    Notes: Summary In a prospective, randomized study 30 post-term pregnant women with low cervical scores were treated by a pericervical application of either 3 ml of 0.9% saline or 3 ml of 0.9% saline containing 1.5 mg PGE2. Twelve patients in the PGE2 group and two in the placebo group were delivered within 24 h (P 〈 0.001). Eight blood samples were taken from each patient and serum PGFM and progesterone levels were measured by radioimmuno-assay. In the women in whom labor started, PGFM serum levels rose progressively and progesterone levels fell consistently as the cervix dilated. After successful induction with PGE2 the variations were more marked than with labor of spontaneous onset. The PGFM-rise after PGE2 treatment must be due to stimulation of endogenous PGF2α production by local therapy.

    Notes: This article addresses the use of meso-tetrahydroxyphenylchlorin-based photodynamic therapy (m-THPC-PDT) to treat recurrent gynaecologic malignancies of the ovary. Photodynamic therapy is an experimental approach in the treatment of neoplasms and results indicate it is a highly tissue selective, relatively simple intervention with few side effects, therefore reducing the overall burden on the patient. Of the three patients involved in the initial study, two were treated solely with photodynamic therapy by laparoscopy, and one underwent additional palliative debulking surgery of metastatic tumours. After a post-operative period of more than two years all three women remained free of relapses.

    Notes: Hot pressing was applied to produce high performance permanent magnets from melt-spun Sm2+δ Fe15Ga2C2 ribbons. The isotropic magnets show densities up to 7.8 g/cm3, coercivities μ0JHC up to 2.01 T (15.9 kA/cm) and energy products (BH)max up to 60.3 kJ/m3 (7.58 MGOe).     Notes: Thermodynamically stable nanocrystalline Sm2Fe15Ga2C2 powders were prepared by intense ball milling of homogenized and coarsely ground ingots, and a subsequent annealing of the powders in an argon atmosphere. Such processing results in magnetically isotropic material with a room-temperature coercivity μ0JHc of up to 1.2 T. Scanning electron microscopy observations suggest a homogeneous microstructure, and combined with Scherrer formula estimations, an average grain size of 〈100 nm has been determined. The coercivity can be preserved at high annealing temperatures when the process is performed at an argon pressure of about 800 mbar. The attempt of a hot-pressing procedure resulted in a magnet with a density of 86% (6.7 g/cm3) of the theoretical density (7.8 g/cm3) and a coercivity of 1.3 T. © 1996 American Institute of Physics.

The concentrations of 13,14-dihydro-15-keto-prostaglandin F2α (PGFM) and oxytocin were measured by radioimmunoassay in the peripheral plasma of 21 women with low Bishop scores in whom cervical ripening and labour were induced with a cervical cap containing 1.5 mg of prostaglandin (PG) E2, left in place for 6 h. Blood samples were taken before and at 3, 6, 9 and 24 h after the cap was applied. Four women (control group) had a cap without PGE2. Labour began in 13 women receiving PGE2, 12 of whom were delivered within 24 h. In these women plasma PGFM rose progressively to levels seen during spontaneous labour, paralleling the changes in cervical dilatation. The increase became significant at 6 h, when cervical dilatation was 4.5 cm (SEM 0.5). Plasma oxytocin also increased significantly while the cap was in place and then decreased. Plasma PGFM and oxytocin did not change in the control subjects, and in the eight women needing further induction of labour the initial rises were transient and not statistically significant.

