    Notes: Summary To asses the long-term evolution of diabetic polyneuropathy after a combined kidney-pancreas transplant, an electrophysiological study was performed in 20 diabetic patients before transplant, and 1 (n=18), 2 (n=16), 3 (n=10) and 4 years (n=5) at a later date. Motor and sensory scores were calculated for conduction velocity and amplitude to determine the physiopathological process. During evolution the scores were not found to be decreasing. Motor and sensory velocity scores were significantly improved (p〈0.05) 1 and 2 years after the graft, when score values tended to stabilize. Motor and sensory amplitude scores, which are more sensitive for axonal loss assessment were slightly but not significantly improved.

    Notes: Using experimental models with normal allogeneic bone marrow (BM) contaminated with Burkitt or neuroblastoma cell lines, and a liquid culture assay, we demonstrated that, when used in optimal conditions, the immunomagnetic depletion technique permitted a reproducible elimination of 3–4 log malignant cells. Results were very similar to those obtained with the complement lysis procedure in Burkitt lymphoma. This immunomagnetic procedure was used in 123 cases of autologous bone marrow transplantation (ABMT) in children with neuroblastoma. The analysis of the cases demonstrated, first, that the procedure induced a significant loss of mononuclear cells but was not toxic for BM precursors. Delays to engraftment observed in a few patients were probably due to the combination of pejorative factors, especially the damage caused to the micro-environment by previous heavy and prolonged chemotherapy or the double ABMT programme. Second, patients presented with profound T-cell defect with undetectable IL2 secretion up to 1 year post-graft but they all had normal NK functions from the first month post-graft, these functions exceeding normal values on the second and third months post-graft. Finally, in 20 cases, dual-immunofluorescence staining permitted the demonstration that the autograft contained malignant cells before purging that were eliminated by the immunomagnetic depletion.

    Notes: Abstract  In the framework of the Global Soil Wetness Project (GSWP), the ISBA land-surface scheme of the ARPEGE atmospheric general circulation model has been forced with meteorological observations and analyses in order to produce a two-year (1987–1988) soil moisture climatology at a 1∘×1∘ horizontal resolution. This climatology is model dependent, but it is the climatology that the ARPEGE model would produce if its precipitation and radiative fluxes were perfectly simulated. In the present study, ensembles of seasonal simulations (March to September) have been performed for 1987 and 1988, in which the total soil water content simulated by ARPEGE is relaxed towards the GSWP climatology. The results indicate that the relaxation has a positive impact on both the model's climatology and the simulated interannual variability, thereby confirming the utility of the GSWP soil moisture data for prescribing initial or boundary conditions in comprehensive climate and numerical weather prediction models. They also demonstrate the relevance of soil moisture for achieving realistic simulations of the Northern Hemisphere summer climate. In order to get closer to the framework of seasonal predictions, additional experiments have been performed in which GSWP is only used for initialising soil moisture at the beginning of the summer season (the relaxation towards GSWP is removed on 1st June). The results show a limited improvement of the interannual variability, compared to the simulations initialised from the ARPEGE climatology. However, some regional patterns of the precipitation differences between 1987 and 1988 are better captured, suggesting that seasonal predictions can benefit from a better initialisation of soil moisture.

