    Description / Table of Contents: Summary Two healthy young males on a constant normal sodium diet (135 mM/day) were infused for 132 h with 3 ng/kg/min of angiotensin II. Plasma angiotensin II levels were thereby raised to the range of moderate sodium depletion. Plasma aldosterone and the urinary excretion rate of aldosterone-18-glucuronide were markedly increased during the whole infusion period and returned to control levels after the infusion was stopped. A slight tendency of aldosterone secretion to decrease towards the end of infusion was probably due to sodium retention (appr. 200 mM and 350 mM respectively) and to a fall in plasma potassium by approximately 0.5 mM/l. Plasma aldosterone during infusion, maintained circadian variations similar to those of cortisol. Plasma cortisol patterns were unaffected by angiotensin II. Blood pressure increased gradually during angiotensin II infusion, reflecting changes in sodium balance. The results, differing from those of studies in dog and sheep, support the assumption that angiotensin II is an important regulator of aldosterone secretion in man rather than a merely permissive factor.
    Notes: Zusammenfassung Zwei gesunde junge Männer, die eine normal gesalzene Diät aßen (135 mM/Tag), erhielten für 132 h eine Infusion mit 3 ng/kg/min Angiotensin II. Die Plasma-Angiotensin II-Konzentration stieg dadurch in einen Bereich wie bei mäßiger Natriumverarmung. — Die Plasma-Aldosteron-Konzentration und die Ausscheidung von Aldosteron-18-Glucuronid waren während der gesamten Infusionsperiode deutlich erhöht. Ein leichter Abfall beider Meßgrößen in der zweiten Hälfte der Infusionsperiode ist wahrscheinlich durch Natriumretention und Abfall von Plasma-(K+) bedingt. Der circadiane Rhythmus des Plasma-Aldosterons und -Cortisols blieb während der Infusion erhalten. — Der Blutdruck stieg mit zunehmender Natrium-Retention von zunächst +8 auf + 15−20 mm Hg an. Die Ergebnisse stehen partiell im Widerspruch zu Befunden anderer Autoren an Hunden und Schafen. Sie bestätigen die Annahme, daß Angiotensin II beim Menschen die Aldosteronsekretion chronisch stimulieren kann und nicht nur eine „permissive“ Funktion hat.

    Notes: Summary Acute leukaemia was complicated by pneumonia in 38 (34.8%) of 109 patients treated between 1979 and 1983; in 39.5% of the patients pneumonia occurred more than once. In 23 patients (60.5%) pneumonia occurred during cytostatic therapy, and 25 patients (65.8%) had less than 1000 mm2 granulocytes. Antibiotic therapy had no or only little effect in 70%. A total of 21 patients (55.3%) died of pneumonia. In 15 patients a direct relationship could be seen between pneumonia and the bacterial spectrum in the sputum. A prevalence of gram-negative bacteria was found (24 of 40 bacteria isolated, especially Enterobacteriaceae (19). Fungi were cultivated in 10 cases. Each of the typical pneumonia bacteria was only seen once respectively. It is most important that therapy begin immediately, even before the bacteria have been identified. Only then is there hope that the survival time of patients with acute leukaemia can be influenced.

    Description / Table of Contents: Summary Case-report of a girl with Debré-de-Toni-Fanconi-Syndrome and glycogenosis with clinical and biochemical data. Genetic transmission similar to primary tubulopathia could be deduced from family screening. The chief symptoms in this patient, who could be observed from the age of 6 months to 5 years, were dwarfsim, hepatomegaly, vitamin D resistant rickets and glucoamino-phosphate diabetes with polyuria and polydipsia. Furthermore hypoglycaemia with glycolability, compensated acidosis, transitory hyperlipidaemia, hypophosphataemia and hypokalaemia could be observed. The extent of the proximal tubular defect was determined by clearance and titration measurements. The glucosuria was caused by a diabetes renalis type B. The phosphate and glucose titration curves were similar. Measurement of tubular reabsorption proved that the unspecific hyperaminoaciduria was purely renal. Tolerance tests demonstrated glucose and galactose utilisation defects, whilst erythrocyte galactose turnover was found to be normal. The treatment consisted in vitamin D administration, phosphate and potassium substitution and in a lowfat, high-protein and carbohydrate diet. We have been able to achieve a normal growth rate during the last 2 1/2 years with these measures.
    Notes: Zusammenfassung Es wird über eingehende klinische und biochemische Untersuchungen bei einer Patientin mit Debré-de-Toni-Fanconi-Syndrom und Glykogenose berichtet. Familienuntersuchungen lassen einen ähnlichen Vererbungsmodus wie bei primären Tubulopathien vermuten. Klinisch im Vordergrund bei der Patientin, die wir vom 6. Lebensmonat bis zum Ende des 5. Lebensjahres beobachten konnten, steht ein erheblicher Minderwuchs, eine Hepatomegalie, eine Vitamin D-resistente Rachitis und ein Gluco-Amino-Phosphatdiabetes mit Polyurie und Polydipsie. Daneben läßt sich eine Hypoglykämie mit Glykolabilität, eine kompensierte Acidose, eine passagere Hyperlipidämie, eine Hypophosphatämie und eine Hypokaliämie nachweisen. Durch Clearance-Untersuchungen und Titrationskurven konnte das Ausmaß des renalen proximaltubulären Defektes bestimmt werden. Bei der Glucourie handelt es sich um einen Diabetes renalis Typ B. Die Phosphattitrationskurve zeigt dasselbe Verhalten wie die Glucosetitration. Die Bestimmung der tubulären Rückresorption zeigt, daß die Hyperaminoacidurie ausschließlich renalen Ursprungs ist und kein besonderes Muster aufweist. Belastungsversuche ergeben eine Verwertungsstörung für Glucose und Galaktose. Der Galaktoseumsatz in den Erythrocyten ist hingegen normal. Die Behandlung wurde mit Vitamin D, Phosphatund Kalium-Substitution sowie häufigen fettarmen-, kohlenhydrat- und eiweißreichen Mahlzeiten durchgeführt. Seit 2 1/2 Jahren beobachten wir eine normale Wachstumsrate unter dieser Behandlung.

    Notes: Abstract Hydrobia ulvae specimens collected from 16 stations along the German North Sea and Baltic coasts exhibited imposex (occurrence of male parts in addition to the female genital system). For the purposes of comparison, a description of both the male and unaffected female genital systems is presented. Four different imposex stages with two types of development were identified and documented with scanning electron micrographs for the first time. The percentage of imposex-affected females (an average over all the localities sampled) was about 44.3%, and 12.9% were definitively sterilized. The phenomenon of sex change was not observed. The vas deferens sequence index, imposex incidence, percentage of sterilized females and the average female penis length are recommended as biological effect monitoring parameters in response to tributyltin pollution.
