    Notes: To evaluate the value of polymerase chain reaction-in situ hybridization (PCR-ISH) for the detection of human papillomaviruses (HPV) in paraffin sections of cervical biopsies fixed either in 10% formalin or in Bouin's solution.〈section xml:id="abs1-2"〉〈title type="main"〉Methods and resultsWe analysed 40 biopsies from Italian women infected with the human immunodeficiency virus type 1 (HIV 1). In-situ hybridization techniques were performed with commercial biotinylated probes. The PCR-ISH was carried out by the ‘hot start modification’. Cervical intraepithelial neoplasia (CIN) was found in 23 of 40 patients (57.5%); eight cases showed condylomatous features. Human papillomavirus was detected in 42.5% by ISH and in 65% by PCR-ISH. Sixty-nine per cent of positive biopsies contained HPV 16, 18, 31 and 33. HPV 6 and 11 were found only in condylomata acuminata samples.〈section xml:id="abs1-3"〉〈title type="main"〉ConclusionsThe results point to a high incidence of HPV infection as well as of CIN in HIV-positive patients. Human papillomavirus type 16 appears to be most frequently associated with CIN. Polymerase chain reaction-ISH is more sensitive than ISH in the detection and typing of HPV DNA both in clinical and in ‘latent’ infections. The two techniques yielded the same results with either formalin- or Bouin's-fixed material.

    Notes: Abstract A prospective, multicenter, open study of fluconazole prophylaxis was performed in AIDS patients to evaluate the efficacy and toxicity of the drug in preventing relapses of esophageal candidiasis. To this aim, 99 AIDS patients who presented a first episode of clinically and microbiologically confirmed esophageal candidiasis were enrolled in eleven clinical centers scattered throughout the Italian territory. After resolution of this initial esophagitis, all subjects were given fluconazole, 100 mg/die, and followed up for a 6 month period. Only 7 out of the 99 patients enrolled had a relapse ofCandida esophagitis, during a mean follow-up period of 138.5 days. All relapsing patients had CD4+ cell number 〈100/µl at baseline. Mild side effects were reported in only eight patients. However, 14 of the 27 subjects from whom serial serum samples were available became (12) or remained (2) antigenemic during fluconazole prophylaxis, independently from relapse, suggesting the persistence of tissue-invasive, proliferatingCandida cells. Overall, the data of this study suggest a beneficial effect of prophylactic maintenance therapy with fluconazole againstCandida esophagitis, particularly in the population with 〉100 CD4+/µl. However, the data onCandida antigenemia in these patients invite the consideration of a relative inefficiency of the drug to eradicate the microrganism from the esophageal tissue.

    Notes: Abstract Objective: To compare the pharmacokinetics of foscarnet administered as an infusion twice daily (BID) or thrice daily (TID), and to compare the effects on the electrolyte balance, cardiac and renal functions over a 3-week induction treatment of Cytomegalovirus (CMV) retinitis. Methods: Pharmacokinetics/dynamics of foscarnet were investigated on treatment days 1, 14 and 21. Twelve AIDS patients with CMV retinitis completed the investigation period. Concentrations of foscarnet and electrolytes were assayed by high-performance liquid chromatography (HPLC) and by an ion-selective analyser, respectively. Results: The pharmacokinetics of the two regimens were essentially similar. Foscarnet plasma and creatinine clearances were 2.0 and 1.6 ml · min−1 · kg−1, respectively, in the BID group at steady state (day 21). In the TID group the corresponding values were 1.8 and 1.7 ml · min−1 · kg−1, respectively. In both regimens the elimination half-life of foscarnet was 2–3 h. Ionized calcium concentrations were transiently decreased and strongly inversely correlated to foscarnet plasma concentrations in both regimens with no significant differences between groups. A trend towards prolongation of the QTc interval was seen when data from both treatments were analysed together. Conclusion: Our data suggest comparable pharmacokinetics of foscarnet after intermittent administration BID or TID during a 3-week induction period.

