    Notes: Due to more complex and severe design restrictions, more effective and faster finiteelement analyses are demanded. There are several ways to compute FE analysis efficiently: parallelcomputing, fast iterative or direct solvers, adaptive analysis and so on. One of the most effectiveanalysis ways is the combination of adaptive analysis and multigrid iterative solver, because anadaptive analysis requires several meshes with difference resolutions and multigrid solver utilizessuch meshes to accelerate its computation. However, convergence of multigrid solver is largelyaffected by initial shape of each element. An effective mesh improvement method is proposed here. Itis the combination of mesh coarsening and refinement. A good mesh can be obtained by the method tobe applied to an initial mesh, and better convergence is achieved by the improved initial mesh

    Notes: Homer-1a/Vesl1S, a member of the scaffold protein family Homer/Vesl, is expressed during seizure and serves to reduce seizure susceptibility. Cellular mechanisms for this feedback regulation were studied in neocortex pyramidal cells by injecting Homer-1a protein intracellularly. The injection reduced membrane excitability as demonstrated in two ways. First, the resting potential was hyperpolarized by 5–10 mV. Second, the mean frequency of spikes evoked by depolarizing current injection was decreased. This reduction of excitability was prevented by applying each of the followings: the calcium chelator BAPTA, the calcium store depletor cyclopiazonic acid (CPA), the insitol-1,4,5-trisphosphate receptor (IP3R) blocker heparin, the phospholipase C (PLC) inhibitor U-73122, the metabotropic glutamate receptor (mGluR) antagonist 2-methyl-6-(phenylethynyl)-pyridine (MPEP), and the large-conductance calcium activated potassium channel (BK channel) antagonist charybdotoxin. The small-conductance calcium activated potassium channel (SK channel) blocker dequalinium was ineffective. These findings suggest that activation of mGluR by Homer-1a produced IP3, which caused inositol-induced calcium release and a consequent BK channel opening, thus hyperpolarizing the injected neurons. In slices from rats subjected to electroconvulsive shock (ECS), a comparable reduction of excitability was observed without Homer-1a injection. The ECS-induced reduction of excitability was abolished by MPEP, charybdotoxin, heparin or BAPTA. Intracellular injection of anti-Homer-1a antibody was suppressive as well, but anti-Homer-1b/c antibody was not. We propose that ECS-induced Homer-1a stimulated the same pathway as did the injected Homer-1a, thereby driving a feedback regulation of excitability.

    Notes: Background. Solar (brachioradial) pruritus is an enigmatic condition affecting the lateral aspects of the arm(s) and is primarily seen in Caucasian people living in the tropics or subtropics. Methods. Over a 5-year period we prospectively evaluated 68 patients with solar pruritus seen in an outpatient dermatology clinic. Clinical, biochemical, and microscopic and ultramicroscopic studies were performed. An open-label trial of capsaicin cream application was initiated in 15 patients. Results. An almost equal number of men and women were seen with an average age of 44 years. Brown-eyed and blue-eyed patients were seen with equal frequency. Most patients (86%) had outdoor occupations or avocations. The pruritus was intense, usually bilateral (72%) and invariably affected the lateral (as opposed to medial) aspects of the arm(s) in the absence of primary lesions. The most commonly affected area was the elbow area superficial to the brachioradialis and extensor carpi longus muscles, but extension of the pruritus to include the forearm and/or upper arm was common. Biopsy specimens (stained with hematoxylin and eosin) were normal or showed mild actinic elastosis in 15 patients tested. Ultramicroscopy showed myelin sheath splitting in all four patients studied. This is a nonspecific finding that may also be found in diabetic or alcoholic neuropathy.Ten of thirteen patients completing the capsaicin-cream study found significant relief (itching much improved or gone) after 3 weeks compared with the untreated control arm. Conclusions. The cause of solar pruritus remains unknown, but circumstantial evidence implicates chronic exposure to solar radiation. Capsaicin may offer relief to these patients, but the patients need to be cautioned about the burning sensation that occurs with capsaicin cream.

    Notes: Summary The histological grades of prostatic carcinoma, as defined by Gleason, were correlated with three methods of morphometry in 254 step-sectioned prostates obtained at autopsy. The variables studied were 1) the number of tumours in each prostate; 2) bilaterality and 3) tumour volume. Each characteristic yielded a statistically significant correlation with histological grade. The strongest correlations were obtained using tumour volume. These autopsy studied help to explain the inconsistent results obtained from morphometric analyses of surgical material, and lend support to the Gleason system as a means of predicting tumour behavior.

    Notes: Summary Twenty-nine autopsy cases of paraquat-induced lung injury were studied by histological and immunohistochemical methods. Two stages of injury were identified. In the early stage, the alveolar epithelium degenerates but the epithelial basement membrane remains intact. In the late stage, the epithelial basement membrane is focally disrupted, the mesenchymal cells grow into the alveolar space, and intra-alveolar fibrosis appears. In spite of these pathological changes, the original framework of the alveolar wall is preserved in many areas. Intra-alveolar fibrosis may follow as a consequence of damage to the epithelium without severe damage to the underlying basement membrane, which occurs at the stage of organization. Morphological variants of intra-alveolar fibrosis seem to occur not only to the size of the defect of the basement membrane but also to the difference in the stages of evolution at the time the lesion is studied. The epithelium regenerates along the basement membrane in the early stage of re-epithelialization, but grows over the luminal aspect of intra-alveolar fibrous tissue which has been laid on the remaining basement membrane in the late stage. It is speculated that the regeneration of epithelial cells may develop without any association with the basement membrane when a fibrous tissue covers the original basement membrane.
