    Notes: The neuropeptide oxytocin has long been known as a potent contractor of the uterus. However, it has remained difficult to attribute a definite role for neurohypophysial oxytocin in either the initiation or continuation of labour (1). Most recently, Lefebvre and colleagues (2) have suggested that oxytocin produced in the uterus, rather than in the hypothalamus, may be more important in parturition since at term the uterus of the rat contains 70-fold more mRNA for oxytocin than the hypothalamus, and this disappears at about the time of parturition. Despite the high levels of mRNA the uterus contains only nanogram quantities of immunoreactive oxytocin per gram wet weight at term (2), compared to microgram quantities present in the pituitary (3,4). Here we show that activation of the neurohypophysial oxytocin system occurs, as reflected by expression of immunoreactivity for Fos in the hypothalamic supraoptic nucleus, and that this activation is indeed critical for normal parturition, since its inhibition results in a significant prolongation of parturition. In addition, we present evidence that pulsatile delivery of oxytocin into the circulation is important for the efficient progress of parturition, indicating that a major role of the neuronal circuits regulating oxytocin secretion for parturition, as is already known for suckling, is to produce an appropriately patterned hormonal output for efficient biological action.

    Description / Table of Contents: Summary The concept of a posttraumatic stress disorder (PTSD) has become important for psychiatric diagnosis and research, particularly following US-american studies on Vietnam war veterans. Recently, studies on traindrivers involved in suicide accidents have shown symptoms of anxiety and depression in more than 30% of concerned drivers and typical symptoms of a PTSD in 15%. Here, we present the case of a traindriver who was involved in 6 suicide accidents within 17 years of traindriving (the average is 2 suicide accidents). Following the 3rd accident, the driver developed increasingly symptoms like anxiety, sleep disturbances, flashbacks and irritability. Following the 6th accident, he has been unable to work. A former expert opinion saw no relationship between the symptoms of the driver and the suicide accidents. Therefore, further information about the concept of a PTSD seems necessary to ensure early psychotherapeutic and pharmacological treatment of patients with such a disorder.
    Notes: Zusammenfassung Das Konzept der Posttraumatischen Belastungsstörung (PTSD) erlangte v.a. durch US-amerikanische Studien an Vietnamkriegsveteranen Bedeutung für die psychiatrische Diagnostik und Forschung. Neuere Studien an Zugführern, die in Suizidunfälle verwickelt waren, zeigten ängstlich-depressive Symptome bei mehr als 30% und typische Symptome einer posttraumatischen Belastungsstörung bei 15% der Betroffenen. Wir präsentieren die Kasuistik eines Lokomotivführers, der im Rahmen seiner Tätigkeit 6 Suizidunfälle innerhalb von 17 Jahren miterlebt hatte. Der Durchschnitt liegt bei 2 Suizidunfällen. Mit dem 3. Unfall entwickelte der Proband zunehmende Ängstlichkeit, Schlafstörungen, Flashbacks und Reizbarkeit. Seit dem 6. Unfall ist er arbeitsunfähig. In einer früheren gutachterlichen Stellungnahme wurde kein Zusammenhang zwischen den Symptomen des Probanden und den Suizidunfällen gesehen. Daher scheint eine weitere Aufklärung über das Krankheitsbild einer PTSD notwendig, um eine frühzeitige psychotherapeutische und pharmakologische Behandlung von Patienten mit einer solchen Erkrankung sicherzustellen.

    Description / Table of Contents: Summary With the introduction of the non-benzodiazepine hypnotics zopiclone and zolpidem it was expected to have hypnotics without side effects and risks characteristically seen with benzodiazepines. We report two cases with high-dose usage and dependency of non-benzodiazepine hypnotics. Both patients were prescribed the drugs to treat sleep disturbances occuring during a depressive episode. While one patient had a polysubstance abuse there was no evidence for an abuse history in the other patient. To reduce withdrawal symptoms long-acting benzodiazepines were given to both patients. Thus, it seems that not only patients with a history of substance abuse but also patients with a psychiatric disorder are at risk for abuse of non-benzodiazepine hypnotics. Considering the increasing number of case reports with abuse and dependence of zopiclone and zolpidem it seems necessary to reevaluate the dependency risk of the currently available non-benzodiazepine hypnotics.
    Notes: Zusammenfassung Mit der Einführung der „Non-Benzodiazepinhypnotika” Zolpidem und Zopiclon war die Hoffnung verbunden, Hypnotika ohne benzodiazepintypische Nebenwirkungen und Gefahren zur Verfügung zu haben. Wir stellen 2 Kasuistiken mit hochdosierter Einnahme von und Abhängigkeitsentwicklung unter „Non-Benzodiazepinhypnotika” vor. Bei beiden Patienten begann die Substanzeinnahme aufgrund von Schlafstörungen i.R. eines depressiven Syndroms. Während bei dem einen Patient eine Polytoxikomanie bestand, fanden sich bei dem anderen keine Hinweise für einen Mißbrauch anderer psychoaktiver Substanzen oder einen Mißbrauch in der Vorgeschichte. Zur Abmilderung der Absetzsymtomatik erfolgte eine Umstellung auf langwirksame Benzodiazepine und eine stufenweise Reduktion derselben. Neben Patienten mit Mißbrauch oder Abhängigkeit in der Vorgeschichte scheinen auch Patienten mit depressiven Störungen gefährdet zu sein, Zopiclon oder Zolpidem mißbräuchlich einzunehmen. Das potentielle Abhängigkeitsrisiko der „Non-Benzodiazepinhypnotika” muß daher auch in der klinischen Praxis beachtet werden.
