    Notes: Abstract. Objective and Design: We investigated the in vitro responsiveness of neutrophils adherent to fibronectin (FN) and laminin (LM), toward natural pro-inflammatory and/or phagocyte-activating agents.¶Materials and Methods: Neutrophils from normal volunteers were layered on polystyrene wells precoated or not with FN and/or LM and tested for their ability of responding to eleven pro-inflammatory mediators by evaluation of superoxide anion (O− 2) production and adherence. Results, expressed as mean ± 1 SEM, were evaluated by non-parametric analyses (Mann-Whitney U-test or Kruskal-Wallis non-parametric ANOVA analysis).¶Results: Precoating polystyrene wells with LM or FN prevented the plastic-induced neutrophil O− 2 production. Among eleven agents, tumor necrosis factor-alpha (TNF, 3.0 ± 0.3 nmoles O− 2/5 × 104 neutrophils/180 min, p 〈 0.001), granulocyte-macrophage colony stimulating factor (GM-CSF, 2.1 ± 0.3 nmoles O− 2/5 × 104 neutrophils/180 min, p 〈 0.05) and formyl-peptides (fMLP, 2.5 ± 0.5 nmoles O− 2/5 × 104 neutrophils/180 min, p 〈 0.01) caused massive O− 2 production by neutrophils adherent to FN. None of the mediators was capable of triggering O− 2 production by neutrophils adherent to LM. LM, mixed with FN to coat wells, caused a dose-dependent inhibition of the oxidative burst triggered by TNF (IC50 LM: 0.84 ± 0.03 μg, mean ± 1 SEM), GM-CSF (IC50 LM: 0.36 ± 0.16 μg, mean ± 1 SEM) and fMLP (IC50 LM: 0.54 ± 0.008 μg, mean ± 1 SEM). To the contrary, fMLP (85.5 ± 27.7%), TNF (163.1 ± 67.5%), and GM-CSF (121.8 ± 66.4%) caused a significant augmentation of neutrophil adherence to LM, suggesting that LM-mediated inhibition of neutrophil oxidative metabolism does not depend on the concomitant LM-induced inhibition of neutrophil adherence. Finally, neither solid-phase FN nor LM affected O− 2 production by neutrophils in response to immune complexes.¶Conclusions: Extracellular matrix glycoproteins dictate the response of neutrophils to soluble mediators but not to immune complexes. This appears to be a biologically meaningful mechanism to localise the risk of cellular reactions to mediators that are able to diffuse easily from tissue sites of generation and become widely distributed in body fluids during inflammatory diseases.

    Notes: Abstract The US, Doppler and CT appearances in a patient with hemangiopericytoma of the greater omentum are presented. Ultrasound revealed a solid mass with heterogenous echo pattern, well-defined margins and marked vascularity with low impedance flow (mean resistive index 0.42). The tumour was mobile. It was detected below the left kidney at the first US examination and had migrated into the pelvis 14 days later. At CT the tumour demonstrated strong but brief peripheral enhancement and a central hypodense s scar.

    Notes: Abstract To investigate the normal internal structure of tendons 11 volunteers without clinical evidence of tendinopathy were examined using conventional spin-echo T1-, T2- and proton-density weighted sequences. The Achilles tendon was chosen because of its high frequency of injury in athletic activity, large size, superficial position and because it is oriented nearly parallel to the static magnetic field, therefore minimizing the “magic angle phenomenon”. The tendons exhibited areas of slightly increased signal in four T1-weighted and in all but one proton-density-weighted scans. No intratendinous signal was detected in T2-weighted images. The possible origin of these findings is discussed. We conclude that the knowledge of these normal signals may be useful to avoid incorrectly diagnosing as pathological.

    Notes: Abstract. The aim of this study is to evaluate capability of contrast enhanced ultrasonography (US) using pulse inversion harmonic imaging (PIHI) to detect liver metastases in comparison to fundamental B-mode ultrasound and spiral CT. Thirty-six consecutive patients with known malignancies and sonographically proved or suspicious liver metastases have been examined with fundamental B-mode US, with PIHI 2', 4' and 6' after Levovist injection and with four phase spiral-CT. Presence, conspicuity and number of lesions have been evaluated comparing PIHI with fundamental B-mode US and spiral-CT. A strong grey-scale enhancement of the liver parenchyma has been observed 2' and 4' after Levovist injection. The optimum parenchymal enhancement and contrast difference between liver and metastases was observed during the 2' measurements. PIHI revealed more lesions than fundamental B-mode US in 56 % of patients, while in 39 % and in 5 % revealed respectively the same number and fewer lesions. PIHI and spiral-CT were in agreement in 67 % of patients, while in 22 % and 11 % PIHI revealed respectively more and fewer lesions. PIHI accurancy presents restrictions in anterior superficial and in deep liver areas, whereas it may be superior to spiral-CT in studying sub-diaphragmatic liver regions.

    Notes: Abstract A case of primary non-Hodgkin's lymphoma (NHL) of the mandible with unusual radiographic presentation is reported. The panoramic radiography revealed diffuse widening of the right mandibular canal with ill-defined margins and absence of edge sclerosis. Computed tomography showed infiltration of the superior portion of the canal with involvement of the vestibular cortex of the bone and of the alveolar ridge.

The aim of our study was to analyze changes in spectral Doppler waveforms between interlobar and interlobular arteries in renal transplants and to determine whether sampling location at interlobular level can be suitable for intrarenal resistive index (RI) measurements. Paired series of spectral tracings from interlobar arteries and respective interlobular branches were obtained in 62 consecutive renal transplants at 6.5-MHz Doppler frequency. The values of peak systolic velocity (PSV), end diastolic velocity (EDV) and RI were significantly (P 〈 0.01) reduced when calculated at interlobular level. In 38 % of cases, an interlobar RI higher than 0.70 corresponded to a normal interlobular RI. The values of PSV, EDV, and RI did not differ significantly at interlobular level between allograft subsets with normal and elevated serum creatinine level. Both intra- and interobserver variation were higher at interlobular than at interlobar level when performing the RI. During a conventional study of renal vasculature, an underestimation of abnormal RI findings can be expected from the incidental evaluation of interlobular tracings. We recommend sonologists to pay attention in accurately locating the sample volume at interlobar–arcuate level when evaluating intrarenal RI.
