    Notes: A chemical vapor deposition method is described for fabricating force microscope cantilevers with single-crystal diamond tips. The (approximately-equal-to)1-μm-diam diamond tips have corner radii of 30 nm, and have been used to study diamond–diamond friction on well-characterized surfaces in UHV. The tip size and orientation can be determined by electron microscopy without altering the surface atomic structure.

The entire spectrum of plastic reconstructive procedures should be available to offer the patient an optimal solution to his problems. Therefore, early interdisciplinary co-operation between pediatric and plastic surgeons is mandatory to plan and perform post-burn reconstruction in children and adolescents.

    Notes: Summary Despite the sophisticated artificial limb devices now available, forequarter amputation following trauma or tumor resection always results in severe impairment of function. To close the extensive post-traumatic or oncologically acquired defects and to improve prosthetic replacement, cover of the thoracic wall should be performed by utilising the whole forearm as an osteomyocutaneous flap. In cases of concomitant serial rib resection, the radius and ulna can serve as stabilizers of the thoracic wall, thus, avoiding a flail chest. Indications, technical details and clinical outcome of three salvage replantations following interscapulothoracic ablation are discussed.

    Notes: Abstract  Transport proteins and acute-phase proteins, including complement factors C3 and C4, immunoglobulin A (IgA), IgM, albumin (ALB), prealbumin (PAB), transferrin (TRF), ceruloplasmin (CER), haptoglobin (HAG), and alpha antitrypsin (AAT), were examined in 29 patients with severe burns with a total burn surface area (TBSA) ranging from 30% to 97% (mean, 50.03%±21.16%). Samples from each patient were obtained at days 2, 7, 14, and 21 postburn and were analyzed using a 3M 300 special automatic analyzing system (Sanofi, France). A population of normal individuals served as controls. Statistical analyses were performed using multivariate and regression analyses. The postburn values of C3, C4, PAB, ALB, TRF, and CER were significantly lower compared to controls (P〉0.01). Slow postburn recovery was observed for C3 and C4, which was negatively correlated with burn severity. TAF, TAB, and ALB values decreased significantly within the first week postburn (P〈0.01); TAF, PAB, and ALB values demonstrated a steady recovery between days 14 and 21 postburn. The recovery was negatively correlated to the severity of the burns. IgA and IgM markedly decreased after 2 days postburn (P〈0.01). After the initial decrease, the levels of IgM slowly increased, whereas the levels of IgA remained depressed for the first postburn week. The levels of AAT rose significantly throughout the entire study period and were positively correlated with the severity of the burns (P〈0.01). This study demonstrates a pattern of certain reactive/acute-phase proteins in the postburn period. It can be concluded that severe burns directly affect the production of serum levels of acute-phase proteins, thereby impairing important physiological functions such as the immune system, protein transportation, metabolism, and repair processes.

    Notes: Abstract  Thibièrge-Weissenbach syndrome is a rare entity of generalized soft tissue calcinosis of the hand associated with scleroderma. Patients present with tumorous deformities of the hand and phalanges which are easily misdiagnosed as rheumatoid arthritis. The diagnosis is confirmed by conventional X-rays. Treatment options are limited to local excision of painful or ulcerated nodules. Multiple operations are frequently required due to the high incidence of local recurrence. Two clinical cases and a review of the etiopathogenesis of the Thibièrge-Weissenbach syndrome are presented.

    Notes: Abstract  Functional results after deep excision and split thickness skin grafts are often limited as a consequence of unstable grafted areas and contractures. In two patients with a full-thickness thermal injury to the lower extremities and one patient with a chronic unstable skin area over the knee, the IntegraTM bilayered membrane dermal substitute was applied. After uneventful dressing changes graft take was complete without infection or other complications. Complete wound closure was achieved in all three patients within 3–4 weeks after the initial operation. The functional range of motion of the involved joints and the skin quality and contour was superior to conventional grafted skin after excision down to fascia and rather more comparable to skin grafts over a tangentially excised eschar. After wound healing, the neodermis was histologically similar to normal dermis. Considering the high incidence of unstable skin and contractures after regular grafting of deeply excised burns, this concept may present a significant improvement not only for the primary but also for secondary reconstructive procedures, with respect to the long-term quality of life for burn patients.

    Notes: Summary Oncologically acceptable excision margins in melanoma surgery are still under discussion. To evaluate the trend towards a more conservative surgical approach and the pattern of defect coverage, 422 patients were studied retrospectively over a ten year period. The incidence of melanoma types, sex related differences in tumor distribution, and procedures for defect coverage were analyzed. There was a significant decrease in excision margins over the study period. This correlated with a steady increase in primary or local flap closure. As far as follow-up results are concerned, there was no higher local recurrence rate after more conservative excision margins and primary/local flap closure.
