    Notes: Abstract Methods of parametric imaging of radionuclide angiography using parameters like appearance time, peak time, transit time, height of peak, arterial slope and area of inflow were developed and evaluated regarding their diagnostic meaning in 111 patients suffering from TIA or PRIND and in 30 normal persons. The meaning of these single parameters could shown depended on the specificity of the diagnostic question. Local cerebral blood flow can be estimated most favourably by parametric images of area of inflow whereas transit time is most promising as a diagnostic tool for evaluation of total cerebral blood flow classified with reference to severity of the perfusion disturbance. Appearance time is suited very well to estimation of collateral perfusion. Blood flow in great cerebral arteries could be seen well by non parametric imaging of radioactivity inflow in the brain supplying arterial vessels in the cranial floor. Applying a combination of the parametric images, the sensitivity for detection of disturbances of cerebral blood flow amounts to 0.91. A specificity of 0.88 and accuracy of 0.90 were found. The described combination of evaluation of RNA using various parameters is considered a well suited method for detection of disturbances in local and total cerebral blood flow by means of planar imaging.

    Description / Table of Contents: Summary The objective of this study was to investigate the relationship between the effect of sleep deprivation, recovery and regional brain perfusion in patients with major depression. Regional cerebral blood flow was assessed by 99mTc-HMPAO-SPECT before and after sleep deprivation in fourteen medicated patients. Three of the patients underwent a follow-up measurement after clinical recovery and with an unchanged antidepressant medication. Before sleep deprivation the responding patients had a significantly higher anterior cingulate perfusion than the nonresponding patients, that normalized after sleep deprivation. Cingulate perfusion uniquely differentiated eventual treatment response from non-responders, as perfusion in no other region under study discriminated the two groups. At baseline all patients revealed hypoperfusion in the left prefrontal cortex when compared to the right side, which was not affected by sleep deprivation, whereas prefrontal hypoperfusion was reversible upon remission. These findings are in agreement with previous PET investigations and provide evidence for cingulate and prefrontal dysfunction associated with depression, that are reversible by successful treatment and may represent state markers.
    Notes: Zusammenfassung Im akuten Stadium einer endogenen Depression unterzogen sich 14 Patienten vor und nach einer totalen Schlafentzugsbehandlung einer Hirnuntersuchung mittels 99mTc-HMPAO-SPECT. Zusätzlich wurden erstmals auch 3 dieser Patienten unter unveränderter Medikation im Stadium der klinischen Remission untersucht. Alle Patienten zeigten in beiden Untersuchungen während der Akutphase eine regionale Perfusionsasymmetrie im präfrontalen Kortex mit signifikant niedrigeren Werten linksseitig, die im Stadium der klinischen Remission nicht mehr nachweisbar waren. Nur bei den Patienten, die auf den totalen Schlafentzug mit einer erheblichen Stimmungsaufhellung reagierten, fanden sich vor der Behandlung signifikant höhere Perfusionswerte im anterioren Cingulum als bei den Nonrespondern, die sich nach der Schlafentzugsbehandlung normalisierten. Bei den Patienten, die klinisch nicht mit einer Stimmungsverbesserung reagierten, zeigte sich keine Perfusionsänderung nach dem Schlafentzug. Auch mit SPECT lassen sich funktionelle Beeinträchtigungen im präfrontalen Kortex und im Cingulum während der akuten endogenen Depression nachweisen, die unter einer erfolgreichen Therapie reversible zu sein scheinen und „state marker” darstellen könnten.

    Description / Table of Contents: Zusammenfassung Die Konzentration von Thyreoglobulin wurde im Serum des Herzblutes von 24 Erhängungsfällen bestimmt. Im Gegensatz zur Kontrollgruppe (plötzliche Todesfälle, Tg-Gehalt 22,7 ± 13,5 ng/ml) ist beim Erhängen eine große Streubreite (12 bis über 400 ng/ml) zu erkennen. Die Konzentrationen von über 400 ng/ml konnten bei den Leichen beobachtet werden, die eine inkomplette Erhängungssituation zeigen (Füße hatten Kontakt mit dem Boden bzw. das Erhängen fand in sitzender oder kniender Position statt). Anscheinend hat die Dauer der Agonie Einfluß auf die Höhe des Tg-Gehaltes im Serum. Bei Berücksichtigung pathologischer Erkrankungen der Schilddrüse (Adenome, Carcinome) kann ein hoher Tg-Spiegel im Blut die Erkennung einer vitalen Gewalteinwirkung auf den Hals unterstützen.
    Notes: Summary Thyroglobulin (Tg) was determined in serum of heart blood from the bodies of 24 people who had died by hanging. In contrast to cases of sudden death (Tg 22.7 ±13.5 ng/ml) the Tg values varied over a wide range (from 12 to more than 400 ng/ml). The highest concentrations were observed in cases of incomplete suspension, e.g. hanging in a sitting or semi-reclining posture. We conclude that the duration of the death agony has an influence on the Tg level in serum. Except of diseases of the thyroid gland (carcinoma, adenoma) a high level of Tg in serum may be a helpful adjunct in the diagnosis of mechanical forces to the neck region inflicted before death.

    Notes: Abstract Trivalent radiometal tracers have been used for tumour imaging and metastatic pain palliation. For better understanding their tumour accumulation, basic model studies of uptake of different169Yb complexes into cultured normal and tumour cells were performed. Whereas the uptake of169Yb citrate is strongly dependent on the metabolic activity and is not tumour-cell specific, the uptake of169Yb complexed with aminocarbonic acids (NTA, EDTA, DTPA) does not correlate to the metabolic activities. These complexes are taken up to a greater amount by the tumour cells (by a factor of about 2). Uptake of both complex types leads to a stable association to cellular compounds,169Yb is not releasable by the strong complexing agent DTPA. Protein binding of the169Yb complexes shows great influence on their cellular uptake. The bound proportion is no more available for cellular uptake. The results indicate that i) uptake of169Yb citrate is an active cellular transport process which is not tumour-specific, ii) the169Yb aminocarbonic acid complexes show a weak favouring by the tumour cells, iii) different from earlier acceptions the Yb complexes studied are not taken up by the cells in protein-bound form. The structure of the Yb complex is decisive for its protein binding and cellular uptake.

    Notes: Abstract A simple and rapid method for labelling proteins with211At by means of a 1,4-diaminobenzene link is described. 1,4-diaminobenzene is transformed into the diazonium salt and subsequently reactions of both211At and proteins with the diazonium salt take place simultaneously. For possibly high yields of astatized protein an appropriate temperature of 273 K was found. The results demonstrate the difference between the reaction mechanisms of iodine and astatine with proteins.