    Notes: Abstract In order to make inferences on the possible future changes of tropical cyclogenesis frequency, we apply the diagnostic computation of the Yearly Genesis Parameter (YGP) proposed by Gray (1975) to the large-scale fields simulated by a GCM. The YGP is an empirical diagnostic of the frequency of Tropical Cyclones (TCs) based on six physical parameters computed from seasonal means of atmospheric and oceanic variables. In this paper, we apply the YGP diagnostic to the results of three climate simulations performed with the atmospheric General Circulation Model (GCM) of Météo-France: ARPEGE-Climat. In a control simulation of the current climate, it is shown that the model has a realistic tropical climatology and that the computed YGP reproduces the geographical distribution of the tropical cyclogenesis frequency. The YGP is then applied to two simulations corresponding to two scenarios of doubled carbon dioxide concentration. The two experiments differ by the sea surface temperatures (SSTs) used as a lower boundary condition. In both simulations the YGP gives a large increase of total cyclogenesis frequency, but without extension of the area of possible cyclone genesis. The increase in YGP is due essentially to the contribution of the ocean thermal energy factor in the thermodynamical potential. The dynamical parameters, on the contrary, limit the cyclogenesis increase and are a major explanation of the difference between the two experiments. This is in agreement with the results of the previous similar study of Ryan et al. (1992) concerning the importance of large-scale atmospheric circulation modifications on tropical cyclone climatology. After discussing the observed relationships between ocean surface temperature and large-scale convection, and questioning the use of a fixed temperature threshold in the diagnosis of tropical cyclone frequency, we propose a modification to the YGP consisting in replacing the thermodynamical potential by a term proportional to the convective precipitation computed by the GCM. For the simulation of the present climate this modification affects only marginally the geographical distribution of tropical cyclone genesis, but for the doubled CO2 case, the modified YGP diagnoses a more limited increase in TC genesis in the Northern Hemisphere and a small reduction in the Southern Hemisphere, which seems in better agreement with other recent modelling studies with high resolution climate models (Bengtsson et al., 1996). We conclude that the modified YGP based on convective precipitation could serve as a useful diagnostic of tropical cyclone genesis, and should be tested in simulations with other GCMs.

    Notes: Abstract Background:Patients with advanced solid tumors may be includedin phase I clinical trials. In such studies, the benefit expected is generallylower than the likelihood of toxicity and may even be non-existent if thepatient's life expectancy is too short. This study was performed to identifyprognostic variables for toxicity and survival in patients who participate inphase I clinical trials. Patients and methods:One hundred fifty-four patients treated ona phase I clinical trial in our institute were evaluated retrospectively.Univariable and multivariable analyses of patients' characteristics wereundertaken to determine their effects on the probability of grade 3 and 4toxicity and on survival. Results:Grade 3 or 4 toxicity was experienced by 56 patients(36%): dosage level at entry (P 〈 0.001) and age over 65years (P = 0.03) were independently associated with the risk oftoxicity. Median overall survival was 5 months. The multivariable analysisidentified performance status 2 or 3 (P 〈 0.001) and lactatedehydrogenase levels greater than 600 UI (P 〈 0.001) asindependent adverse prognostic variables for overall survival. Using these twoparameters, we determined a prognostic index which allowed us to discriminatethree risk groups of patients with an observed median survival of 8.5, 4.5 and1.5 months, respectively. Conclusions:Subgroups with different survival expectancy can beidentified among patients who are eligible for phase I clinical trials. Ifconfirmed, the proposed prognostic model may be useful for therapeuticdecision making in palliative oncology.

    Notes: Abstract This study evaluates histological response, long-term outcome, and toxicity in an intensive chemotherapy program given before surgery. Patients and methods: Sixty-two patients (39 males, 23 females; median age 14) with biopsy, chest computerised-tomography, technetium bone-scan and magnetic resonance imaging, were enrolled. Primary localisations were femur (44%) and tibia (26%). Induction chemotherapy involved seven courses of high-dose methotrexate and two courses of HELP (ifosfamide, eldesine (vindesine), cisplatin (platinum)–doxorubicin. After surgery, patients received six courses of high-dose methotrexate and two courses of HELP–doxorubicin. Results: Pre- and postoperative toxicities were similar. Fifty-nine patients underwent surgery: histological response was good in thirty-eight patients (64%) and poor in twenty-one (36%). Median follow-up is 57 months (range 30–80), with 77% overall survival and 59% progression-free survival. In a multivariate analysis, age under 10 years is the only prognostic factor that significantly correlates with outcome. Conclusions: This regimen appears to increase histological necrosis, but associates with severe toxicity. Results for patients with less necrosis at surgery are encouraging. Future trials should determine the minimum effective doses to reduce toxicity. New drugs should be added.
