    Notes: Abstract The differentiation between neonatal neuroblastoma and adrenal haemorrhage may be very difficult in the individual case. We investigated eight neonates with adrenal haemorrhage and one patient with congenital neuroblastoma by colour coded Doppler sonography and power Doppler sonography. Six haemorrhages were cystic, whereas the neuroblastoma and four adrenal haemorrhages had a solid appearence. Colour coded Doppler sonography and power Doppler demonstrated vessels within the neuroblastoma. In none of the ten adrenal haemorrhages could flow be shown either with conventional colour coded Doppler sonography or power Doppler sonography. Conclusion Conventional colour coded Doppler sonography and power Doppler sonography are useful to differentiate between neuroblastoma and adrenal haemorrhage.

    Notes: Zusammenfassung Es wird über den vierten Fall eines solchen Tumors in der Weltliteratur berichtet, der bei einer 53 Jahre alten Frau im Rahmen einer totalen Hysterektomie als Zufallsbefund erhoben wurde. Die Morphologie und Pathogenese der extraovariellen Brenner-Tumoren werden diskutiert.

    Description / Table of Contents: Summary Background: Hypoperfusion of the brain stem due to certain positions of head and body followed by central apnea may be an important cause of sudden infant death syndrome. Methods: 23 infants with a mean age of 13±12.5 weeks (6 days to 11 months) with apparent life threatening event were investigated by cranial Doppler sonography. In all patients flow measurements were performed in one anterior cerebral artery, both internal carotid arteries, the basilar artery, and both vertebral arteries in dependence on head (right/left/neutral) and body position (prone/supine). Results: In none of our infants with apparent life threatening event we found significant flow alterations in the anterior cerebral artery or both internal carotid arteries following certain positions of head and body. In 16 of the 23 patients no dependence of flow in the basilar artery and both vertebral arteries on head and body position could be found. In 7 infants however pathologic flow profiles and flow velocities could be shown in the contralateral vertebral artery if the head was turned to the other side. In 4 of these 7 infants additionally pathologic flow profiles and flow velocites in the basilar artery could be found. The reduction of the blood flow is caused by compression of the vertebral arteries at the craniocervical junction. Conclusion: The reduction of blood flow in the vertebral arteries and especially the basilar arteries may cause hypoperfusion of the brainstem followed by central apnea. Hypoperfusion of the brainstem in dependence on head and body position may be a significant cause of sudden infant death syndrome. With the help of cerebral Dopplersonography infants at risk for sudden infant death can be detected. Body and head positions which should be avoided can be evaluated noninvasively.
    Notes: Zusammenfassung Fragestellung: Eine bisher wenig beobachtete Ursache des plötzlichen Kindstods könnte eine Minderperfusion des Hirnstamms in Abhängigkeit von Körperlage und Kopfstellung mit konsekutiver zentraler Apnoe sein. Methode: Zur Überprüfung dieser These wurden 23 Säuglinge im Alter von 6 Tagen bis 11 Monaten (Mittelwert 13±12,5 Wochen) mit anscheinend lebensbedrohlichem Ereignis (ALE) dopplersonographisch untersucht. Bei allen Kindern wurden Flußmessungen in der A. cerebri anterior, in beiden Aa. carotides internae, der A. basilaris und in beiden Vertebralarterien in Abhängigkeit von Körperlage (Bauch/Rückenlage) und Kopfhaltung (rechts/links/Mitte) durchgeführt. Ergebnisse: Bei keinem Kind mit ALE konnte eine Abhängigkeit der Blutströmung in der A. cerebri anterior und A. carotis interna von Körperlage und Kopfhaltung gefunden werden. Bei 16 der 23 Kinder mit ALE fand sich zudem keine Abhängigkeit der Blutströmung in der A. basilaris und in den Vertebralarterien von Körperlage und Kopfhaltung. Bei 7 Kindern konnten bei Kopfwendung zur Seite pathologische Flußprofile und Flußgeschwindigkeiten in den kontralateralen Vertebralarterien, bei 4 Kindern auch in der A. basilaris gefunden werden. Wir erklären den Abfall der Blutströmung durch die Kompression der Vertebralarterien am kraniozervikalen Übergang beim Austritt aus dem Foramen transversarium und beim Verlauf um die Atlasschlinge. Schlußfolgerung: Der Abfall der Blutströmung im Vertebralis- und v.a. Basilarisstromgebiet kann zur bedrohlichen Minderperfusion des Hirnstamms führen und zentrale Apnoen bedingen. Die Minderperfusion des Hirnstamms in Abhängigkeit von der Kopfhaltung und der Körperlage könnte somit eine wesentliche Ursache des plötzlichen Kindstods sein. Mit der zerebralen Dopplersonographie steht eine nichtinvasive Methode zur Verfügung, mit der Risikokinder erfaßt und Körperlagen und Kopfhaltungen, die vermieden werden sollten, erkannt werden können.

