    Notes: Abstract The aim of this work was the establishment of a novel method to determine the metabolic load on host-cell metabolism resulting from recombinant protein production in Escherichia coli. This tool can be used to develop strategies to optimise recombinant fermentation processes through adjustment of recombinant-protein expression to the biosynthetic capacity of the host-cell. The signal molecule of the stringent-response network, guanosine tetraphosphate (ppGpp), and its precursor nucleotides were selected for the estimation of the metabolic load relating to recombinant-protein production. An improved analytical method for the quantification of nucleotides by ion-pair, high-performance liquid chromatography was established. The host-cell response upon overexpression of recombinant protein in fed-batch fermentations was investigated using the production of human superoxide dismutase (rhSOD) as a model system. E. coli strains with different recombinant systems (the T7 and pKK promoter system) exerting different loads on host-cell metabolism were analysed with regard to intracellular nucleotide concentration, rate of product formation and plasmid copy number.

    Description / Table of Contents: Zusammenfassung Ein 49-jähriger Busfahrer stirbt 6 Tage nach schweren Nackenschmerzen im Vollbild des Wallenberg-Syndroms. Obwohl der neurologische Befund einen Stammhirnprozeß nahelegte, gelang der definitive Ultraschall- und Röntgennachweis zweifelsfrei erst nach mehreren Tagen, als der Infarkt und der Verschluß der A. basilaris komplett waren. Hätte sich der Patient in einem früheren Stadium ohne neurologische Notfallsymptome vorgestellt, wäre womöglich eine chirotherapeutische Behandlung erfolgt, und das Wallenberg-Syndrom wäre fälschlicherweise unzutreffend der Chirotherapie angelastet worden. Um diesen Fall zu bewerten und einzuordnen, wurde eine Literaturrecherche im Internet durchgeführt. Nur 30 Prozent aller Wallenberg-Syndrome stellten sich als Folge minimaler Nackenverletzungen dar, davon nur eine kleine Zahl als chirotherapeutische Eingriffe. Die große Mehrheit sind spontane Ereignisse. Daraus läßt sich ableiten, daß der Chirotherapie der Kopfgelenke eine gründliche neurologische Prüfung voranzustellen ist. Leitsymptome des Wallenberg-Syndroms sind einseitige Nackenschmerzen verbunden mit Hirnnervenausfällen, Ataxie und Hypästhesien.
    Notes: Summary The case of a 49-year-old busdriver is referred, who died 6 days after heavy neck-ache in course of a Wallenberg's syndrome. Although neurological examination gave all the signs of a brain-stem-vessel-disease, the definite verification by ultrasound and x-rays (CT and MRI) took several days and was not out of doubts until the infarction and the occlusion of the basilar artery were complete. In an earlier state, before the neurological aspects showed acute emergency, this man might have been treated chirotherapeutically. If this had occurred, the chirotherapy might have made due for the Wallenberg syndrome without reason. To compare and asses this case, an Internet-Search was performed. Most Wallenberg's syndromes are related as spontaneous events. Only about 30 percent follow minimal neck-injuries. Only a small account of those proved as chiropractic treatments. Conclusive advise is to determine brain stem infarctions by a good neurological examination before chirotherapy. Leading symptoms of the Wallenberg's syndrome are unilateral neck-ache combined with the loss of brain-stem-nerve functions, ataxia and hypesthesia.

    Notes: Zusammenfassung Um die Frage zu beantworten, ob man im Zusammenhang mit Mehoden der Kopfgelenkmanipulation von “weichen” und von “harten” Techniken sprechen kann, wurden 3 Methoden im Verlauf einen Quartals miteinander verglichen. Alle drei Methoden zeigten erfolgreiche Ergebnisse. Ernsthafte Komplikationen traten nicht auf. Unterschiede zeigten sich in der Behandlungszeit, bei der das “capital base release” mit 10–20 Minuten deutlich mehr Aufwand forderte als die “Meistertechnik” und die “Traktion” mit maximal 5 Minuten. Dies schlägt auch in einer vermehrten Ermüdung des Therapeuten zu Buche. Physikalisch-mechanische Berechnungen über die Kräfte, die bei den Techniken zum Einsatz kommen, konnten zeigen, daß der weitaus größere Energieaufwand in die Halte- und Einstellarbeit geht, während der eigentliche Manipulationsimpuls gering ist und weit unter der physiologischen Belastung bleibt.

    Description / Table of Contents: Zusammenfassung Die Diagnose und Therapie von HWS-Beschleunigungsverletzung besteht in wesentlichen Teilen aus Techniken der Manuellen Medizin. In der Regel erfolgt die Behandlung ambulant und wird vom Hausarzt geleitet. Unterschiedliche Symptomkomplexe erlauben zwischen Kopfgelenks- und zervikothorakalen Übergangstraumata zu unterscheiden. In den meisten Fällen werden die Verletzungen im Anschluß an Autounfälle bei niedriger Geschwindigkeit durch Scherkräften auf die Facettengelenke ausgelöst. Die frühe Mobilisation und Rehabilitation hat sich der Langzeitversorgung mit Zervikal-Orthesen als überlegen erwiesen.
    Notes: Summary Important parts of the diagnosis and therapy of whiplash-associated injuries of the cervical spine belong to chiropractic and manual medicine. The treatment is carried out on an outpatient basis in consultation with physicians. Different symptoms allow injuries of the spine next to the cranial border to be distinguished from the spine next to the thoracic border. In most cases, the injuries result from violent horizontal distractions of the cervical junctions, following low-speed car accidents. Early mobilization and rehabilitation show better results than long-term fixation by cervical ortheses.
