    Notes: Summary At the end of blastokinesis, serosal epitheliae of 4- to 5-day-old embryos of Locusta migratoria contain an immunohistologically detectable cytosolic protein (Mr ∼240 kDa) which is related to the juvenile hormone carrier-protein in the haemolymph of the same species and which binds tritiated juvenile hormone 3 (JH3) (Kd∼10−8 M). At this early stage of development the corpora allata of the embryo are not yet fully differentiated and do not synthesize JH3 in organ cultures. The earliest detectable JH3 production by corpora allata in isolated heads is on day 6. On the other hand, serosal epitheliae of 4- to 5-day-old embryos produce JH3 in organ cultures, as has been shown by methylation of (10-3H)-JH3-acid to (10-3H)-JH3, and by incorporation of tritiated CH3 from l-(methyl-3H)-methionine into JH3. Isolated heads and abdomens of the embryos used as donors for the serosal preparations did not show methyl transferase activity responsible for JH3 biosynthesis. The serosal cells represent a hitherto unrecognized source of methyl transferase activity and of JH3 production. Degradation of JH3 to JH3-acid was also observed.

    Notes: Summary In our preceding paper, the prevalence and development of retinopathy in 231 Type 1 diabetic children and adolescents were reported to be associated with the duration of diabetes and its age at onset. This paper analyses the relationships between the development of retinopathy and the following factors: age, sex, puberty, blood pressure, insulin dosage, HLA antigens, long-term glycaemic control, and serum cholesterol and triglycerides. All these variables were longitudinally evaluated in a cohort of 322 insulin-dependent patients aged 16.2 ± 4.9 years with diabetes for 7.4 ± 5.2 years, including those 231 subjects whose eyes were examined once or repeatedly by ophthalmoscopy and fluorescein angiography. Long-term glycaemic control from the onset of diabetes to the retinal examination was assessed by both an arbitrary score comprising different parameters and by mean values of glycosylated haemoglobin, and was categorised as good, fair, and poor. With life-table analysis, the overall median individual risk for developing early retinal changes (9.1 years) was found to be significantly influenced by glycaemic control. Minimal lesions developed earlier (8.0 years) with poor control, but later with fair (10.5 years) and good glycaemic control (12.5 years) (p 〈 0.01). Mean HbA1 values below 10% delayed the onset of both incipient (10.8 years) and background retinopathy (16.6 years), while values above 10% advanced it (8.0 and 11.8 years respectively) (p 〈 0.05 and 〈 0.008). By multivariate regression and stepwise discrimination analyses, only 4 out of 14 variables were found to exert significant independent influences on the development of retinopathy: diabetes duration, long-term glycaemic control, serum triglycerides and age. The correlation coefficients with retinopathy cumulatively increased fromr=0.78 for duration of diabetes only tor=0.81 for the sum of these four factors. None of the other variables was found to increase this value any further.

    Notes: Summary The number of different Legionella species is increasing at an impressive rate. In two prospective studies, one involving 110 intensive-care unit (ICU) patients with mainly nosocomial pneumonias and the other 105 patients with community-acquired pneumonias, we investigated the incidence and significance ofLegionella pneumophila and non-pneumophila pneumonias on the basis of 17 different main serogroups. In the first study, 14 ICU patients had 15 (13.6%) Legionella pneumonias, which, in 5 cases (33%), were of non-pneumophila etiology. In the second study, 9 patients with community-acquired pneumonias had 10 (9.5%) Legionella pneumonias. Leading this study were 6L. gormanii infections, followed by 2L. dumoffii and only 1L. pneumophila and 1L. longbeachae pneumonia. Of the total, 22 of 23 patients with Legionnaires' disease suffered from severe basic diseases and complications (acute renal failure, respiratory insufficiency, etc.) predominant among the nosocomial pneumonias. The mortality rate was significant in these patients at 33% (5 patients) in the ICU group and 10% (1 patient) in the group with community-acquired pneumonias. We conclude that non-pneumophila Legionella species should receive more diagnostic and therapeutic consideration in patients with nosocomial or community-acquired pneumonias.

    Description / Table of Contents: Summary The activities of the glykosaminoglykan-hydrolases: Hyaluronidase, β-Glucuronidase and β-Acetylglucosaminidase were measured in fourteen human organs. The highest enzyme-activities were found in thymus, liver, spleen and kidney. Small activities were measured in connective tissue of the skin, in the heart and brain. The activities of the male grown-up organs were higher than the female, while in the infant organs: liver and thymus, the female organs showed the highest activities. Hyaluronidase-activity could not be found in pancreas and brain. The function of the glycosaminoglykan-hydrolases was discussed.
    Notes: Zusammenfassung In der vorliegenden Arbeit wurden in 14 menschlichen Organen (männlich und weiblich getrennt) die Aktivitäten der Glykosaminogly-kano-Hydrolasen: Hyaluronidase, β-Glucuronidase und β-Acetylglucosaminidase gemessen. Die höchsten Enzymaktivitäten fanden sich in Thymus, Leber, Milz und Niere. Geringe Aktivitäten wurden im subcutanen Bindegewebe, im Herz und Gehirn gemessen. Dabei lagen die Enzymaktivitäten in den männlichenErwachsenen-Organen höher als in den weiblichen, während in den untersuchtenSäuglings-Organen: Leber und Thymus, die weiblichen Organe die höheren Aktivitäten aufwiesen. In Pankreas und Gehirn konnte der Nachweis von Hyaluronidase-Aktivität nicht erbracht werden. Die Funktion der Glykosaminoglykano-Hydrolasen wurde diskutiert.

    Notes: Summary The pharmacokinetics of methotrexate (MTX), 7-hydroxymethotrexate (7-OHMTX), 2,4-diaminomethylpteroic acid (APA), folinic acid, and 5-methyltetrahydrofolate (5-MTHF) have been studied during 21 high-dose MTX (HDMTX) infusions (5 g·m−2 in 24 h) with leucovorin (LCV) rescue, a component of the therapy of 5 children with acute lymphoblastic leukemia (ALL). The median steady-state concentration of MTX was 66 μmol·l−1. Three elimination half-lifes were determined for MTX: 1.8 h, 6.4 h and a terminal 15 h. The median systemic MTX clearance was 110 mg·m−2·min−1. The 7-OHMTX level increased during each infusion and a Cmax of 19 μmol·l−1 was achieved at the end. Its initial half-life was 5 h and the terminal half-life was 12 h. Thus, the peak serum concentration ratio of 7-OHMTX to MTX was reached 24 h after the end of the infusion at a median ratio of 8. The MTX metabolite APA was detected in concentrations less than 0.06 μmol·l−1. The median folinic acid level during rescue, 48 h after starting the infusion, was 7.0 μmol·l−1 and 18 h following the last dose of LCV it was 0.44 μmol·l−1, leading to ratios of folinic acid to MTX of 31 and 6, respectively. The median 5-MTHF level during rescue was 0.44 μmol·l−1 with a median ratio of 5-MTHF to MTX of 2. Twenty infusions with 48 h MTX levels of less than 0.5 μmol·l−1 were without marked toxicity. Only one patient with a 48 h MTX concentration of 5.5 μmol·l−1 and a ratio of 5-MTHF to MTX of 0.08 suffered from ulcerating mucositis and septicaemia despite increased and prolonged LCV rescue.
