    Description / Table of Contents: Zusammenfassung Das auftreten nichtchromaffiner Paragangliome im Kindesalter ist relativ selten und nur wenig bekannt. Da eine Fehldeutung dieser Tumoren zu ernsten therapeutischen Schwierigkeiten führen kann, werden an Hand von zwei Eigenbeobachtungen und 34 Fällen aus der Literatur (1919–1969) klinische und morphologische Besonderheiten der verschiedenen lokalisierten Paragangliome erörtert. Mit steigendem Lebensalter treten die Tumoren häufiger auf. Eine geschlechtliche Disposition im Kindesalter besteht nicht. Ebenso wie im Erwachsenenalter werden auch bei Kindern neben hormonal inaktiven auch hormonal aktive Tumoren beobachtet, die Katecholamine produzieren und von den Phäochromocytomen klinisch abgegrenzt werden müssen. Die Metastasierungs- und Rezidivneigung im Kindesalter ist wesentlich höher als bei erwachsenen Patienten. Differentialdiagnostisch können durch histologische Spezialfärbungen Hämangiome und Hämangioendotheliome sowie die Glomustumoren des Typ Masson von den nichtchromaffinen Paragangliomen abgegrenzt werden. Auch die Abrikossoff-Tumoren und die alveolären Weichteilsarkome lassen sich in der Regel histologisch und histochemisch von nichtchromaffinen Paragangliomen unterscheiden. Die Therapie der Wahl besteht trotz einer relativ hohen postoperativen Mortalität zur Zeit in der operativen Entfernung der nichtchromaffinen Paragangliome.
    Notes: Abstract The incidence of non-chromaffin paragangliomas in children is rare and therefore hardly ever thought of. However, a false diagnosis of these tumours can lead to severe therapeutical difficulties. For this reason the clinical and morphological features of paragangliomas of varying localisation are discussed, on the basis of 2 own cases and 34 cases reviewed in the literature (1919–1969). The rate of tumour growth increases with increasing age. A sex dependence was not observed. As in adults, hormone-active and inactive tumours are also observed in children. These tumours produce catecholamines and must therefore be distinguished clinically from phaeochromocytomas. Metastases and recurrences are more frequent in children than in adults. Haemangioma, haemangioendothelioma, and the Masson glomus-tumour must be considered in the differential diagnosis; they can be histologically excluded by special staining methods. In most cases Abricossoff-tumour and alveolar soft part sarcomas can also be distinguished by histological and histochemical methods from non-chromaffin paragangliomas. At present the therapy of choice is the surgical removal of non-chromaffin paragangliomas, although there is a high frequence of post-operative mortality.

    Description / Table of Contents: Zusammenfassung. Hämangiome des Gastrointestinaltraktes sind im Kindes- wie im Erwachsenenalter sehr selten. In der vorliegenden Arbeit wird ein Kind mit uncharakteristischen abdominellen Schmerzen und diagnostisch nachgewiesener Stenosierung im terminalen Ileum vorgestellt. Bei der Laparotomie zeigten sich ausgedehnte hämangiomatöse Veränderungen in der Ileocöcalregion. Im Resektat wurde histologisch ein hamartomatöses venöses Hämangion nachgewiesen. In der Diskussion wird auf die Problematik der Klassifizierung und Diagnostik solcher Veränderungen eingegangen.
    Notes: Summary. Hemangiomas of the gastrointestinal tract are very rare in both children and adults. The following case history describes a child with uncharacteristic abdominal pain and stenosis of the terminal ileum. Intraoperatively we found an enlarged hamartomatous alteration of the ileocecal region. The histological analysis showed a hamartomatous venous hemangioma. Problems concerning classification and diagnosis are discussed.

    Notes: Zusammenfassung Wir stellen einen 49jährigen Patienten vor, der nach einer Auslandsreise an einer Q-Fieberinfektion mit atypischem Verlauf erkrankte. Beim Q-Fieber handelt es sich um eine Infektion durch Coxiella burneti, einer Rickettsienart. Während die Erkrankung üblicherweise unter dem Bild eines fieberhaften Infektes (oft verbunden mit einer interstitiellen Pneumonie) verläuft, kann gelegentlich doch die Leberbeteiligung ganz im Vordergrund stehen. Auch unser Patient litt an einem unklaren, hochfieberhaften Infekt mit erhöhten Leberenzymwerten. Histologisch fand sich eine granulomatöse Hepatitis mit multiplen Fibrin-Ring Granulomen. Außerdem waren einzelne Granulome auch im Knochenmark nachweisbar. Obwohl Fibrin-Ring Granulome in der Leber nicht pathognomonisch für das Q-Fieber sind, so stellt diese Infektion doch bei weitem die häufigste Ursache für diese Granulomform dar. Daher kann der Nachweis von Fibrin-Ring Granulomen vor allem bei atypischen, apneumonischen Verläufen des Q-Fiebers diagnostisch den entscheidenden Hinweis liefern.

    Notes: Summary Histological and autoradiographical studies were performed on the liver of rats after local thermonecrosis. The results were compared with those after local cryonecrosis. After thermocoagulation (740°C/4 sec) each animal received an i.p. injection of3H-TdR (2.5 µC/1 g body weight). The survival times ranged from 12 hours to 30 days. In the HE stained histological slides the morphological changes and the percentages of the cells in the granulation tissue and in the stripping film autoradiograms the percentages of radio-actively labeled fibroblasts and hepatocytes were determined. After 12–24 hours the thermonecrosis is complete with leucocytic demarcations. Five days later widespread granulation tissue develops in the marginal zone of the necrosis. 20 to 30 days after thermocoagulation small necrotic parts are still seen in the center surrounded by a fibrous scar. Autoradiographically the highest percentages of labeled epithelial and mesenchymal cells are observed between the second and the third postoperative day. Four weeks after thermonecrosis there are still increased cellular proliferative activities. Therefore, it can be stated that the inflammatory resorptive woundhealing processes following thermonecrosis in the liver are maintained much longer than after other physical injuries especially after cryonecrosis.

    Notes: Summary The changes in the DNA-synthesis of lymphocytes in the different zones of the rat thymus were autoradiographically studied with3H-thymidine after focal thermonecroses (740°C/4 sec) on abdominal organs and sham operation (laparotomy). A single thermolesion was performed in separate groups of rats on the liver and spleen, whereas a third group received multiple heat lesions (liver, kidney, stomach wall). A slight decrease of the labeling index occured during the first 36h after the sham operation and was attributed to an unspecific stress effect leading to an increased release of corticosterones. The most remarkable and contrasting result was the observation that after thermonecroses a decrease of the labeling index of thymus-lymphocytes did not happen during the first 48 h but rather a marked increase with a maximum on the fifth day. It was concluded that the stimulated DNA-synthesis in the thymus might be related to a cell mediated immune response against thermocoagulated tissue components or to an immediate T-cell dysfunction, which is known to develop after burns. In both cases the thymus might react with an increased division and maturation of lymphocytes regulated through a feed-back mechanism.
