    Beschreibung / Inhaltsverzeichnis: Zusammenfassung Bei einer 20jährigen Patientin wurde 3 Wochen nach einer intraglutealen Injektion von Tetanus-Toxoid und 6–7 Wochen nach einem Polytrauma eine Lähmung mehrerer Muskeln des rechten Armes beobachtet. Die EMG-Befunde zeigten vorübergehend eine Leitungsblockade der Nerven zum M. deltoides und pectoralis und eine schwerere, axonale und Markscheidenläsion der Fasern zum M. biceps. Die Ursache der Lähmung ist möglicherweise in der Applikation des Tetanus-Toxoid zu sehen, andere mögliche Ursachen werden diskutiert.
    Notizen: Summary A reversible paresis of several muscles of the right arm was observed in a 20-year-old female, 3 weeks after the intragluteal injection of tetanus toxoid and 6–7 weeks after multiple injuries. The EMG findings indicated a transitory conduction block of nerve fibers to the deltoid and pectoralis muscles and a more severe axonal-demyelinating lesion of fibers to the biceps muscle. The cause of the palsy was probably a reaction to the administration of tetanus toxoid; other possible causes are discussed.

    Beschreibung / Inhaltsverzeichnis: Zusammenfassung Untersucht wurden an 3 Kontrollpersonen und an 6 Patienten mit genuiner Epilepsie vom Aufwachtyp über einen Zeitraum von 72 h die Herzfrequenz, die Körpertemperatur sowie die Ausscheidung der Elektrolyte Calcium, Kalium und Natrium im Urin. Für alle Patienten, Aufwachepileptiker und Kontrollpersonen, bestanden auf der Station konstante Bedingungen, sie erhielten eine Standarddiät mit konstanten Werten für Calcium, Kalium und Natrium, mit der 2 Tage vor Beginn der eigentlichen Meßreihe begonnen wurde. Während der Untersuchungszeit wurden keinerlei Medikamente eingenommen. Bestimmt wurden anhand der Originaldaten die Zeiten der Kurvenwendepunkte und durch die Power-Spektralanalyse die Periodendauern der einzelnen gemessenen Parameter. Bei den Aufwachepileptikern ergaben sich inkonstantere Befunde hinsichtlich der Lage der Maxima und Minima und auch der 24 h-Periodik, ohne daß jedoch eine konstante Abweichung in eine Richtung, d. h. entweder eine Vorverschiebung oder eine Verspätung in der Phasenlage, vorlag und auch ohne konstante Veränderung in den Periodendauern aller gemessenen Größen bei ein und demselben Aufwachepileptiker. Zumindest hinsichtlich der hier unter den genannten Bedingungen gemessenen Parameter fand sich kein sicherer Unterschied zwischen den Aufwachepileptikern und den Kontrollpersonen, der als spezifisches, allen Aufwachepileptikern gemeinsames Merkmal einer gestörten circadianen Rhythmik gelten könnte.
    Notizen: Summary In 6 patients with idiopathic epilepsy of the awakening type and 3 control subjects, the heart rate, body temperature and urinary excretion of sodium, potassium and calcium were measured over 72 h. The patients and the control subjects stayed in the hospital under constant environmental conditions including a standard diet with a constant content of sodium, potassium and calcium. The diet began 2 days before the onset of data collection. No drugs were given during the whole period. The data obtained during the 72-hour period were scored visually concerning the position of the maxima and minima, whereas the period duration was calculated by means of power spectral analysis. The epileptic patients showed more inconstant results concerning the time of maxima and minima as well as the 24-hour periodicity. However, a constant deviation i.e. a constant phase shift or a constant change in period duration into the same direction could not be observed. The different parameters showed different behavior even in the same patient. Thus a constant difference in the circadian periodicity between normal subjects and patients with epilepsy of the awakening type could not be found in the data recorded in the present study.

