    Notes: Summary The effective Tween 20 concentration at which 70% hemolysis was achieved in vitro correlated with the plasma vitamin E content of chicks (r=0.85). Addition of catalase, MnCl2, CoCl2 or dithiothreitol in vitro showed significant protection against the hemolysis induced by Tween 20 in vitamin E-deficient chick and kid erythrocytes.

    Notes: Abstract The concentration-dependent effects of copper acting either as an antioxidant or as a prooxidant were examined in vitro using Tween 20-induced hemolysis. When cupric ion concentration was more than 10 μM, free copper(II) acted as a prooxidant; both extensive hemolysis and production of unknown thiobarbituric acid-reactive substance occurred in hamster and pig erythrocytes irrespective of vitamin E status. However, when cupric ion concentration was 2–4 μM in the incubation medium, copper showed a clear antioxidant activity, reducing both hemolysis and malondialdehyde production induced either by diluted peroxide-containing Tween 20 with ascorbic acid and sodium azide in vitamin E-deficient hamster erythrocytes, or by peroxide-containing Tween 20 in pig erythrocytes containing marginal amounts of vitamin E. Copper(II) is taken up by the erythrocytes, where copper(I)-complexes may contribute to the protection of cells with membrane vitamin E against oxidative radical attack.

    Notes: Abstract Facial palsy occurred in 21 (19.6%) of 107 Japanese patients with multiple sclerosis (MS) during a mean follow-up period of 4.3 years. We observed residual signs of facial palsy in five other patients in whom acute onset was confirmed from medical records. Facial palsy began on average 7.6 years after the onset of MS but in five patients (4.7%) was the first symptom of MS, preceding the next MS symptom by 0.5–3 years. Facial palsy was usually associated with other brainstem signs, while two patients showed only facial palsy 1 and 3 years after the onset of MS. Twenty-one (84.0%) of the 25 patients who underwent brain magnetic resonance imaging (MRI) showed brainstem lesions in the pontine tegmentum ipsilateral to the facial palsy. However, the two patients without other symptoms or signs had no apparent causal lesion on MRI, which suggests difficulty in differentiating idiopathic Bell’s palsy from MS- associated facial palsy by MRI, although it has an excellent capacity to detect causal lesions of facial palsy associated with MS.

    Notes: Summary Tissue distribution of type V collagen in normal human skin was studied using an indirect immunofluorescent technique to determine whether type V collagen is present in the interstitium or in the basement membrane. Type V collagen was isolated from the human placenta by pepsin digestion and was purified with fractioning salt precipitations. Sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE) demonstrated that type V collagen contained α1(V) and α2(V) chains, but not the α3(V) chain. Specificity of the rabbit antibodies to type V collagen was assessed using enzyme-linked immunosorbent assay (ELISA) and an immunoblotting method. Antibodies showed no cross-reactivity to other collagens, laminin, and fibronectin. With an indirect immunofluorescent technique, type V collagen was found to be widely distributed throughout the dermis. Intense fluorescent staining was noted in the papillary dermis and adnexal dermis surrounding hair follicles and eccrine glands. The basement membrane of the dermoepidermal junction, skin appendages, and capillaries was not stained. By indirect immunoperoxidase double staining, type V collagen was not found to be deposited on type IV collagen present in the basement membrane. Immunoelectron microscopic studies showed that type V collagen was not located in the basal lamina. These results suggest that type V collagen is distributed in the interstitium, but not in the basement membrane of normal human skin.

