    Notes: Summary A monoclonal antibody (Am-3) was produced against senile plaques in the brain of a patient with Alzheimer's disease. Am-3 was reactive with senile plaques of typical, primitive and diffuse type not only in the brain used as immunogen, but also those in the brain of 15 out of 25 autopsy cases of the aged people. Moreover, Am-3 was also reactive with granular materials of various sizes scattered in the 1st, 3rd and 4th layers of the cerebral cortices of the cases with severe dementia. Am-3 was also reactive with vessel wall of the congophilic angiopathy. By immunoelectron microscopic examination, Am-3 was positive with amyloid fibril in the core and crown of senile plaques, and in the congophilic angiopathy.

    Notes: Aims:  The pathogenesis of breast carcinoma in very elderly women is of interest, because oestrogen levels are likely to be extremely low during the development of the disease. In an effort to understand the pathogenesis of breast carcinoma in these women, this study was undertaken to compare the histological patterns and hormone receptor status of breast carcinomas arising in very elderly and younger women.Methods and results:  Thirty-seven breast carcinomas from women over the age of 85 years at the time of their operation were examined histologically and compared with those from a large group of premenopausal women. The proportions of mucinous carcinoma and apocrine carcinoma were significantly greater in older women. The expression of steroid hormone receptors was studied immunohistochemically. Androgen receptor-positive carcinomas were significantly more frequent among older women, whereas progesterone receptor-positive carcinomas were significantly less frequent. There was no statistically significant difference in oestrogen receptor-α or -β expression between the tumours from both groups.Conclusion:  Breast carcinomas in women over the age of 85 years have a different morphological spectrum from carcinomas in younger age groups and may have different pathogenesis mechanisms that may be more dependent on androgen and androgen receptor interaction. Differences from the results of the other studies are discussed.

    Notes: Summary The function of cilia present in the tubotympanum plays an important role in the pathogenesis and chronicity of otitis media with effusion (OME). We used a photoelectric method to examine the ciliary activity in the eustachian tube, in the central pharynx, as well as the nasal cavity of normal animals and animals with otitis media. In normal animals, the ciliary activity in the nasal cavity was not different from that in the eustachian tube. In some animals with otitis media, however, there was a significant difference between the ciliary activity in the nasal cavity and that in the eustachian tube. On the other hand, no significant differences were recognized in any given group of animals examined between the level of the ciliary activity in the central pharynx and that in the eustachian tube. Our findings show that the function of the cilia present in the pharynx is a valid index of that in the eustachian tube. Our study further indicates that it may be possible to clinically assess the ciliary activity in the pharyngeal mucosa biopsied from patients with OME to determine objectively the effects of medical treatments of the disease.

    Notes: Summary We have used a tissue culture technique and a photoelectric method to examine the direct effect of lipopolysaccharide (LPS) on the ciliary activity present in the eustachian tube. Since LPS possesses the major part of the biological activity of endotoxin, our results show clearly that LPS deteriorates the ciliary activity in a dose-response fashion: LPS does not deteriorate the ciliary activity up to 168 h if its concentration is 1ng/ml or less; 10 ng/ml LPS can cause deterioration of the ciliary activity with extended exposure (more than 96 h); LPS can cause dysfunction of the cilia rather quickly if the concentration is 100 ng/ml or more. Our results show that the ciliary activity in the eustachian tube under clinical conditions can be affected by endotoxin.

    Notes: Summary Immunohistochemical and clinicopathological studies were performed in 27 autopsy cases with indisputable DIC, which had been selected from 1,800 autopsy cases of elderly people based on the following two criteria; 1. presence of fibrin thrombi in glomeruli, and 2. presence of fresh patchy necrotic foci in myocardium and/or fibrin thrombi in splenic sinuses. A high incidence of liver lesions (22/27) was revealed in autopsy cases with indisputable DIC. The liver lesions could be classified into four groups. Group-I (Central degeneration) was characterized by massive precipitation of fibrin irregularly around the central vein, causing parenchymal damage. Group-II (Central necrosis), showed coagulation necrosis in the cental zone due to circulatory disturbance caused by either shock as a cause of DIC or abrupt cessation of blood flow into the lobules following fibrin thrombus formation in vessels of Glisson's sheath. Both group-I and -II showed a short clinical duration of DIC. Group-III (Sinusoidal thrombosis), showed the presence of fibrin thrombi in sinusoids with mild parenchymal damage and long clinical duration of DIC. Group-IV (No thrombosis), showed neither parenchymal damage nor fibrin thrombi in sinusoids, but a long clinical duration of DIC.

    Notes: Summary We have examined the function and fine structure of the mucociliary system of the eustachian tube in an experimental study of otitis media with effusion induced by X-ray irradiation. Functional examination demonstrated that the ciliary activity was diminished in such a condition, while morphological observations showed pathological findings including compound cilia, vacuolation of ciliated cells and expansion of intercellular space. These findings show that irradiation-induced otitis media with effusion results in impairment of the mucociliary system. As evidenced by these studies, the mucociliary system in the eustachian tube has an important role in the clearance of fluid produced in the tympanic cavity as well as affording improvement in this disease.

    Notes: Summary We have introduced a tissue culture technique in which the ciliary activity of the cultured tubotympanum can be studied. We have found that ciliated cells of the eustachian tube and middle ear lining continued a beating activity during 168 h of observation. The mean loss of activity of the ciliated cells from the eustachian tube or mucosal sites proximal to the tube was 20% or less, while that of mucosal sites distal to the tube was about 40% even after 168 h of cultivation in our culture medium of RPMI 1640. Our technique of tissue culture is available for studying the long-term effects of various therapeutic agents or adverse factors on the ciliary activity in the tubotympanum.

    Notes: Summary We studied the mucosal pathology present in the eustachian tubes of guinea pigs following exposure to sulfur dioxide. Secretory sthenia but with slight deterioration of ciliary activity was observed in the mucosal samples examined. Our data have shown that 24-h exposure to 300 ppm sulfur dioxide does not cause otitis media with effusion in the guinea pig since active ciliary function will prevent any secretions produced from stagnating in the tympanic cavity.
