    Description / Table of Contents: Zusammenfassung An Hand von 4 eigenen Fällen und 76 Fällen aus der Literatur wird eine zusammenfassende Übersicht über das cerebral bedingte Gigantismus-Syndrom gegeben. Das Syndrom hat keine typischen pathognomonischen Zeichen, aber das gleichzeitige Auftreten von Gigantismus mit abnormaler Armlänge, typischer Physiognomie, beschleunigtem Knochenalter und unterschiedlich starker psychomotorischer Retardierung erlauben eine Differentialdiagnose.
    Notes: Abstract The cerebral gigantism syndrome is reviewed; 4 own cases are reported and are analysed together with 76 cases from the literature. Cerebral gigantism has no pathognomic signs but can be separated from other conditions by the simultaneous occurrence of gigantism with abnormally great arm span, a special physionomy, advanced bone age and a variable degree of psychomotor retardation.

    Notes: Abstract A Turkish girl presented with a history of fever, diarrhoea, convulsions, recurrent infections and failure to thrive from the age of 5 months. Megaloblastic anaemia was present and profound folate deficiency was evidenced in plasma and in CSF. Treatment with oral folic acid cured the anaemia, diarrhoea and infections but failed to prevent convulsions and the appearance of mental retardation and cerebral calcifications. Loading tests with folic acid and its derivatives led to the conclusion that the folate deficiency was caused by a defect in folate transport both across the gut and the blood-brain barrier. Low plasma concentrations of methionine prompted a therapeutic trial with methionine associated with vitamin B12 and folic acid that spectacularly improved the convulsions.

    Notes: Abstract Anthropometric variables were collected in 10230 clinically healthy children under 5 years of age. The children belong to three African tribes living in different environments. The conventional anthropometric methods of screening children nutritionally at risk were used and their validity is scrutinised. It is shown that, to assess nutritional condition, arm muscle area and arm fat area add essential information to data on weight-for-age, weight-for-height and arm circumference-for-age. It is suggested that the choice of anthropometric methods and standards must be adapted to the ecological situation.

    Notes: Abstract Persistent neutropenia and repeated respiratory infections were documented in a girl with glycogen storage disease type Ib. A termino-lateral portacaval shunt resulted in normalisation of the granulocyte counts and disappearance of the recurrent infections. The platelet dysfunction that was apparent before surgery, was also corrected by the shunting procedure. A marked hypochromic anaemia, however, probably caused by a sequestration of iron in the spleen and resistant to therapy, remains a persistent feature in this patient.

    Notes: Abstract A 7 5/12 year-old boy developed pseudoprecocious puberty. The diagnosis of Leydig cell tumour was suggested by clinical and hormonal findings and greatly facilitated by ultrasonographic investigation of the testes. Surgical exploration was in keeping with the diagnosis and the tumour was removed. Orchidectomy was not necessary. It appears that ultrasonography may be very useful in diagnosing a testicular tumour. It facilitates surgical intervention since it permits localization of the lesion which is usually very small and limited to a small part of the testis, as in this case.

    Notes: Abstract From January 1970 to June 1976, 45 children with the haemolytic uraemic syndrome were admitted to our department. They all received heparin in addition to supportive therapy. For the last ten patients heparin was given with dipyridamole (Persantin®). Three children died in the acute stage of the illness giving an acute fatality rate of 6.6%. A fourth patient immediately needed chronic haemodialysis. In the other 41 patients, kidney function only partially recovered in two; they subsequently developed terminal renal insufficiency after 18 months and four years, respectively. The remaining 39 children have been regularly followed for three to eight years. None has been lost to follow-up. With only one exception, they all have shown a favourable evolution with negative urinalysis, normal blood pressure, and an endogenous creatinine clearance within the normal range for age.

    Notes: Summary. The influence of rest versus hard work on fetal growth at the end of pregnancy was assessed prospectively in a population of women who normally work hard during pregnancy. A group of 554 women living in Kalima area (Central Zaire) was admitted for rest in a maternity village for a median duration of 22 days. Self-selection, age, parity, number of living children, socio-economic status, maternal weight and height and gestational age were controlled for. Energy intake estimated in a sample of women in the maternity village and a sample of those staying at home was similar. Protein intake was found to be higher in the resting women. The duration of rest had a strong influence on birth-weight and length in the newborn females but to a lesser extent in the newborn males. There was a 7.5-fold decrease in the rate of low birth-weight in girls when the duration of maternal rest was 〈21 days. In boys the low-birthweight rate remained unchanged. Although this sex difference could not be explained it is concluded that, in developing countries, avoidance of heavy work can help to raise birth weight and perhaps reduce perinatal mortality.