    Description / Table of Contents: Zusammenfassung Es wird über einen Fall einer Leiomyomatosis peritonealis disseminata berichtet. Hierbei handelt es sich um multiple multizentrisch entstandene benigne Leiomyome unter dem Peritoneum. Da sie meistens im Verlauf einer Schwangerschaft auftreten und sich danach weitgehend zurückbilden, ist eine hormonelle Induktion des Tumorwachstums wahrscheinlich. Wegen der guten Prognose darf diese Krankheit nicht mit einem metastasierenden Leiomyosarkom verwechselt werden, damit inadäquate schwerwiegende therapeutische Maßnahmen vermieden werden.
    Notes: Summary Report of a case of disseminated peritoneal leiomyomatosis which is characterized by the occurrence of multiple benign leiomyomas of multicentric origin below the peritoneum. Usually they appear in the course of pregnancy and are prone to regress after delivery. Therefore a hormonal induction appears probably. In regard of the favourable prognosis this lesion must not be mistaken for a metastasizing leiomyosarcoma in order to avoid inadequate serious therapeutic measures.

    Description / Table of Contents: Zusammenfassung Eine heterotope Knochenneubildung in malignen Tumoren oder deren Metastasen ist extrem selten. Bei einem 60jährigen Patienten zeigten multiple Muskelmetastasen eines Bronchialcarcinoms (Adenocarcinoms) eine ausgedehnte Verknöcherung. Auf Grund der histologischen Untersuchungsbefunde wird der Mechanismus der Knochenneubildung im Rahmen maligner Geschwülste diskutiert. Danach induzieren die Tumorzellen eine Proliferation des ortsständigen Mesenchyms mit Ausdifferenzierung der Mesenchymzellen zu Osteoblasten.
    Notes: Summary The formation of heterotopic bone tissue in malignant tumors or in their metastases is extremely rare. In a 60 years old male patient with bronchogenic carcinoma (adenocarcinoma) extensive bone formation was observed within multiple metastases in the skeletal muscles. On the basis of the microscopic findings, the mechanism of bone formation by malignant tumors is discussed. Obviously, proliferation of local mesenchymal tissue and differentiation of mesenchymal cells to osteoblasts is induced by the tumor cells.

    Notes: Summary Seven cases are reported that initially presented on endoscopic examination as duodenal polyps originating from islands of gastric mucosa within the duodenal bulb. Microscopic examination revealed mucosal cysts (MC), focal foveolar hyperplasia (FFH), and hyperplastic polyp (HPP). These lesions must be added to the list of neoplastic and tumorlike lesions of the duodenum that may endoscopically present as polyps.

    Notes: Abstract Immunohistochemistry was performed on biopsies of columnar mucosa from 11 patients with Barrett's esophagus and 11 patients with columnar mucosa in the cranial esophagus, the “inlet patch.” Both epithelia contained endocrine cells, immunoreactive to antisera against serotonin, glucagon, somatostatin, and pancreatic polypeptide; the specialized mucosa of Barrett's esophagus contained, in addition, neurotensin-immunoreactive cells, and in the mucosa of an inlet patch we found a gastrin cell. These findings are not compatible with some of the current theories on the origin of these epithelia. The mucosa of the inlet patch has been considered to consist of heterotopic gastric mucosa. The mucosa of the adult human stomach, however, does not contain glucagon cells. These cells are only present in the early embryonic stomach, and they disappear during embryonogenesis. According to our findings, the mucosa of the inlet patch therefore represents embryonic gastric mucosa. The specialized columnar epithelium of Barrett's esophagus has been considered to have evolved from gastric mucous neck cells. However, although glucagon cells are a feature of the embryonic stomach, neurotensin-immunoreactive cells have not been found in the gastric mucosa. Our study suggests that the specialized columnar epithelium of Barrett's esophagus originates from a very immature multipotent gastrointestinal stem cell.
