    Notes: We present a time-multiplexed interferometer based on Faraday mirrors, and apply it to quantum key distribution. The interfering pulses follow exactly the same spatial path, ensuring very high stability and self balancing. The use of Faraday mirrors compensates automatically any birefringence effects and polarization dependent losses in the transmitting fiber. First experimental results show a fringe visibility of 0.9984 for a 23-km-long interferometer, based on installed telecom fibers. © 1997 American Institute of Physics.

    Description / Table of Contents: Zusammenfassung Bei morbider Adipositas wurde der Einfluß der jejuno-ilealen Bypass-Operation auf den Mineralstoffwechsel untersucht. Mit postoperativer oraler Calcium- und Kalium-Substitution blieben 2 und 5 Jahre nach Bypass-Operation im Serum die Calcium-Fraktionen, Parathormon, Phosphat, Kalium und der Knochenmineralgehalt unverändert, während Serum-Magnesium abfiel. Serum-25-Hydroxyvitamin D war bereits präoperativ niedrig und blieb so postoperativ. Nach der Operation stieg die Urinausscheidung von Oxalat bis in den oberen Normbereich an, während diejenige von Calcium, Magnesium und Citrat stark abfiel. Das Aktivitätsprodukt von Calciumoxalat im Urin blieb unverändert, während das von Brushit abfiel. Da diese Veränderungen im Urin aller Patienten nachweisbar waren, bleiben die Ursachen der postoperativen Steinbildung bei 4 von 19 Patienten zur Zeit unklar. Wir folgern, daß die Empfehlung zur präzisen oralen Calcium-Substitution nach jejuno-ilealem Bypass gerechtfertigt erscheint, weil dadurch langfristige ernste Störungen des Calcium-Stoffwechsels vermieden, außerdem die intestinale Oxalat-Absorption reduziert werden.
    Notes: Summary The influence of jejuno-ileal bypass surgery on mineral metabolism was studied in patients with morbid obesity before operation, and 2 and 5 years after-wards. When calcium and potassium were orally substituted post-operatively, in serum calcium fractions, parathyroid hormone, phosphate, potassium and the bone mineral content remained unchanged, while serum magnesium decreased. Serum 25-hydroxyvitamin D was already low before bypass operation, and did not change thereafter. Post-operatively, the urinary excretion of oxalate rose into the upper normal range, while that of calcium, magnesium and citrate was markedly reduced. The urinary activity product of calcium oxalate rose slightly, but that of brushite decreased. Since these changes were manifest in the urine of all patients, the reasons for the post-operative formation of renal stones in 4 of 19 patients remain unclear at the moment. We conclude that the recommendation for precise oral calcium substitution after jejuno-ileal bypass operation seems justified in order to avoid serious disturbances of calcium metabolism in the long term, and to reduce intestinal oxalate absorption.

    Description / Table of Contents: Abstract Background: Dropout is a neglected area of research; patients with anorexia nervosa are a group with high risk for dropout.Research questions: 1. What are the frequency and type of dropout in disorder-specific, integrated treatment programs for anorexic patients? 2. What are the predictors for dropout? Method: Analysis of 80 prospectively documented, consecutive treatment episodes of anorexic inpatients, based on multimodal clinical and psychometric parameters. Results: Twenty percent of patients terminated on their own initiative, often after reaching target weight; 10% were discharged by the treatment team. At least 43% of dropouts continued with some sort of psychotherapy within the 3 months after discharge. Previous dropout was a predictor for dropout in the present episodes. Patients with no comorbidity had a higher risk of dropout. Patients discharged by the team had personality disorders more often. Patients with comorbid depression stayed in treatment. Conclusions: The form of termination of treatment should be routinely assessed. Previous dropout and comorbidity are indicators of the risk of dropout.
    Notes: Zusammenfassung Hintergrund: Behandlungsabbruch ist ein ungenügend bearbeitetes Thema; die Behandlung der Anorexia nervosa (AN) gilt als besonders abbruchgefährdet.Fragestellungen: 1. Wie häufig und welcher Art sind Therapieabbrüche bei störungsspezifischer integrierter AN-Therapie? 2. Lassen sich Prädiktoren für Behandlungsabbrüche finden? Methode: Analyse von 80 prospektiv dokumentierten, konsekutiven stationären Behandlungsepisoden von Anorexiepatienten (ICD-10) auf der Basis multimodaler Breitspektrummessung. Ergebnisse: 1. 20% der Patientinnen brachen auf eigene Initiative ab, oft erst nach erfolgreicher Gewichtsrestitution; 10% wurden von Seiten des Behandlungsteams frühzeitig entlassen. Mindestens 43% der Abbrecherinnen nahmen innerhalb von drei Monaten wieder therapeutische Angebote wahr. 2. Behandlungsabbrüche waren häufiger bei Patientinnen mit Therapieabbrüchen in der Anamnese. Patientinnen mit regulärem Behandlungsabschluss waren häufiger depressiv. Abbrecherinnen aus eigener Initiative zeigten eine geringere Komorbidität, während teamseitig entlassene Patientinnen häufiger persönlichkeitsgestört waren. Schlussfolgerungen: Die Art der Therapiebeendigung sollte differenziert beschrieben werden. Frühere Behandlungsabbrüche und Art der Komorbidität sind Prädiktoren der Abbruchgefährdung.

