Summary The bone changes in the skull invon Recklinghausen's neurofibromatosis are described. A solitary periostal neurofibroma has an appearance like an epidermoid, dermoid or lipoma—viz: a depression with smooth edges and with a well defined sclerotic bone margin. In correlation with plexiform neuromata (elefantiasis neuromatosa) of the face there are frequently a very definite enlargement of the sella, pronounced enlargement and distortion of the orbit, developemental anomalies of the eyeball, defects in the orbital roof and outward bulging of the floor of the skull. It is emphasized that the size of the plexiform neuroma usually corresponds with the territory of skin innervated by one or more trigeminal branches and the size of the bone lesions is corresponding on the other hand with the area of innervation of the dural and meningeal branches of the same nerve. On the basis ofKautzky's researches on the innervation of the meninges andMarchesani's investigations into the innervation areas of the eyeball, it is believed that the changes in the skin, bones and eyeball are based on a superordinated affection of the parasympathetic fibers (so called parophthalmic nerve) accompanying the ophthalmic nerve (“Parophthalmicus Syndrome”).

Résumé Description des altérations osseuses du crâne au cours de la neurofibromatose deRecklinghausen. Des neurofibromes périostés solitaires créent dans les os des dépressions à bords lisses avec des arêtes saillantes scléreuses semblables à des formations épidermoïdes, dermoïdes et des lipomes. En relation avec les neuromes plexiformes (éléphantiasis des formations cutanées) dans le territoire du visage on trouve souvent une selle turcique très élargie, une orbite augmentée et grimaçante, un aspect anormal du globe oculaire sinsi que des manques dans le toit de l'orbite et des échancrures dans le squelette de la base. Il apparait que l'étendue du neurome plexiforme est en accord le plus souvent d'une manière ample avec le territoire de distribution cutanée d'une ou de plusieurs branches du trijumeau et que les modifications osseuses se trouvent là où la même branche du trijumeau innerve la dure-mère et le périoste. En se basant sur les recherches deKautzky sur l'innervation cutanée et sur celles deMarchesani sur l'innervation du globe oculaire, on admet que les lésions au niveau de la peau, des os et du globe oculaire sont dans le cadre de la neurofibromatose des symptomes d'un trouble du parasympathique dont les branches accompagnent le nerf ophtalmique (syndrome parophtalmique).

Zusammenfassung Es werden die Knochenveränderungen am Schädel bei NeurofibromatosisRecklinghausen beschrieben. Solitäre periostale Neurofibrome erzeugen gleich Epidermoiden, Dermoiden und Lipomen glattrandige Mulden mit scharfen sklerosierten erhabenen Rändern im Knochen. In Verbindung mit Rankenneuromen (Lappenelefantiasis) im Gesichtsbereich finden sich häufig eine stark erweiterte Sella, eine verzerrte vergrößerte Orbita, Augapfelfehlbildungen sowie Defekte im Orbitaldach und Ausbuchtungen an der Schädelbasis. Es wird hervorgehoben, daß die Ausdehnung der Rankenneurome mit dem Hautversorgungsgebiet eines oder mehrerer Trigeminusäste meist weitgehend übereinstimmt und die Knochenveränderungen sich dort finden, wo der gleiche Trigeminusast die Dura und das Periost innerviert. Auf Grund der Untersuchungen vonKautzky über die Hirnhautinnervation und jener vonMarchesani über die nervöse Versorgung des Augapfels wird angenommen, daß die Veränderungen an der Haut, am Knochen und am Augapfel Symptome einer im Rahmen der Neurofibromatose auftretenden übergeordneten Störung des Parasympathicus („Parophthalmicussyndrom“) sind, dessen Äste den N. ophthalmicus begleiten.
