    Notes: Summary It is generally accepted that the main action of glomerular prostanoids (GPs) on glomerular filtration rate (GFR) is to modulate the activity of different vasoconstrictors, specially in states of renal hypoperfusion. However it was also suggested that GPs may directly affect GFR. The present study was focused on this last hypothesis, in different experimental models, in rats. In adriamycin induced acute renal failure, the transient decrease of GFR is associated with higher levels of thromboxane B2. Later on, when GFR returns to normal, vasodilator prostaglandins synthesis was also increased. In captopril induced renal failure in Na depleted rats (where GPs synthesis remained normal), stimulation of PGE2 and PGI2 production by K and NaCl was associated with a significant improvement of GFR. Furthermore, the increase in GFR induce by NaCl was prevented by inhibition of prostaglandin synthesis. Infusion of atrial natriuretic peptide in euvolemic rats induce a marked elevation both of GFR and PGE2 synthesis. It was abolished by previous administration of prostaglandin synthesis inhibitor. In conclusion, glomerular prostanoids may influence GFR, either directly, or as mediator or modulator of other vasoactive hormones.

    Notes: Summary The modes of action of 5-fluoro-2′-deoxyuridine (FdUrd) and 5-fluoro-2′-deoxycytidine (FdCyd) were studied in PHA-stimulated lymphocytes from normal volunteer donors and a fragile X patient. In both cell types, FdUrd and FdCyd inhibited cell proliferation at concentrations of 3x10-8 M. Thymidylate synthetase was identified as the decisive target for the action of both FdUrd and FdCyd, as judged from the following observations: First, addition of thymidine to the culture medium was able to counteract both FdUrd and FdCyd toxicities, whereas addition of dCyd had no observable effect. Second, inhibition of the in situ thymidylate synthetase activity measured as an increase in the level of [3H]-dThd incorporation coincided with the inhibition of cell proliferation. Third, the inhibition of the thymidylate synthetase-dependent incorporation of [3H]-dUrd into newly synthesized DNA coincided with the inhibition of cell proliferation. The effects of FdUrd and FdCyd on the in vitro expression of fragile site Xq27 of fragile X chromosomes was shown to be based on the depletion of the intracellular pool of thymidine-5′-monophosphate (dTMP), as fudged from the following observations: First, both the FdUrd- and FdCyd-dependent induction of site Xq27 coincided with the antiproliferative effects of the respective fluoropyrimidines. Second, addition of thymidine (dThd) to the culture medium both prevented the expression of site Xq27 and neutralized the cytotoxicity of FdUrd and of FdCyd. On the basis of these findings, we provide further evidence for the concept that the fragile X site is located in an AT-rich region.

    Notes: Abstract A 14-year-old boy presented with anorexia and weakness whereon the diagnosis of dimorphic anaemia was made. An excellent response to iron and vitamin B12 was observed. In addition, the patient had non-obstructive hypertrophic cardiomyopathy. At endoscopy 2.5 years later, an adenocarcinoma was diagnosed and the patient underwent a high subtotal gastrectomy. To the best of our knowledge, this rare association has never been reported in children. Conclusion We report a youngster with pernicious anaemia, associated with non-obstructive hypertrophic cardiomyopathy in whom gastric adenocarcinoma was found. Patients with pernicious anaemia are at greater risk of developing gastric carcinoma than the general population, therefore we recommend routine periodic gastroscopic surveillance in the paediatric population with pernicious anaemia.

    Notes: In a double-blind randomized parallel-group study, six investigators enrolled 43 subjects to study the prophylactic effect of 10 mg cetirizine b.i.d. on grass pollen-induced asthma. The control group received 60 mg b.i.d. terfenadine, given to avoid withdrawals from the trial because of hay fever symptoms. Subjects were included in the study between the appearance of the first symptoms of hay fever and those of pollen-induced asthma. The hay fever and asthma symptoms, visual analogue scores (VAS), FEV1 and self-assessment data on complaints, salvage treatment and peak-flow values were statistically analysed. Both treatments were well tolerated, with a low and similar incidence of side-effects. During the last 3 weeks of treatment, six (32%) of the 19 subjects on cetirizine who were evaluable for efficacy remained free of asthma complaints, and another two (10%) had only a single minor attack. None had a grade 3 (incapacitating) attack. Conversely, only one (5%) of the 20 evaluable subjects on terfenadine remained complaint free, and all others (95%) had multiple attacks, which incapacitated three (15%) of them. Nasal obstruction, dyspnoea, morning peak flow, consumption of β2-mimetics and an efficacy index on asthma, combining complaints and rescue drug consumption, were significantly better on cetirizine (P 〈 0·05). It is concluded that cetirizine is able to prevent the exacerbation of asthma induced by grass pollen.

