    Notes: Summary Eastern Austrian regional mortality patterns of oral cancer (oral cavity, pharynx and larynx) and oesophageal, lung and urinary bladder cancer were compared to smoker rates and to liver cirrhosis mortality by type of residence: Vienna (1.7×106 inhabitants), middle towns (50 000–100 000 and 10 000–50 000 inhabitants), small towns (2000–10 000 inhabitants) and rural areas categorized by agrarian quota ≦10%, 10%–20% and 〉20%. The study area (Vienna, Lower Austria and Burgenland) covers 23 600 km2 with 3.23×106 inhabitants. In men, liver cirrhosis correlated negatively with smoker rates (r=−0.74, P=0.1). Deaths from oral cancer and oesophageal cancer correlated significantly with deaths from liver cirrhosis (r=0.81, P=0.03; r=0.78, P=0.04, respectively) but not with smoker rates; lung cancer and bladder cancer correlated significantly with smoker rates (r=0.91, P=0.01; r0.83, P=0.04, respectively), but not with liver cirrhosis. In women, similar urban-rural gradients for all parameters resulted in a positive correlation between liver cirrhosis and smoker rates (r=0.59, P=0.22) and a significant correlation of lung cancer with liver cirrhosis (r=0.75, P=0.05). Oral cancer correlated significantly with liver cirrhosis (r=0.83, P=0.02), but not with smoker rates; lung cancer correlated more significantly with smoker rates (r=0.92, P=0.01) than with liver cirrhosis; bladder cancer correlated positively with smoker rates (r=0.70, P=0.12). Geographical distribution of oral and oesophageal cancer in Eastern Austria seems thus to be highly subject to the prevalence of heavy drinking. Sociocultural influences upon the occurrence of these cancers seem to be mediated through drinking habits rather than through smoking habits alone.

    Description / Table of Contents: In many situations where tibial nailing is indicated unreamed nails are now preferred; their use has almost superseded reamed nailing. This development is illustrated with reference to two groups of patients, and the resulting treatment concept is presented in this paper. In a secondary clinical introduction period of this implant at our hospital, within 9 months 40 out of 61 tibial fractures for which nailing was indicated were treated with unreamed tibial nails (UTN). This was the primary treatment in 19 of these cases and a secondary treatment in 21; review of 35 of the fractures was possible after an average of 10 months. According to the AO classification, 85% of these fractures belonged to groups A and B, and the remaining 15% were group C fractures; there was a high incidence of closed and open soft tissue damage: G0 = 6; G1 = 8; G2 = 6; O1 = 6; ¶O2 = 13; O3 = 1. The following complications were observed: 2 haematomas needed to be evacuated (no infections appeared). In 1 case nonpersisting peroneal palsy was observed. Nonunion was observed in 2 cases. In the 33 directly healed fractures the average time taken for consolidation was 15 weeks (12–20 weeks). Over half the patients (65.7%) were found to be free of pain and to have an uninhibited range of movement even during full weight bearing. In 42.8%, however, kneeling was painful, but this should improve after removal of the implant.

Diese Entwicklung wird anhand von 2 Kollektiven nachgezeichnet, und das resultierende Behandlungskonzept für Unterschenkelfrakturen mit „Nagelindikation“ wird vorgestellt. In der 2. Einführungsphase des Implantats an der Abteilung Unfallchirurgie, Chirurgische Universitätsklinik Freiburg, wurden in einem Zeitraum von 9 Monaten 40 von 61 Unterschenkelfrakturen mit Nagelindikation mit dem UTN versorgt, 19 primär und 21 sekundär. Davon konnten 35 Frakturen nach einem durchschnittlichen Intervall von 10 Monaten nachuntersucht werden. Bei der AO-Klassifikation überwogen mit 85% die Frakturen der Gruppe A und B, die restlichen 15% der Frakturen waren Gruppe C zuzuordnen. Das Kollektiv wies einen hohen Anteil an geschlossenen und offenen Weichteilschäden auf: G0: 6; G1: 8; G2: 6; O1: 6; O2: 13; O3: 1. ¶Postoperativ mussten 2 Hämatome ausgeräumt werden (Bakteriologie negativ), ¶Infekte traten nicht auf. In 1 Fall kam es zu ¶einer passageren Peronäusparese, in 2 Fällen zur Ausbildung von Pseudarthrosen. Bei den 33 direkt verheilten Frakturen betrug die durchschnittliche Konsolidierungszeit 15 Wochen (12–20 Wochen). In 65,7% der Fälle wurden bei Vollbelastung Schmerzfreiheit und ein freies Gangbild gefunden. Häufig bereitet eine Einschränkung beim Knien aufgrund von Schmerzen an der Insertionsstelle Probleme (42,8%).