    Notizen: Summary In unrestrained cats with, chronically implanted electrodes reflex discharges evoked by afferent sciatic nerve stimulation have been recorded from the cervical sympathetic nerve during wakefulness and during the two states of sleep. The following results were obtained: 1. A single peripheral stimulus caused a mass discharge in the sympathetic nerve with three deflections due to different conduction velocities of three fibre groups (group I, II, and III). 2. The amplitude of the potentials of all fibre groups were progressively reduced during synchronized sleep with respect to wakefulness. During REM sleep a further reduction was observed. The latencies of group II fibres had a longer duration during synchronized sleep compared to that seen in REM sleep. For group III the shortest latencies were seen during synchronized sleep. 3. The height of the potentials during the REM state of sleep depended on the temporal relation to the burst of rapid eye movements. The amplitudes of the potentials of fibre groups II and III were largest when the stimulus was applied within 6 sec before the onset of a burst of rapid eye movements. It might be concluded from the present study that the sympathetic reflex excitability is not only homogeneously reduced with increasing depth of sleep but beyond it, different fibre systems are specifically influenced by the sleep wakefulness cycle.

    Notizen: Summary Evoked potentials in the efferent vagal and sympathetic nerves following reticular stimulation were studied during wakefulness and sleep in unrestrained cats with chronically implanted electrodes. The following results were obtained: A single stimulus within the mesencephalic reticular formation caused an evoked response in both the sympathetic and the vagal nerve. The sympathetic evoked potential consisted of three peaks due to different conduction velocities of three fibre groups. The amplitudes of the three peaks are reduced during synchronized sleep with respect to wakefulness. A further reduction was seen in REM-sleep. The latency of the potential of fibre group II had a longer duration during synchronized sleep than during REM state. In most of the cases the evoked response in the vagal nerve consisted of one peak only. The amplitudes of the potentials showed an increase during synchronized sleep while they were almost identical during wakefulness and REM-sleep. The shortest latencies were seen during synchronized sleep. The results show that the excitability of the central sympathetic neurons is progressively reduced with increasing depth of sleep. The excitability of the efferent vagal neurons is markedly increased during synchronized sleep. In other words, the synchronized state of sleep is the most vagotonic phase of sleep; the lowest sympathetic activity is seen during REM-sleep.

    Notizen: Summary Experiments were performed on 45 cats. One group of animals was operated upon utilizing ether anesthesia which was discontinued after brain stem transsection. During the following experimental procedure the animals were immobilized with Flaxedil and artificially respired. Another series of experiments was carried out under Pentobarbital Sodium anesthesia (Nembutal). The brain stem was successively transsected according to the Horsley-Clarke coordinates A 2 (section I), P 2 (section II), and P 6 (section III). These sections are located rostrally to the rhombencephalic vasomotor center. Blood pressure elevations were produced by intravenous injection of adrenaline. The height of blood pressure did not change if mesencephalic structures were removed by the three sections. The duration of the adrenaline-induced rise in blood pressure was significantly prolonged after successive removal of the mesencephalic brain stem structures. This effect was abolished under Nembutal anesthesia. In this case the duration of the adrenaline-induced rise in blood pressure was found to be identical with the response to transsection in position III in unanesthetized cats. The increased duration of the adrenaline-induced blood pressure elevation after successive transsection of mesencephalic structures can still be observed after denervation of the carotid sinus bilaterally. Even though the differences observed were less pronounced, they remained statistically significant. Comparable results were obtained when both carotid sinus and depressor nerves were interrupted. If all buffer nerves including both vagi were cut, the duration of the rise in blood pressure continued to increase with each step of the mesencephalic transsection. In another series of experiments the rhombencephalic vasomotor center was isolated by deafferentiation from all buffer nerves and separated from higher brain stem structures by section III. The duration of an adrenaline-induced rise in blood pressure under these circumstances was not significantly longer than that observed after a subsequently performed spinal section at C1. In conclusion it can be said that mesencephalic brain stem structures are involved in the phasic regulation of blood pressure. The results of the experiments suggest that the baroreceptor reflex arc sends collaterals to the mesencephalon. Since a less pronounced mesencephalic influence on blood pressure regulation can still be observed after deafferentiation from all buffer nerves, the conclusion is reached that additional pressosensitive structures within the mesencephalon participate in the regulation of blood pressure. Application of barbiturates induces complete blockage of the described mesencephalic influence on blood pressure regulation.
