    Notes: Abstract Following the entry, the organic foreign material (thorns, twigs, wood fragments, etc.) may become embedded in different structures. Depending upon its proximity to the bone(s), it may or may not result in certain osseous changes. There is a paucity of adequate information about different clinical and pathologic conditions related to organic foreign bodies. This was the stimulus of the present communication. We have reported two additional thorn-induced bony lesions along with reviewing 44 other reported cases. Among these, only 11 cases were reported to have positive radiographic findings (eight thorn-induced, two wood fragment-related, and one due to a twig.) Because of their clinical, laboratory, and pathologic similarity to other synovitides, three cases of tenosynovitis and 16 cases of infective and noninfective synovitis are among the group under review. Geographic distribution of thorns so far reported is illustrated and the pathophysiologic possibilities are discussed.

    Notes: Abstract Four cases of candidal arthritis and osteomyelitis are presented and 17 others are reviewed from the literature. The clinical and radiographic aspects of the entity are discussed in detail and compared with those of bacterial septic arthritis. The role of predisposing factors are elaborated. An increase in the incidence of the disease is anticipated and the diagnostic difficulties are enumerated.

    Notes: Abstract Nine cases of congenital aplastic-hypoplastic lumbar pedicle (mean age 27 months) are described. Their data are compared to those of 18 other reported cases (mean age 24.7 years) and the following conclusions are made: (1) Almost exclusively, the pedicular defect in infants and young children is due to developmental anomaly rather than destruction by malignancy or infectious processes. (2) this anomaly, we think, is more common than it is believed to be. (3) Unlike adults, infants and young children rarely develop hypertrophy and/or sclerosis of the contralateral pedicle. (4) Detection of pedicular anomaly is more than satisfying a radiographic curiosity and may lead to discovery of other coexisting anomalies. (5) Ultrasonic screening of the patients with congenital pedicular defects may detect the associated genitourinary anomalies, if present, and justify further studies in a selected group of patients.

    Notes: Abstract Over a period of three years we have seen nine patients with subtalar dislocation, all of whom sustained violent trauma to the region of the ankle and hind foot. All but one patient were males. Clinically a subtalar dislocation resembles a complicated fracture dislocation of the ankle but a definitive diagnosis can only be made radiographically. The mechanism of injury and radiographic features of this injury are discussed.

    Notes: Abstract Two cases of primary infantile hyperparathyroidism (PIH) are reported. In both cases the diagnosis was initially suspected from chest radiographs which were obtained to assess the etiology of fever and respiratory distress in one case and heart murmur in another. The first case responded well to subtotal parathyroidectomy. The second case had many unique features. (1) She never became overtly symptomatic. (2) She displayed a constellation of findings that are not yet emphasized. (3) Her indisputable radiographic findings of hyperparathyroidism vanished spontaneously by two months of age, whereas her biochemical alterations have persisted up to now, 21/2 years after birth. (4) Three members of her family have subclinical hyperparathyroidism (elevated serum parathormone, hypercalcemia, and hypophosphatemia). Our review of 19 more cases showed that PIH has no specific clinical symptoms and/or signs. Of the laboratory findings, hypercalcemia was most consistantly encountered. The radiographic finding, although not identical to those described in hyperparathyroid adults, had the greatest diagnostic specificity. The disorder carried a grave prognosis if not diagnosed promptly and managed surgically.

    Notes: Abstract A case of tropical pyomyositis in a temperate climate is reported and 29 more cases are reviewed from the literature. Because of its rarity and deceiving clinical presentation, the disorder may go unrecognized for weeks in nontropical regions. The role of various imaging modalities in timely and accurate diagnosis and nonsurgical management of this disorder is discussed.

    Notes: Abstract Traction on wrist and metacarpophalangeal joints during radiography produces ‘vacuum phenomenon’ due to the release of intra-articular gas. The resultant pneumoarthrogram clearly portrays the articular cartilage. Manual traction radiography of the wrist and the metacarpophalangeal joints has the following potential uses. 1. Detection of joint effusion; increased interosseous space during traction without release of intraarticular gas is indicative of joint disease with effusion. 2. Separation of the carpal bones; the traction separates the carpal bones and allows evaluation. 3. Differentiation of arthralgia from arthritis. 4. Evaluation of midcarpal, radiocarpal compartments, and the metacarpophalangeal joints can be performed before the secondary ossification centers are ossified. 5. Evaluation of the integrity of the joint cartilage.

    Notes: Abstract Two cases of infantile cortical hyperostosis are presented in the hope of eliciting word of increased incidence seen by others. These two children are cousins born within 24 h of each other in separate cities, and their disease followed similar clinical courses. Their 14-year-old maternal uncle appeared to have had symptoms identical to those of Caffey's disease at three months of age. The apparent immunity of premature infants to infantile cortical hyperostosis is a previously unnoticed feature that we have come across in reviewing the literature. In analyzing the many theories proposed by different investigators as the pathogenesis of Caffey's disease, the following conclusions are reached. I) Multiple causal factors may have to combine to produce Caffey's disease. II) There may be a spectrum of Caffey's diseases with different pathogeneses. III) Should the incidence of infantile cortical hyperstosis increase, all the previously suspected causal factors must be reexamined.

    Notes: Abstract Two infants who had obstructive jaundice due to congenital stenosis of the ampulla of Vater improved dramatically with sphincteroplasty. In one, the jaundice was complicated by hepatocellular dysfunction and mild biliary cirrhosis. Only a few cases of such an anomaly have been reported, mostly with different modes of clinical presentation.
