber den normalen Kontrollen. Die Vermehrung des Mesangiumvolumens und der Mesangiumzellen ist gekennzeichnet durch eine positive Korrelation zum Insulinantikörpertiter. Eine Abhängigkeit des Mesangiumvolumens oder des prozentualen Mesangiumzellgehaltes vom Serumcholesterinspiegel läßt sich statistisch nicht sichern. Bei Dauertherapie mit Insulin finden sich im Mittel höhere Mesangiumvolumina als bei sporadischer Insulinmedikation oder Anwendung von oralen Antidiabetika. Wir sehen in der diabetischen Glomerulosklerose aufgrund der quantitativen Befunde eine durch Mesangiumvolumenzunahme und echte zelluläre Proliferation gekennzeichnete glomeruläre Erkrankung, die sich in ihren quantitativ morphologischen Charakteristika nicht von einer interkapillären proliferativen Glomerulonephritis unterscheidet. Es ist möglich, daß ein Immunprozeß im Mesangium, ausgelöst durch Insulin-Antiinsulin-Komplexe, die Ursache dieser Erkrankung ist.

Summary We have determined quantitatively the mesangial volume and the number of glomerular cells in 30 kidneys of diabetics by means of semi-thin sections. The results were explored with respect to the insulin-binding capacity of the serum, the cholesterol level, chronic renal failure and the therapy. We detected a considerable increase in the mesangial volume, up to twice that of the standard. We also observed a true proliferation of the mesangial and endothelial cells. The decrease in epithelial cells was caused by the adaptation to the simplified glomerular structure. The proliferation was on an average 47% compared with normal controls. The increase of the mesangial volume and the mesangial cells is characterized by a positive correlation to the titre of insulin antibodies Correlations of mesangial volume and of the number of mesangial cells to the level of serum cholesterol could not be fixed statistically. After long-term therapy with insulin we found higher mesangial volumes than after sporadic insulin therapy or application of oral antidiabetic agents. We see in the diabetic glomerulosclerosis a glomerular disease characterized by an increase of the mesangium and by a true cell proliferation. In the description of the morphometric features it does not differ from an intercapillary proliferative glomerulonephritis. It is possible that these changes are caused by an immune-process in the mesangium produced by insulin-anti-insulin complexes.