    Notes: Abstract Nonsteroidal anti-inflammatory drugs display a chemopreventive effect on polyps and cancer of the large bowel. This study evaluated the inhibitory effect of aspirin on the distribution and growth of aberrant crypt foci (ACF), the earliest putative preneoplastic and early neoplastic lesions in a rat model. For initiation of ACF, Sprague Dawley rats were injected with azoxymethane (30 mg/kg), a well-established rat carcinogen. After the second injection the rats were allocated to three groups, which received 0.2% or 0.6% aspirin or the solvent only (control group). After 6 weeks the animals were killed, and their colons removed, fixed in formalin, and screened for distribution and size of ACF, separately for middle and distal parts of the large intestine. The rats injected with azoxymethane showed a 100% incidence of ACF. Administration of 0.2% and 0.6% aspirin resulted in 55% and 54% reduction, respectively, in overall frequency of ACF. Aspirin significantly reduced the frequency of medium-sized (four to six crypts per focus) and large (three to six crypts per focus) but not the small (one to three crypts per focus) ACF. In the control group the ACF of the same multiplicity were larger than those in the aspirin-treated rats. No statistically significant difference in ACF-inhibiting effect was noted between 0.6% and 0.2% aspirin solution. Aspirin given at a concentration of either 0.2% of 0.6% thus has a chemopreventive effect on ACF, acting on postinitiation stage of azoxymethane-induced colonic carcinogenesis model in rats.

    Notes: Résumé Chez l'embryon de poulet femelle nous avons comparé à l'aide de la technique cytophotométrique de Lison la teneur en ADN des cellules germinales à noyau réticulé à celle des cellules au stade leptotène. Comme pendant la phase S d'un cycle cellulaire régulier, la quantité de DNA au cours du stade préleptotène, semble au moins doubler. Néanmoins nos résultats ne permettent pas d'affirmer ou d'exclure la possibilité d'une synthèse de DNA supplémentaire d'autre origine.

    Description / Table of Contents: Summary 100 specimens of cadaver blood (a variety of causes of death) have been examined by zone electrophoresis. In 73 cases the haptoglobin type could be definitely determined. In non-haemolytic sera it was possible to determine the haptoglobin type, but the great majority of the haemolytic sera could also be classified. In all cases of haemolytic serum in which it was not possible to estimate haptoglobin, attempts were made to raise the serum level of haptoglobin by concentration. This enabled us to determine the haptoglobin picture in a number of cases. The haptoglobin type could not be determined in 6 haemolytic sera and also in all decomposed blood from which serum was not expressed. The investigation appeared to be interfered with by changes in the serum proteins in the region of the alpha globulin fraction and by increase of haemoglobin in the beta globulin fraction. The causes of death and the time of death did not appear to affect haptoglobin estimation in our cases.
    Abstract: Résumé Des échantillons de sang prélevés après la mort dans 100 cas divers ont été soumis à une électrophorèse de zone. Le type d'haptoglobine a pu être établi dans 73 cas. L'analyse s'est effectuée sans difficulté pour tous les sérums non hémolytiques et pour 32 des 45 sérums hémolytiques. Pour les 13 sérums hémolytiques restants, il tut cependant possible d'obtenir une détermination dans 7 cas en augmentant la concentration sérique (dessication à froid et reprise en eau distillée). Dans 6 cas de sérum hémolytique et dans tous les cas où l'altération cadavérique ne permettait plus d'obtenir du sérum, le type d'haptoglobine n'a pas pu être établi. L'analyse a été manifestement perturbée par les modifications dans la fraction globulinique alpha et par l'augmentation de l'hémoglobine dans la fraction beta-globulinique. Dans la limite des cas examinés, la cause de mort ne semble pas avoir eu d'influence sur la détermination de l'haptoglobine, non plus que le temps écoulé entre le décès et le moment de l'analyse.
    Notes: Zusammenfassung Es wurden Blutproben von 100 Leichen mit verschiedenen Todesursachen zonenelektrophoretisch untersucht. In 73 Fällen ließen sich die Haptoglobintypen einwandfrei bestimmen. In nicht-hämolytischen Seren war der Haptoglobintyp in jedem Fall einwandfrei zu ermitteln. Der größte Anteil der hämolytischen Seren ergab noch klare Haptoglobinbilder. In allen Fällen hämolytischer Seren, wo eine Haptoglobin-Bestimmung nicht mehr möglich erschien, wurde versucht, die Haptoglobin-Serumkonzentration durch Einengung zu erhöhen. Dadurch waren in weiteren Fällen noch einwandfreie Haptoglobinbilder zu erhalten. Bei 6 hämolytischen Seren und bei allen faulen, kein Serum mehr absetzenden Bluten konnte der Haptoglobintyp nicht mehr ermittelt werden. Die Untersuchung war offensichtlich durch Serumprotein-Veränderungen im Bereich der Alpha-Globulin-Fraktion und durch Hämoglobin-Anreicherung in der Beta-Globulin-Fraktion empfindlich gestört. Die Todesursachen und die von uns untersuchten Todeszeiten schienen auf die Haptoglobin-Bestimmung keinerlei Einfluß genommen zu haben.
