    Notes: Abstract:  This study used population-based tumor registry data to describe the patterns of adjuvant hormone therapy and to examine the correlates of hormone therapy for women with breast cancer. The study population included 5101 women (age 20 years) who were diagnosed with breast cancer in 1991 through 1997 in the entire state of New Mexico. Overall, 32% of women with stage I, II, or IIIA breast cancer received adjuvant hormone therapy. The likelihood of receiving adjuvant hormone therapy increased with tumor stage at diagnosis. Women less than 50 years of age were significantly less likely to receive adjuvant hormone therapy compared to those age 50 to 54 years, but there was no significant difference in the use of adjuvant hormone therapy for women age 55 years and older. The use of adjuvant hormone therapy was influenced by hormone receptor status and lymph node status. Patients who received adjuvant chemotherapy were also more likely to receive adjuvant hormone therapy than those who did not. The use of adjuvant hormone therapy alone was relatively stable over time and the use of adjuvant chemotherapy alone increased, but the receipt of chemotherapy combined with hormone therapy decreased from 1991 to 1997. There was no significant difference with age in the use of adjuvant hormone therapy among 55-year-old women compared to those age 50 to 54 years, whereas women less than 50 years of age were significantly less likely to receive this therapy. The use of adjuvant hormone therapy varied significantly by tumor stage, lymph node status, hormone receptor status, and the receipt of adjuvant chemotherapy.

    Notes: Abstract Background.Three important events in the history of breast cancer treatment occurred between 1983 and 1995: a large clinical trial, first lady Nancy Reagan's choice of mastectomy and the publishing of an NIH consensus statement. Objective.To assess the effects of these events on use of breast conserving surgery (BCS). Research design.Data from the cohort study of the surveillance, epidemiology and end results (SEER) Program from 1983 to 1995 were divided into four periods: Baseline, Trial, Celebrity, and Consensus. Subjects.Of the women, 169,466 diagnosed with early stage breast cancer in nine SEER areas. Measures.Monthly percentages of BCS. Results.A linear regression model generated a separate intercept and slope term for four time periods, adjusting for demographic characteristics of breast cancer patients. For the Baseline, Celebrity and Consensus Periods, slopes indicated an increasing use of BCS which varied between 0.24% and 0.28% per month. Slopes for these three periods were not statistically different (p = 0.120). In contrast, there was no change in use of BCS during the trial period (p = 0.247). We tested the magnitude of discontinuity between periods. At the beginning of the trial, celebrity and consensus periods, there were increases in BCS of 5.54% (p 〈 0.001), −3.55% (p 〈 0.001), and 2.37% (p 〈 0.001), respectively. Conclusions.The use of BCS was substantially affected by the reports of a clinical trial of BCS and by celebrity action. These effects were abrupt but transient. The NIH consensus statement stimulated a small change in use of BCS and may be an important intervention for maintaining the increasing trend in use of BCS since the 1990s.

    Notes: Abstract Objectives: To determine any changes in usage of axillary dissection over time from 1983 to 1993, and to characterize those patients who do not receive axillary node dissection. Methods: A total of 129,946 female patients receiving cancer-directed surgery for early stage breast cancer, who were identified from the SEER (Surveillance, Epidemiology and End Results) program and diagnosed from 1983 to 1993, were included in the study. The surgical treatment and axillary dissection were based on treatment data collected by the SEER during the first course of therapy. Logistic regression analyses were used to assess the effect of patient and tumor characteristics on the likelihood of not receiving different types of treatment. Results: The percentage of women not receiving axillary dissection has increased over time, from 14.4% in 1983 to 16.8% in 1993. Nearly a quarter of women undergoing breast conserving surgery for local stage breast cancer receive neither axillary dissection nor radiotherapy. Older, unmarried, and non-white women and those with very small (〈0.5 cm) or very large (〉=4 cm) tumors were most likely to receive neither axillary node dissection nor irradiation. Among those not receiving axillary dissection, 42% had tumors 1–3 cm in size and were not receiving radiation therapy. Conclusions: There is an apparent divergence between the type of patient who has been recommended as an appropriate candidate for avoiding axillary dissection (those with small tumors and those who will receive adjuvant therapy) and those who are actually not receiving axillary dissection in the community.
