    Description / Table of Contents: Zusammenfassung Placenten von Schwangerschaften, die durch eine Rhesus-Inkompatibilität kompliziert sind, weisen regelmäßig einen schweren villösen Reifungs-Arrest auf. 14 Placenten von Schwangerschaften mit einer manifesten Rhesus-Inkompatibilität werden im Vergleich mit 10 „normalen“ Placenten morphometrisch untersucht. Während beide Kollektive eine ähnliche Größe der Trophoblast-Oberfläche aufweisen, ist die synzytio-kapilläre bzw. -sinusoidale Austauschfläche bei Rhesus-Placenten um fast zwei Drittel kleiner als in normal ausgereiften Placenten, die intravillöse Vascularisation ist um ca. drei Viertel geringer. Die funktionelle Bedeutung für die placentaren Austausch-Vorgänge wird erläutert. Durch die Korrelation der morphometrischen Ergebnisse mit dem Kinds-Schicksal wird gefolgert, daß die absolute Placenta-Insuffizienz, die mit perinatalem Fruchttod vergesellschaftet ist, in folgender Größenordnung anzunehmen ist: Oberfläche der Stoffwechsel-Membranen unter 3,6 m2, intravillöses Blutvolumen unter 45 ml, intravillöser Vascularisations-Grad geringer als 0,1. Die Gültigkeit dieser Werte wird für placentare Ausreifungsstörungen auch aus anderer Ursache angenommen.
    Notes: Summary Placentae from pregnancies complicated by Rhesus-incompatibility regularly show a severe arrest of maturation. 14 placentae from pregnancies with manifest Rhesus-incompatibility are morphometrically studied and compared with 10 normal placentae. While both groups show a trophoblast surface of similar size, the syncytiocapillary, respectively—sinusoidal metabolic exchange surface in Rhesus-placentae is about 2/3 smaller than in normal, mature placentae. The intravillous vascularisation is about 3/4 lower. The functional importance for the placental exchange processes is explained. By correlating the morphometrical results to the newborns' fate, it is concluded that the absolute placental insufficiency—always combined with perinatal death—must be assumed in the following approximate range: Surface of membranes for metabolic exchange below 3.6 sqm, intravillous blood-volume below 45 ml and intravillous vascularisation—grade below 0.1. The validity of these figures is assumed also for disturbances of placental maturation for other reasons.

    Notes: Summary We assessed the value of preoperative chest x-rays in gynecological patients. Abnormalities were noted in the chest x-rays of 10% of 1175 patients with genital or breast disorders. When the chest x-ray was abnormal, 48% of patients had no clinical features of intrathoracic disease. Abnormal chest x-ray results did not seem to have a major influence on the decision to operate or on the type of anesthesia used. In our series, probably only one patient would have received inappropriate treatment in the absence of a chest x-ray. We conclude that preoperative chest x-rays are of value in ruling out metastases in patients with a suspected neoplasm, but have little value in other circumstances.

    Description / Table of Contents: Zusammenfassung Zur Untersuchung des Einflusses der plazentaren Reife auf das fetale Wachstum eignen sich Mehrlingsplazenten in besonderer Weise, da man hier die extraplazentaren Faktoren für beide Kinder in etwa gleichsetzen kann und Unterschiede im Kindszustand weitgehend den plazentaren Funktionsbedingungen zuzuschreiben sind. Untersucht wurden Plazenten von 46 Zwillingspaaren, deren histologische Präparate von 1968–1974 an der Universitäts-Frauenklinik Düsseldorf gesammelt wurden. Bei 21 Zwillingspaaren fanden sich unterschiedlich ausgereifte Plazentaanteile. Die Kinder, die der reiferen Plazenta zugeordnet waren, zeigten im Vergleich mit dem anderen Zwillingskind im Durchschnitt bessere Werte bei Gewicht, Länge und Apgar. Nur in 3 Fällen wiesen die Kinder der reiferen Plazenta einen schlechteren Apgar-Wert auf. Hierbei handelte es sich jeweils um das zweitgeborene Kind. Bei 25 Zwillingspaaren fanden sich gleichreife Plazenten. In diesen Fällen wiesen auch die Früchte nur geringe Unterschiede auf; Differenzen im Apgar-score mußten extraplazentare Ursachen zugeschrieben werden. Bemerkenswert ist, daß von 86 untersuchten Plazentaanteilen, bei denen eine Zuordnung zur Schwangerschaftsdauer möglich war, 31mal eine Maturitas praecox placentae und 26mal eine reife Plazenta gefunden wurde, d. h. in 68% war ein Plazentaanteil ausgereift. Von den insgesamt 28 frühgeborenen Zwillingspaaren zeigte 24mal mindestens einer der beiden Plazenten eine Maturitas praecox. Nur bei 4 Frühgeborenen fehlte die vorzeitige Ausreifung eines Plazentaanteiles. Die Bedeutung der häufig nachgewiesenen vorzeitigen Ausreifung der Plazenta auf die Entwicklung und den Kindszustand post partum sowie auf die häufig gefundene Frühgeburtlichkeit wird diskutiert.
    Notes: Summary Multiple pregnancy placentas are especially efficient for examining the influence of placental maturity on fetal growth; extraplacental factors can be considered the same, and difference concerning the state of the child can be correlated with the conditions of the placental function. In 46 twin-couples placentas have been examined. In 21 cases placental parts varied in maturity. Children whose placentas had reached a better stage of maturity, were better in weight, length and Apgar score. Only in 3 cases with ripe placenta Apgar score was low (all cases were second-born children). In 25 twin-couples placentas of equal maturity were found, and the children showed insignificant differences. Differences concerning the Apgar score, could be caused by extraplacental etiological factors. 68 placental parts were correlated to the gestational age; a precocious placenta maturity was found in 31 cases, and a mature placenta in 26 cases. The importance of precocious placental maturity and its influence on the development and condition of the child post partum are being thoroughly discussed.