    Notes: Abstract Objective: Patients with heart failure have abnormal neurohormonal regulation during orthostatic stress, and abnormal arterial baroreflex function. This study investigated the effects of alacepril, a new angiotensin-converting enzyme inhibitor with sulfhydryls, on changes in neurohormonal factors during tilt and on the arterial baroreflex control of heart rate. Methods: Plasma concentrations of noradrenaline, adrenaline, renin activity, angiotensin II, and atrial natriuretic peptide were measured at supine rest and after 30° head-up tilt with measurements of central venous pressure and cardiac dimensions in seven patients with congestive heart failure (65 years, ejection fraction = 34%). Arterial baroreflex control of heart rate was assessed by phenylephrine bolus. The arterial baroreflex test was re-examined 3 h after oral alacepril (37.5 mg). The tilt and arterial baroreflex tests were repeated 12 weeks after alacepril treatment (50 mg␣·␣day−1). Results: Heart rate, blood pressure, and neurohormonal factors did not differ before and after chronic alacepril, except for a trend toward an increase in renin activity (2.0 vs 4.9 ng · ml−1· h−1). Head-up tilt decreased central venous pressure (−2.5 mmHg) with a decrease in cardiac dimensions in the pre-alacepril phase. These changes were accompanied by increases in noradrenaline, adrenaline, and angiotensin II and a decrease in atrial natriuretic peptide. After chronic alacepril, the increase in noradrenaline during head-up tilt tended to be smaller (84 vs 30 pg · ml−1), with similar changes in central venous pressure (−3.4 mmHg) and cardiac dimensions. Both acute (3.6 vs 4.8 ms · mmHg−1) and chronic (3.6 vs 6.7 ms · mmHg−1) alacepril treatment was associated with a trend towards an increase in the arterial baroreflex control of heart rate. Conclusion: These results suggest that treatment with alacepril may cause a reduction of sympathetic activation during orthostatic stress and may enhance arterial baroreflex function in patients with mild to moderate heart failure.

    Notes: A primary adenoid cystic carcinoma of the skin is reported. Light microscopy revealed pseudocysts. PAS-positive basement membrane and true glandular lumen, which in aggregates are specific for adenoid cystic carcinoma. Perineural invasion was also observed. Ultrastructural examinations revealed three types of cystic spaces; pseudocysts, true glandular lumens and intercellular spaces. Enzyme histochemical examinations showed positive reactions for eccrine enzymes, including phosphorylase and succinic dehydrogenase and negative for apocrine enzymes. Immunolocalization of collagens and laminin revealed that basement membranes of the pseudocysts involve Type V collagen as well as Type IV collagen and laminin.

    Notes: Six patients with macular amyloidosis were investigated by electron microscopy and various morphological changes in fibroblasts were identified. Many cells, which seemed to be macrophages by light microscopy, proved to be fibroblasts. The digesting action of fibroblasts was well developed and many melanosomes were taken into the cells making the cells resemble melanophages. The fibroblasts extended long, thin and branched cytoplasmic processes to surround the amyloid mass, and the cells selectively extended these into the narrow spaces between collagen and amyloid. The fibroblast showed highly developed endocytotic activity and probable ingcstion of amyloid by pinocytosis or phagocytosis. The developed rough cndoplasmic reticulum contained much protein and was active in secretion.

    Notes: Improved ruthenium red staining has been applied to normal human skin and lichen myxoedematosus skin lesions for the detection of the fine structure proteoglycan aggregates. In 4 samples of normal skin and in a patient with lichen myxoedematosus, the typical fine ultrastructures of proteoglycan aggregates, i.e., many short irregularly-curled filaments of varying lengths attached to long and rather straight filaments of hyaluronic acid were shown. Although lichen myxoedematosus is thought o be due to the faulty formation of proteoglycan aggregates, we suggest a different view, that this improved method of ruthenium red staining is useful for the detection of proteoglycan aggregates, and provides a better preparation for electron microscopy.

    Notes: A case of pretibial myxedema associated with Graves' diseases has been studied by electron microscopy utilizing an improved ruthenium red stain. An organized network of microfibrils with knobs were found within the widened interfibrous spaces. This network was clearly demonstrated by the ruthenium red stain, but only a weak trace could be observed when conventional staining techniques were utilized. Knobs of the network seemed to be composed of ruthenium red-positive granules which surrounded normal fibers and granules of degrading collagen fibers. Many fibroblasts showed dilated rough endoplasmic reticulum containing much amorphous material which indicated a significant role of fibroblasts in the pathogenesis of this disease.