    Notes: Abstract The authors present the AIDS cases (CDC '93) observed in Brescia from 1983 to 1994. They observed 1189 subjects (M 84%, F 16%) with a mean age of 32.7 years (intra-venous drug users 75.1%, heterosexuals 14%, homosexuals 9.6%). The mean survival observed was 56.7 weeks from the diagnosis of AIDS (mortality per year 78%). The most frequent AIDS-defining events were Visceral Candidiasis, P.carinii Pneumonia (PCP) and Neurotoxoplasmosis, while the longest and shortest mean survival was for Kaposi's Sarcoma (89 weeks) and Wasting Syndrome (8.4). The mean value of CD4+ lymphocyte counts on AIDS diagnosis was 72.6/μl (1166 cases) and the highest and lowest were in non-Hodgkin's Lymphoma (NHL; 147.6/μl) and Cryptosporidiosis (18.8/μl). Antiretroviral therapy had been given for at least a month in 41.4% subjects (mean treatment duration of 74.8 weeks). The Cox model has demonstrated the favourable effect on survival of high CD4+ lymphocyte counts on diagnosis, antiretroviral therapy, the diagnosis of Tuberculosis (TBC) and PCP as initial markers and the diagnosis of TBC, PCP or Cytomegalovirus infection (CMV) during the entire clinical evolution. Moreover, the unfavourable effect of high age, diagnosis of Progressive Multifocal Leucoencephalopathy (PML), Wasting Syndrome and NHL as initial markers and diagnosis of PML or NHL in any moment of the disease has been demonstrated.

    Notes: Abstract Systemic fungal infections are diseases of increasing incidence in immunocompromised patients. They are an important cause of morbidity and mortality in patients with severe granulocytopenia caused by hematological malignancies or high-dose corticosteroid treatments. Systemic candidiasis is usually attributed to colonization of the patient's digestive mucosa or central venous/peritoneal catheters by endogenous Candida spp. Esophageal candidiasis can be a serious problem in predisposed individuals, and it can be the first opportunistic infection suggesting overt AIDS in HIV-seropositive patients. The aim of this review is to assess the epidemiology of alimentary tract mycoses in “at risk” groups because of iatrogenous or pathological reasons, such as in-patients undergoing solid organ and bone marrow transplantation, peritoneal dialysis and enteral/parenteral nutrition and in HIV infected patients.

    Description / Table of Contents: Zusammenfassung Diese Untersuchung berichtet über Veränderungen der Ultrastruktur und Funktion bei menschlichen Monozyten des peripheren Blutes von Personen, die mit einer Brucella-Vakzine behandelt wurden, und Erkrankten, die eine akute Brucella-melitensis-Infektion überstanden hatten. Die Monozyten dieser Patienten zeigten eine steigende phagozytäre und bakterizide Aktivität in vitro und morphologische Veränderungen, die als ein Charakteristikum der sog. Makrophagenaktivierung angesehen wurden. Es handelt sich hierbei um eine funktionelle Eigenart, die lediglich bei Tieren in Verbindung mit dem „cell-mediated“ Immunstatus beobachtet wurden. Die Autoren diskutieren ihre Befunde im Hinblick auf früher gewonnene Resultate bei der humanen Tuberkulinallergie. Folgende Hypothese wird aufgestellt: Die beobachteten morphologischen und funktionellen Veränderungen der Monozyten könnten in der Weise zustande kommen, daß „lymphokine factors“, die auf mononukleäre Phagozyten wirken, hierfür verantwortlich sind.
    Notes: Summary This report deals with some ultrastructural and functional changes which were observed in human monocytes from peripheral blood in patients treated with Brucella vaccine during the remittance phase of an acute B. melitensis infection. Monocytes from these patients showed increased “in vitro” phagocytic and bactericidal activities and displayed morphological attributes which are considered characteristic of the so-called macrophage activation, a functional modification observed solely in animals in association with cell-mediated immunity states. The authors discuss their findings in relation to previous results obtained in human tuberculin hypersensitivity and put forward the hypothesis that the observed modifications in morphology and function of monocytes might be obtained through release by sensitized lymphocytes of lymphokine factors acting on mononuclear phagocytes.
