    Notes: Abstract Almost 15 years after its introduction, endosonography or endoscopic ultrasonography is an important technique in a wide range of gastrointestinal diseases. Other imaging techniques are CT, MRI and barium examination. There is a general consensus that the most important prognostic factor in gastric and colorectal carcinoma is the presence or absence of lymph node invasion, but malignant fixation of tumour through direct invasion of adjacent tissues also appears to be very important. Non-invasive preoperative assessment of tumour stage based on one or a combination of the above imaging modalities should allow appropriate treatment to be planned in each case.

    Notes: Abstract A series of 11 malignant lymphomas of the breast (6 primary non-Hodgkin lymphomas, 4 secondary non-Hodgkin lymphomas, 1 Hodgkin's disease) is reported. Mammography, performed in 10 cases, revealed an ill-defined density (total or partial) (6 cases), a well-defined mass with multicyclic contours (2 cases), a mass with spiculations (1 case) and a false negative (1 case). The sonographic patterns analysed for 7 patients were non-specific, showing a higly hypoechoic, heterogenous mass with marginal irregularity in all cases. There was no correlation between the radiological appearances and the sonographic patterns; the latter were similar for both primary and secondary lesions. In contrast to mammography, ultrasonography has never been used in large literature series for diagnosis of these exceptional sites of lymphoma. Ultrasound also appears useful for patient follow-up to evaluate the response to treatment.

    Notes: Abstract A review of liver sonograms obtained for cancer patients (excluding primary liver cancers) over a 12 year period found 829 benign lesions: non-parasitic cysts (427 cases), hemangiomas (216 cases), solitary calcifications (79 cases), focal fatty infiltration (62 cases), and miscellaneous lesions (45 cases). These benign pathologies represented 41.8% of the focal hepatic lesions observed during this period in this population; hepatic metastases accounted for the remaining 58.2%. Marked female predilection was noted for the nonparasitic cysts, hemangiomas, and focal fatty infiltration; 63–78.7% of these lesions were solitary, and first-line imaging by US was sufficient for diagnosis of 66.1–98.2% of cases. Analysis of lesion evolution over more than 5 years revealed modifications in 17% of hemangiomas, 23.9% of nonparasitic cysts, and 75% of cases of focal fatty infiltration. Systematic pretherapy liver sonography can be proposed owing to the high frequency of benign liver lesions that can create diagnostic problems during follow-up of cancer patients.

    Notes: Abstract Benign fibrous mesothelioma of the pleura is a rare tumor of mesodermal origin. We describe the MR findings in three pathologically proven cases. All three tumors were imaged by MR as well-circumscribed lesions with smooth margins in contact with the pleura, but without chest wall invasion. Their low signal intensity on T1-and T2-weighted sequences reflect their fibrous nature. In one case a pedicle connecting the tumor to the chest wall was visualized on a sagittal MR scan. In two cases gadolinium-enhanced T1-weighted gradient echo sequences revealed intense contrast uptake by the tumor correlated with the intratumoral hypervascularization noted by histologic examination. Although the number of cases presented is small, MR seems to be the most accurate imaging modality in the assessment of the diagnosis.

    Notes: Abstract The purpose of this study was to analyse the sonographic patterns of solitary solid liver lesions in cancer patients and to evaluate the diagnostic accuracy of ultrasound alone and in combination with other techniques (liver function tests, histology). A total of 422 solitary solid liver lesions (SSLL) were diagnosed by ultrasound in cancer patients; 197 lesions were benign and 225 malignant. The predominant aetiology for hypoechoic SSLL (128 cases) was metastasis (112 cases), whereas most hyperechoic SSLL (265 cases) were haemangiomas (155 cases) rather than a metastasis (86 cases). The 29 isoechoic SSLL included 27 metastases and 2 benign lesions. A halo was found to be highly predictive of malignancy (97%–100%). The positive predictive value for malignancy of an SSLL was very high when the results of liver function tests were abnormal (97%–100%). In our experience, histological proof is unnecessary in the majority of liver lesions in cancer patients.

    Notes: Abstract  Wasting is a major complication of advanced head and neck cancer. Concomitant chemotherapy and twice-daily continuous radiotherapy with no acceleration represents a promising treatment modality for these tumors, but increases the risk of mucositis. This report describes the results achieved with percutaneous fluoroscopic gastrostomy (PFG) and its impact on the quality of life of patients with head and neck cancer in terms of their nutritional status. A total of 50 stage IV tumors of the oropharynx and hypopharynx recorded in a prospective database were reviewed retrospectively. All patients were managed by PFG, which was found to be a safe and effective technique with no technical failures. PFG feeding resulted in a mean increase in body weight of 2.5 kg within 3 weeks. The body mass index (BMI) was maintained at 3 and 6 weeks. Minor complications occurred, but no major complications were noted. The overall procedure-related mortality rate was nil. Further prospective investigations are necessary to determine whether treatment of wasting improves patient survival.

    Notes: Abstract Despite the infrequency of gastrointestinal pseudoaneurysms, the potential risks of these vascular lesions make early diagnosis essential. Seven cases were reviewed to compare the diagnostic value of various imaging procedures. Three lesions had a pancreatitic origin and 3 others developed after surgery: no origin was identified for the last case. Ultrasonography was performed in 3 cases, but the sonographic patterns failed to suggest the diagnosis. By contrast, computed tomography constantly recognized the pseudo-aneurysms (diameters ranged between 1.5 and 3.5 cm). Angiography (5 cases) confirmed the diagnosis and was helpful for presurgical workup. A review of the literature and the authors' experience underscore the diagnostic value of CT for these lesions.

    Notes: Abstract  The goal of this work was to evaluate the costs and benefits of percutaneous interventional radiological procedures (PIRP) in terminal cancer patients, from the perspective of the Radiodiagnostics Department. The subjects were 225 patients who underwent different kinds of treatments, such as placement of endovenous or urinary stents, percutaneous gastrostomy, alcoholization of metastatic disease, celiac plexus block, tumor embolization, and inferior vena caval filter. We retrospectively analyzed the consequences in terms of survival, quality of life and cost ratios and found that this study fully justifies the use of interventional radiology in palliative oncology: 60% and 40% of the patients, respectively, were still alive at 1 month and 3 months; the additional cost of PIRP procedures is low (〈12%) compared with the total cost of hospitalization.