    Notes: Summary Bilateral rupture of the patellar ligament in a 49-year-old woman is reported. She sustained a relatively minor trauma and no underlying systemic disease could be found. This is the 14th case of simultaneous spontaneous rupture of the patellar tendons reported in the literature. The diagnostic features were diffuse swelling around the knees, visible and palpable infrapatellar defects, and an inability to extend either leg.

    Notes: Summary Conclusions on results after resection arthroplasty were made by analyzing patient records with regard to completed questionnaires. Sixty-four patients were followed up for 2.1–15.9 years post-operatively. The indications for explantation were septic or aseptic loosening with extensive bony destruction. The average age of the patients was 69.7 years. A specially prepared questionnaire summarized each patient's condition at that time. Twenty-five patients had no discomfort in the operated hip; 25 others reported occassional mild pain during increased strain, and six patients complained of constant pain. All patients were able to walk with aids. In six cases the infection recurred. In summary, we can say that the goal of maintaining an infection- and pain-free hip was achieved in over 90% of cases, but the functional weight-bearing capacity was limited. Resection arthroplasty appears to be a successful treatment; it relieves the patients from pain and infection and can often end a long series of surgical operations.

    Notes: Summary The effect on the urinary excretion of calcium of an oral cation exchange resin with-out phosphorus was studied in healthy control subjects and patients with recurrent calcium lithiasis under out-patient conditions. An immediate reduction of intestinal calcium absorption and urinary calcium excretion was found in five control subjects and in one patient after ingestion of resin, whereas calcium excretion remained unchanged in all other patients during long-term treatment. In addition, signs of mild transitory hyperparathyroidism together with an increase in intestinal calcium transport were observed during treatment. It is suggested that intraluminal binding of calcium ions to the resin leads to substantial changes in calcium metabolism with the result that urinary calcium excretion returns to pretreatment values.

    Notes: Summary The topographical distribution of adrenocorticotropic hormone, growth hormone and prolactin was studied in nine multihormonal pituitary adenomas and two normal adenohypophyses using quantitative immunohistochemical methods. For each pituitary hormone paraffin sections stained by antihormone antibodies were scanned with a light microscope equipped with a projection and scanning device. The number of immunoreactive cells of each square in the grid was counted and transformed into pseudocolors. In this way, the distribution of each hormone was documented. A chi-square test was applied to these distribution patterns to prove randomness. A nonrandom distribution of immunoreactive cells was observed in all pituitary adenomas and normal pituitary glands. Cluster formations of immunoreactive cells were found for each bormone. The clusters were surrounded by halo-like areas of decreasing number of immunoreactive cells from center to periphery. The cluster-halo type of immunoreactive cells was found not only in the adenomas but also in the normal pituitary gland. This may be explained by the similarity of biological processes during cell proliferation either in normal development or in adenoma formation. It can be concluded from our studies that pituitary adenomas in most cases develop by multicentric but not diffuse proliferation of cell clones of different endocrine activity.
