Summary 1. The current knowledge of histological, histochemical and electronmicroscopic studies of the conduction system are discussed and the normal blood supply of this system is described. 2. The types of complete interruption of the AV-transmission are classified etiologically into experimental, congenital, traumatic, surgical and acquired non-surgical heart block. The most frequent heart block is the acquired non-surgical heart block. Complete heart block is most frequently found between the fifth and seventh decade, and occurs more often in men than in women. 3. Our own studies and reports of the literature indicate that coronary sclerosis, especially arteriosclerosis of the intramural branches of the coronary arteries is the predominant lesion in the development of complete AV-block. Examination of the conduction system in such cases revealed diffuse and focal fibrosis of the AV-node, the bundle ofHis and of both bundle branches. The observations in the conduction system have been supplemented by planimetric measurements of the connective tissue of the myocardium using the integrational table (Leitz). It was found that in complete heart block there is a significant increase of the connective tissue in the myocardium of both the right and left ventricle and in the septum indicating disturbances of the coronary blood supply. 4. The interpretation ofLenègre andZoob that 50–60% of all cases of complete heart block are due to idiopathic, primary, essential or so-called primitive degeneration of the conduction system is not confirmed by our studies. We have only encountered one such case among 15 cases carefully studied in which the heart of a 22-year-old woman showed fibrosis limited to the conduction system and in which there were no abnormalities of the myocardium or coronary vessels. 5. Other causes of acquired complete heart block are described such as inflammatory diseases of the myocardium and endocardium, calcification of the anulus fibrosus, Chagas cardiopathy, osteitis deformans, Hodgkin's disease and tumors of the heart.

Zusammenfassung 1. Die Ergebnisse der feingeweblichen, elektronenoptischen und histochemischen Untersuchungen am Reizleitungssystem sowie die normale Gefäßversorgung dieses Systems werden beschrieben. 2. Die Formen der kompletten Unterbrechung der atrioventriculären Überleitung werden nach ätiologischen Gesichtspunkten eingeteilt in einen experimentellen, angeborenen, traumatischen, chirurgischen sowie erworbenen, nicht chriurgischen Herzblock. Die häufigste Form ist der erworbene, nicht chirurgische AV-Block. Der komplette Herzblock wird am häufigsten bei Patienten zwischen dem 50. und 70. Lebensjahr beobachtet; Männer erkranken häufiger als Frauen. 3. Unsere eigenen Untersuchungen und die Ergebnisse der Literatur lassen die überragende Bedeutung der Coronarsklerose, insbesondere der Arteriosklerose der intramuralen Äste der Coronararterien, für die Entstehung des erworbenen Herzblocks erkennen. Am Reizleitungssystem werden diffuse und herdförmige Fibrosen im Bereich des AV-Knotens, des Hisschen Bündels und an beiden Tawara-Schenkeln beobachtet. Die Untersuchungen am Reizleitungssystem haben wir durch ausführliche planimetrische Messungen des Bindegewebsgehaltes der Triebmuskulatur des Herzens mit Hilfe des Integrationstisches (Leitz) ergänzt. Beim kompletten AV-Block ist der Anteil des Stützgewebes sowohl im Myokard der rechten und linken Herzkammer als auch im interventriculären Septum als Folge coronarer Durchblutungsstörungen gegenüber dem normalen Herzmuskel signifikant erhöht. 4. Die Auffassung vonLenegre undZoob, daß in 50 bis 60% der Fälle von komplettem Herzblock eine idiopathische, primäre, essentielle oder sog. primitive Degeneration des Reizleitungssystems vorliegen soll, wurde durch unsere Befunde nicht bestätigt. Wir haben nur in einem einzigen Fall (von insgesamt 15 sorgfältig untersuchten Fällen) bei einer 22jährigen Frau eine auf das spezifische Muskelsystem begrenzte Fibrose beobachtet. Veränderungen der Triebmuskulatur und der Coronargefäße, insbesondere eine Myokardfibrose oder Myokarditis konnten ausgeschlossen werden. 5. Als weitere Ursachen für einen erworbenen kompletten Herzblock werden die entzündlichen Erkrankungen des Herzmuskels und des Endokards, die Verkalkung des Anulus fibrosus, die Chagaskardiopathie, die Osteitis deformans, die Lymphogranulomatose und die Tumoren des Herzens beschrieben.
