    Notes: Summary Between 1969 and 1977 in a total of 395 patients severed peripheral nerves were reconstructed by microsurgical technique. In 30% of the cases primary reconstruction was performed, in 22% secondary reconstruction, and in 48% a free nerve transplantation. The best results occurred after direct suturing without tension, the least favorable after secondary reconstruction of sensory nerves. In children results were much better than in adults. The results were better after tidy and clean injuries. They also depended on the length of the nerve grafts. Early postoperative exercise of the operated limb is important.

    Notes: Summary Fournier's gangrene is a rare but life-threatening condition in western countries; it consists of acute subcutaneous necrotizing infection of the genital and perineal region. Predisposing diseases are characteristically those which impair the patient's immunological defense and wound healing. After emergency debridement, there is extensive loss of scrotal and perineal tissue, usually these defects heal satisfactorily by secondary intention, and reconstructive procedures are seldom indicated. Defects of the penis, abdomen or thighs are split skin grafted, while for scrotal reconstruction, various methods are recommended, such as medial thigh fasciocutaneous flaps, bilateral superolateral thigh flaps or bilateral gracilis musculocutaneous flaps. In Fournier's gangrene due to predisposing disease, split-thickness skin grafting in scrotal reconstruction can, in the authors' opinion, achieve excellent results. In addition, it is the least disabling reconstructive procedure in such patients.

    Description / Table of Contents: Zusammenfassung Die Ergebnisse der Wiederherstellung verletzter peripherer Nerven durch direkte Naht hängen neben anderen Faktoren ganz wesentlich von der Narbenbildung im Nahtbereich und der Länge des zu überbrückenden Defektes ab. Je mehr eine Nervennaht unter Spannung steht, desto schlechter wird das Ergebnis. Die endoneurale Narbenbildung läßt sich am besten verhindern durch mikrochirurgische Adaptation einzelner Faszikel oder Faszikelgruppen des verletzten Nerven. Die Nahtspannung läßt sich vermeiden, wenn schon bei Nervendefekten, die noch weit unter der bisher gültigen kritischen Resektionslänge liegen, die Überbrückung durch freie Transplantation körpereigener Hautnerven erfolgt. Die Ergebnisse der Wiederherstellung von bislang 21 peripheren Nerven durch autoplastische Kabeltransplantation unter Verwendung des Operationsmikroskopes werden mitgeteilt.
    Notes: Summary The results of the reconstruction of injured peripheral nerves by direct suture depend among other factors to a great extent on scar formation in the region of the suture and on the length of the defect to be bridged. The more a nerve suture is under tension the worse the result. Endoneural scar formation is best prevented by microsurgical adaptation of individual fascicles or groups of fascicles of the injured nerve. Suture tension can be prevented by bridging with autogenous skin nerve transplantation even defects well below the now accepted critical length. The results of reconstruction of 21 peripheral nerves by autoplastic cable transplantation with the aid of the operation microscope are reported.

    Notes: Summary A report of 1161 cases of pyogenic infection treated from 1960–1972. The right hand was affected more often than the left (57%), and female more often than male patients.     Description / Table of Contents: Zusammenfassung Von 1969-1977 führten wir an 171 Patienten 210 Nerventransplantationen an den oberen Extremitäten durch. Bei den nachuntersuchten Transplantationen konnten bei den gemischten Nerven hinsichtlich der Motorik in über 3/4 der Fälle, bei der Rückkehr der Sensibilität in 65 % gute bis sehr gute Ergebnisse erzielt werden. Bei rein sensiblen Nerven war oft nur eine Schutzsensibilität zu erreichen. Bei Kindern waren die Ergebnisse wesentlich günstiger als bei Erwachsenen. Eine frühzeitige Beübung der operierten Gliedmaße, z.B. durch baldige Wiederaufnahme der Berufsausübung, spielt eine wesentliche Rolle in der postoperativen Therapie.
    Notes: Summary From 1969-1977 a total of 210 nerve transplantations were performed in the upper extremities of 171 patients. The follow-up examinations showed good or very good motor function in 3/4 of all cases with mixed nerves. In 65 % of these the nerve recovery of sensory function was also good or very good. For sensory nerves alone, only protective sensitivity could be achieved in most cases. In children the results were much better than in adults. Early exercise of the limb after surgery, e.g., resuming work as soon as possible, is important in postoperative therapy.

    Description / Table of Contents: Zusammenfassung Die ineinander verwobenen physiologischen, morphologischen und biochemischen Vorgänge bei der Wundheilung und deren Störungen wer. den kurz dargelegt. Sie zu kennen und ihnen begegnen zu können ist heute eine Voraussetzung für die optimale Behandlung offener Handverletzungen. Sie finden Berücksichtigung in der Indikationsstellung der Vorbereitung des Patienten, der Wundversorgung selbst und auch während der Nachbehandlungsphase. Da es sich bei Handverletzungen in überwiegender Mehrzahl um gesunde junge Menschen handelt, stehen für den normalen Ablauf der Wundheilung, die Beurteilung des Lokal-befundes und die Wundbehandlung im engeren Sinne mehr im Vordergrund als bei ausgedehnten Verletzungen großer Eatremitätenabschnitte, bei chirurgischen Erkrankungen innerer Organe oder großflächigen Verbrennungen. In den Jahren 1965–1969 kamen an der Chirurgischen Klinik der Universität Erlangen 11196 Patienten mit Verletzungen oder Erkrankungen der Hände zur Behandlung. Bei 12,85% (1439 Patienten) wurden Infektionen beobachtet. Bei 5,41% waren diese Infektionen nach offenen Verletzungen (545 Patienten) oder nach aseptischen Operationen (61 Patienten) aufgetreten.
    Notes: Summary Physiological, morphological and biochemical processes in wound healing and their complications are discussed. Everyone, who is concerned with handsurgery should be familiar with these facts, because they are essential for the indication to the operative procedure, the operative technique itself and the postoperative management. The majority are young and healthy people, the local findings and local treatment are under these circumstances much more important than in cases of large wounds of the upper and lower limbs or in cases of surgical treatment of abdominal lesions or large burns. From 1965–1969 1196 patients were treated at the Surgical Department for Hand Surgery of the University Erlangen-Nürnberg. In 12,85% (1435 patients) infections have been observed. In 5,41% these infections followed open wounds (545 patients) or aseptic operations (61 patients).

    Notes: Abstract In the field of turbomachinery special attention is devoted to the dynamic behavior of the blading, because from time to time vibrations are transferred to the machine. Numerical calculations of eigenfrequencies and eigenmodes are often made during design, but verification of these numerical processes by experiment is necessary. In addition to several holographic measuring methods, a new method has been developed to determine the vibration modes of blades in general, especially for a rotating blade. This is the object-related laser triggering method (OLT method), which yields interferograms without using an image derotator, thus avoiding special alignment of the derotator axis and machine axis. Some brief remarks are given on the well known numerical method. The experimental equipment is described and the results are discussed.
