    Notes: Abstract Lisfranc fracture dislocations are uncommon foot injuries and can be difficult to diagnose clinically and radiographically. Associated dislocation of the medial cuneiform is rare and also is difficult to diagnose. We present a case of combined Lisfranc and medial cuneiform fracture dislocation; the dislocation of the medial cuneiform was overlooked on initial radiographs. Subsequent CT examination showed the degree of midfoot disruption better than plain radiographs.

    Notes: Abstract The purpose of this study was to review fractures and radiographic abnormalities that are detectable, but often overlooked, on a standard ankle radiographic series. We carried out a retrospective review of 556 consecutive ankle radiographic series obtained between June 1, 1995, and May 31, 1996. From this population, 433 complete ankle radiographic series on patients with suspected trauma were selected. The original radiologist's interpretation was compared to a twostep “gold standard” interpretation, consisting of reinterpretation by a musculoskeletal radiologist with the patient's medical and imaging records at hand, with review of discrepant cases by a consensus panel. Eighteen studies were incorrectly interpreted at the initial reading, yielding an overall error rate of 4.2%. Fifteen of the errors were missed fractures, ankle syndesmotic widening, or incorrect classification of old fractures as acute. The rate for this type of error was 3.5%. The most commonly missed fractures were of the talus (4 patients), followed by fracture of the base of the fifth metatarsal and calcaneal stress fracture (2 cases each); tibiofibular syndesmotic injury was missed in 2 cases. Missed fractures on ankle radiographs most commonly involved bones of the hindfoot, especially the talus. It is important to recognize these uncommon and easily missed fractures, so that a modified search pattern may result in improved accuracy of radiographic interpretation.

    Notes: Abstract A normal radionuclide bone image can facilitate distinction between a bone island and significant pathologic processes, especially an osteoblastic metastasis. This distinction becomes more crucial when growth is detected in an isolated sclerotic bone lesion or if a relatively large sclerotic lesion is detected de novo in patients with a known neoplasm. This report presents three patients with isolated bone islands: two with interval growth, the other with a relatively large stable lesion; all showing a normal radionuclide bone image.

    Notes: Abstract Nine cases of congenital aplastic-hypoplastic lumbar pedicle (mean age 27 months) are described. Their data are compared to those of 18 other reported cases (mean age 24.7 years) and the following conclusions are made: (1) Almost exclusively, the pedicular defect in infants and young children is due to developmental anomaly rather than destruction by malignancy or infectious processes. (2) this anomaly, we think, is more common than it is believed to be. (3) Unlike adults, infants and young children rarely develop hypertrophy and/or sclerosis of the contralateral pedicle. (4) Detection of pedicular anomaly is more than satisfying a radiographic curiosity and may lead to discovery of other coexisting anomalies. (5) Ultrasonic screening of the patients with congenital pedicular defects may detect the associated genitourinary anomalies, if present, and justify further studies in a selected group of patients.

    Notes: Abstract Over a period of three years we have seen nine patients with subtalar dislocation, all of whom sustained violent trauma to the region of the ankle and hind foot. All but one patient were males. Clinically a subtalar dislocation resembles a complicated fracture dislocation of the ankle but a definitive diagnosis can only be made radiographically. The mechanism of injury and radiographic features of this injury are discussed.

    Notes: Abstract The radiological diagnosis of Paget's disease of bone is usually straightforward because most cases conform to well-established classic descriptions. Diagnosis becomes more difficult, however, when radiological appearances are not typical or other disease processes mask or alter the behavior of Paget's disease. Examples are presented to illustrate four categories of unusual radiological presentation of Paget's disease: (1) unusual disease progression, (2) massive post-immobilization lysis, (3) metastatic spread to pagetic bone, and (4) vertebral end-plate destruction that mimics infection.

    Notes: Abstract Sixty-five patients with slipped capital femoral epiphysis were studied retrospectively for the development of aseptic necrosis. Seven (11%) patients developed segmental or total aseptic necrosis of the capital femoral epiphysis. Radiographically the changes of aseptic necrosis were detectable as early as four months. The cause for the aseptic necrosis is thought to be due to interruption of the blood supply to the capital femoral epiphysis at the time of slippage or during surgical manipulation.
