    Notes: Summary 701 cases of traumatic lesions of the facial soft tissues and facial bones were treated within 5 years at the Marburg University ENT Clinic. In 68 cases fractures of the anterior wall of the frontal sinus — often in combination with posterior wall-, rhinobasis- and other fractures were found. In all these cases, as early as possible after the trauma, in an often long time consumin procedure, the anterior wall of the frontal sinus was reconstructed immediately. Reconstruction was performed by wiring the osseus fragments using even particles down to a size of one squarecentimeter. In no case, even in cerebrospinal liquorrhoea and open brain damage, any complications — as inflammation or rejection of bone particles — have occurred. The excellent cosmetic results of this immediate reconstruction are lasting now up to 5 years. Bone scintigrams with TC 99 show that these bone particles from the desmocranium undergo a certain degree of transformation and are integrated again into the cranial bones without much resorption.

    Notes: Summary Of 496 patients with laryngeal cancer, 45 developed cancer in another organ. In seven patients, triple primary tumours were detected. Multiple malignant tumours in the region of the upper respiratory and alimentary tracts were frequent, especially noticeable was the combination larynx and lung tumours. The differential diagnosis between primary multiple cancers and metastasis, which is often difficult, shall be discussed. The prognosis of the double cancer is bad, since the second tumours are diagnosed late in the patients, who are usually very old. Of patients, who were free of the first cancers during the five years limit, each fifth later developed a new cancer. This indicates carrying out the postsurgery examination beyond the five years limit.

    Notes: Summary Cancers arising in Morgagni's ventricle are considered a specially insiduous form, which metastasizes early, grows hidden and is diagnosed late. At present the opinion is discussed that some cancers arise within a laryngocele and break through into the larynx secondarly. In order to clarify these questions step serial section investigations on nine typical ventricular cancers and three combinations of laryngocele and carcinoma have been carried out. The ventricular carcinomas showed themselves without exception to be vocal cord carcinomas, arising from the bottom of the ventricle at the superior arcuate line and growing into the ventricle. They are therefore special forms of glottic carcinomas with a preference of one growth direction. In the combination of laryngocele and carcinoma it could also be proved in all cases that the tumors didn't arise from the respiratory epithelium. The reason for the relatively frequent combination of laryngeal and carcinoma remains open.

    Notes: Summary 160 patients were operated on due to injuries, inflammations or tumors of the frontal sinus. Postoperatively there occurred in 23.8% temporary and in 8.1% permanent double vision. Through synoptometric investigations the disturbances of the ocular motility were analysed and separated into several groups. Different reasons for the cause of the motility disturbances are given. The consequence of that is perhaps to change the method of operating.

    Notes: Summary Clinical and pathological review of vocal cord cancers demonstrated that carcinomas of the same size can present completely different behavioural patterns, although given identical treatment. This was the reason why we determined the histomorphological features of 108 subserial sections of non irradiated vocal cord carcinomas according to the evaluation method of Jakobsson et al. This grading system includes four parameters for the tumour cell population: structure, differentiation, nuclear polymorphism, mitosis, and four parameters for the tumour-host relationship: mode and stage of invasion, vascular invasion, plasma-lymphocytic cellular response. Each one has a malignancy score of 1–4. As there is a lack of equivalency between the different features, we modified the method, simplifying it at the same time for better practical application. The defined malignancy index of each cancer of this series is compared to the clinical course. The malignancy indices correspond conspiciously to the prognosis of the individual tumour.

    Notes: Summary Ameloblastomas usually arise in the mandible, very rarely (15–20%) in the maxilla. The tumour is characterized by its slowly infiltrating and destructive growth, a lack of typical roentgenological structures in the upper jaw and the occurrence of multiple recidives after incomplete resections. The histologic structure of the ameloblastoma and some of its variants are described. Because of its histologic and clinical similarity the ameloblastoma must be differentiated from the adenoidcystic carcinoma and the craniopharyngeoma. An evaluation of case reports in literature shows — as well as our own experience — that ameloblastomas of the upper jaw must be operated radically from the beginning like carcinomas. Irradiation can only achieve temporary remission of the tumour.

    Notes: Summary A serie of our observations of so-called “verrucous carcinomas” of the larynx show that these peculiar lesions do not develop true infiltrating growths or metastases, and do not fulfill the criteria of a true carcinoma. Our experiences indicate that these cases can be treated successfully by repeated endoscopic resections. We have found that no aggressive radical treatment — either by surgery or irradiation — is necessary in managing these lesions. Although the etiology of “Ackerman's tumor” of the larynx is still uncertain, we suggest that the lesions develop as the result of a virus infection and propose again the term “verrucous acanthosis” instead of verrucous carcinoma for this disease.

    Notes: Abstract Over the last few years the development of new high-frequency ultrasound equipment has improved the application and resolution of endoluminal ultrasound. Following our in vitro study on the anatomical basis of endolaryngeal high-frequency ultrasound we present the preliminary results of our sonographic examinations during microlaryngoscopy in order to characterize clinical applications. For the present study 38 patients underwent endolaryngeal ultrasound examinations. Laryngeal pathology included vocal fold polyps, laryngeal cysts, chronic laryngitis, Reinke’s edema, epithelial dysplasia and cancer. The ultrasound examination was performed during microlaryngoscopy in a standardized pattern. A newly developed ultrasound apparatus was utilized with 10 MHz and 20 MHz catheters. Site, size and depth of the laryngeal lesion were evaluated. In 23 examinations of laryngeal carcinomas tumor size and infiltration could be measured and involvement of the thyroid cartilage or anterior commissure could be visualized. The exact extension of laryngeal cysts could also be seen. Ultrasound added no additional information to the endoscopic impression of the other laryngeal lesions. Present findings suggest that endoluminal high-frequency ultrasound might supplement microlaryngoscopy in the assessment of certain laryngeal lesions, especially for the evaluation of the size and infiltration of laryngeal tumors and to perform a more accurate preoperative staging without using computed tomography or magnetic resonance imaging.

    Notes: Abstract To our knowledge only a few defined studies have been carried out on laryngeal cysts. These cysts represent a mixed group of benign laryngeal lesions that can cause diagnostic and therapeutic difficulties. The aim of this study was to characterize their histological structure and localizations in the larynx as well as to discuss theories about their genesis. Between 1973 and 1996, 342 laryngeal cysts were treated at Phillips University of Marburg, while from 1990 to 1996, 74 were treated at Justus Liebig University of Giessen. In all, 416 laryngeal cysts were treated by endolaryngeal microsurgery. All clinical charts were reviewed retrospectively and surgical specimens examined histomorphologically. Findings showed that 58.2% of the laryngeal cysts were located in the glottic area and 18.3% in the ventricular folds. The remainder were located on the aryepiglottic fold (2.2%) and interarythenoid region (0.7%). Two congenital cysts were also treated. Approximately 56% of the laryngeal cysts were lined by squamous cell epithelium, 37% by respiratory epithelium and 7% by oncocytic epithelium. In general, the laryngeal cysts were found to be a collection of inhomogenous lesions from different histogenetic origins with diverse symptoms related to their site and size. On the basis of our investigations, a new classification was established concerning the genesis and development of laryngeal cysts by subdividing cysts into congenital cysts, retention cysts, and inclusion cysts.