    Description / Table of Contents: Zusammenfassung Bei 40 Frauen wurde 1,25 mg PGE2 mittels Portiokappe lokal an die Cervix uteri appliziert. In 22 Fällen wurde dabei die Geburt erfolgreich eingeleitet, in 11 Fällen konnte ein priming-Effekt erzielt werden, der am nächstfolgenden Tag eine erfolgreiche Geburtseinleitung mittels Amniotomie gestattete. Drei Patientinnen wurden per sectionem entbunden, in einem Fall wegen Spätdezellerationen bei liegendem Portioadapter. Viermal mußte die Kappe mehrmals angelegt werden. Bei keiner der 40 Schwangeren wurden Nebenwirkungen beobachtet.
    Notes: Summary A total of 40 patients was treated with 1.25 mg PGE2 placed in a cup which was applied to the vaginal portion of the cervix. There were 22 patients who went into labor and in 11 patients the cervix was ripened sufficiently to allow successful induction by amniotomy on the next day. Three patients were delivered by Caesarean section, late decelerations of the fetal heart rate being the main indication in one of them. In four patients the cervical cup had to be applied more than once. No side effects were observed.

    Notes: Summary In a prospective clinical and microbiological study covering 67 prepubertal children the vaginal flora of 31 asymptomatic children was compared to 36 children suffering from abnormal vaginal discharge and/or vulvovaginitis. Vaginoscopy was employed to obtain bacteriological specimens without contamination. Swabs were inoculated into prereduced transportmedia (PORT A CUL, BBL) and cultured aerobically and anaerobically. The most prevalent aerobic organisms were Staphylococcus epidermidis, Enterococci and Escherichia coli; the predominant anaerobic bacteria were Peptococcus and Peptostreptococcus, Veillonella parvula, Eubacteria, Propionibacterium and Bacteroides species. A similar microbiological pattern was found in both groups, although anaerobes, like Peptococci and Peptostreptococci and Bacteroides species, as well as yeasts, like Candida albicans were significantly more frequent in the group with signs and symptoms. Lactobacilli were less common in this group.

    Notes: Diskussion Bis heute blieb der exakte Wirkungsmechanismus der Prostaglandine ungeklärt. Derzeit wird von den meisten Autoren ein direkter Angriffspunkt an der glatten Muskulatur der harnableitenden Organe postuliert. Da die Detrusorkontraktionen nach PGE2-Induktion im Vergleich mit Azetylcholinstimulation schwächer imponieren, aber wesentlich länger anhalten, wird auch vermutet, daß Prostaglandine zur Aufrechterhaltung eines für die komplette Blasenentleerung notwendigen Muskeltonus verantwortlich sind. Dieser Wirkungsmechanismus könnte auch die Ergebnisse unserer „Pilot-Study” erklären, konnnten doch bereits nach einmaliger PG-Instillation 44% der Patientinnen die Harnblase komplett entleeren - nach Therapiewiederholung waren gar 16 von 25 Frauen (64%) restharnfrei. Eine Interpretation der überraschend guten Resultate bei primär absoluter Harnverhaltung bedarf zusätzlicher Argumente. Offenbar gelingt es, den durch den tagelang in Blase und Urethra liegenden Verweilkatheter gestörten Synergismus zwischen Detrusor- und Spinktermuskulatur zu aktivieren. Von zusätzlichem Vorteil könnte auch die bei Verwendung von Prostaglandin E2 mehrfach beobachtete Relaxierung der Harnröhrenmuskulatur sein. Bei der von uns erprobten lokalen Applikationsform erwies sich vor allem die rasche und einfach zu handhabende Durchführung als vorteilhaft. Auch das Fehlen jeglicher unerwünschter systemischer Nebenwirkungen war erfreulich.

    Notes: Summary The systemic and uterine hemodynamic effects of dopamine were studied in six near-term pregnant Texel sheep. Dopamine was administered by i.v. infusion in doses ranging from 2 to 16 μg per kilogram per minute. After administration of 2 and 4 μg/kg/min, Dopamine produced a statistically significant increase in uterine blood flow (p 〈 0.02). The mean value of the increase was 68.3 ml/min. When doses of 8 and 16 μg/kg/min were given, the responses were variable. Systolic and diastolic arterial pressure remained unchanged up to dopamine dosages of 8 μg/kg/min. Heart rate also showed no significant increase but a trend in this direction was seen. Low-dosages of dopamine could be used in the treatment of patients with placental insufficiency.
