    Notes: Abstract Objective: Inhalation of nitric oxide (NO) can improve oxygenation and decrease mean pulmonary artery pressure (MPAP) in patients with the acute respiratory distress syndrome (ARDS). It is not known whether inhaled NO exerts a similar effect in hypoxemic patients with chronic obstructive pulmonary disease (COPD). Design: Prospective clinical study. Setting: General intensive care unit in Sabadell, Spain. Patients: Nine mechanically ventilated COPD patients (mean age 72±2 years; forced expiratory volume in 1 s 0.91±0.11 l) and nine ARDS patients (mean age 57±6 years; mean lung injury score 2.8±0.1) Measurements and results: We measured hemodynamic and gas exchange parameters before NO inhalation (basal 1), during inhalation of 10 ppm NO (NO-10), and 20 min after NO was discontinued (in basal 2) in the ARDS group. In the COPD group, these parameters were measured before NO inhalation (basal 1), during different doses of inhaled NO (10, 20, and 30 ppm), and 20 min after NO was discontinued (basal 2). A positive response to NO was defined as a 20% increment in basal arterial partial pressure of oxygen (PaO2). MPAP and pulmonary vascular resistance (PVR) decreased significantly, while other hemodynamic parameters remained unchanged after NO-10 in both groups. Basal oxygenation was higher in the COPD group (PaO2/FIO2 (fractional inspired oxygen) 190±18 mmHg) than in the ARDS group (PaO2/FIO2 98±12 mmHg), (p〈0.01). After NO-10, PaO2/FIO2 increased (to 141±17 mmHg, p〈0.01) and Qva/Qt decreased (39±3 to 34±3%, p〈0.01) in the ARDS group. There were no changes in PaO2/FIO2 and Qva/Qt when the NO concentration was increased to 30 ppm in the COPD group. In both groups, a correlation was found between basal MPAP and basal PVR, and between the NO-induced decrease in MPAP and in PVR. The NO-induced increase in PaO2/FIO2 was not correlated with basal PaO2/FIO2. In the ARDS group, six of the nine patients (66%) responded to NO and in the COPD group, two of nine (22%) (p=0.05). Conclusions: NO inhalation had similar effects on hemodynamics but not on gas exchange in ARDS and COPD patients, and this response probably depends on the underlying disease.

    Notes: Catalyzed reporter deposition fluorescence in situ hybridization (CARD-FISH) is apowerful method with a growing number of applications in the quantitative evaluation of microbialpopulations of complex ecosystems. CARD-FISH is an improvement over traditional fluorescencein situ hybridization (FISH) especially suitable for aquatic habitats with small, slow growing, orstarving bacteria, in which the signal intensities of hybridized cells is frequently below detectionlimits or lost in high fluorescence background of dense mineral matrixes. In this work we report thedevelopment of protocols and probes for the identification and quantification of the microorganismsinvolved in the continuous bioleaching of a cobaltiferrous concentrate using a four tank-leachingreactor operated by BRGM. After steady state was reached, samples were taken to identify andquantify the microorganisms present in the each of the tanks used in the process

    Notes: Summary  Mass and cystic lesions of the posterior fossa constitute an important chapter in neurosurgical paediatric pathology. Their histological definition is essential for specific treatment. Different types of cysts, can be found besides gliomas, tuberculomas, vascular malformations, radionecrotic lesions or other masses. This paper summarises the results of an observational, descriptive, retrolective study of a group of 30 children with posterior fossa lesions seen from October 1989 to January 1997 at the Hospital Infantil de México (Children's Hospital of México). All the lesions were approached stereotactically to get biopsies and/or to drain cystic lesions.  Clinical case notes were reviewed detailing different variables. It should be mentioned that in among these variables, all procedures were carried out using general endotracheal anaesthesia and that in spite of having performed the biopsy or drainage in the CT scanning suite, no infectious processes supervened. Tissue or fluid samples were obtained using the above mentioned procedure although the specimen was insufficient for histological diagnosis in one. For this case, a biopsy done afterwards by direct microsurgical exploration revealed an astrocytoma. In the rest, anaplastic astrocytomas were found in five patients, low grade astrocytomas in 11, and one case of each with the following lesions: medulloblastoma, reactive gliosis, germinoma, teratoma, tuberculoma, bacterial abscess, primitive neuro-ectodermal tumour and chronic leptomeningitis with a decreased cerebellar neuronal population. Two biopsies reported normal cerebellar tissue and in the remaining three, acellular fluid from cystic lesions was obtained. In four patients, cystic drainage was considered part of the treatment. Our results indicate that biopsies of masses and fluid drainage of cystic lesions of the posterior fossa are safe and simple methods. The empirical treatment that is sometimes used for these kinds of lesions, as well as for lesions at any other location in the central nervous system, should be abandoned.

    Notes: Cysticercosis is infestation of human tissues by Cysticercus cellulosae, the encysted larva of Taenia solium. An endemic disease in tropical countries, especially in Latin America, an increase is expected in its frequency in Europe because of increasing shifts in population. We report the case of a Spanish woman who had lived for some time in Bolivia and Peru who presented at our Department with a tumoral growth on the mucosa of her lower lip. Clinically, this was interpreted as a mucocele (salivary mucoid ‘cyst’); but histological study of the excised piece revealed, surprisingly, that it was a single–element cysticercosis.

    Notes: Scleroderma is an autoimmune disease characterized by early inflammatory infiltrates followed by fibrosis in the skin and internal organs. CREST is a relatively benign cutaneous variant of scleroderma that features calcinosis, Raynaud's phenomenon, oesophageal dysfunction, sclerodactyly and telangiectases. Glomerulonephritis is a rare association of CREST. We are reporting a patient with CREST who developed glomerulonephritis and had anticentromere and antineutrophil cytoplasmic autoantibodies (ANCA) in her serum.

    Notes: Multinucleate cell angiohistiocytoma (MCAH) was first described by Smith and Wilson-Jones in 1985. It is an uncommon entity but probably underdiagnosed because of lack of recognition by clinicians and pathologists. We report a 47-year-old man with asymptomatic grouped violaceous papules on the dorsum of the hands for 3 years. The histopathological and immunopathological features of our case revealed characteristics of MCAH similar to the initial description of Smith and Wilson-Jones and other reports.
