    Description / Table of Contents: Abstract Although still rare, pregnancy and delivery in women with spinal cord injuries is becoming more frequent (Table 2). In 85%–90% of patients with lesions above T6 symptoms of autonomic hyperreflexia (Table 1), especially paroxysmal and excessive increases in arterial blood pressure, may occur. In anaesthesising a 31-year-old paraplegic primigravida with a complete transverse spinal lesion at T4 for an elective caesarean section, no indication of hypertonic cardiovascular dysregulation either intra- or postoperatively was observed after repeated epidural administration of bupivacaine. Distinct intraoperative spasticity of the abdominal wall muscles was, however, not influenced by the dosages selected.
    Notes: Zusammenfassung Es wird berichtet über das geburtshilflich-anästhesiologische Vorgehen und den peripartalen Verlauf bei einer 31jährigen Erstgravida mit traumatischer Querschnittsläsion in Höhe Th 4 und klinischen Zeichen einer autonomen Hyperreflexie (AH). In der 37. Schwangerschaftswoche erfolgte eine primäre Sectio caesarea aus kindlicher Indikation. Hypertone Kreislaufdysregulationen im Rahmen von AH-Attacken wurden unter repetitiver Bupivacainanwendung über einen Periduralkatheter nicht beobachtet, eine ausgeprägte intraoperative Spastik der Bauchdeckenmuskulatur war mit den gewählten Dosen allerdings nicht zu beeinflussen.

    Description / Table of Contents: Abstract. A 25-year-old woman with Kartagener's syndrome was admitted to hospital for laparoscopy because of sterility. Kartagener's syndrome is a rare disorder involving the combination of situs inversus, bronchiectasis and sinusitis. A dynein deficiency leads to ciliary dyskinesia. When general anaesthesia is to be induced in a patient with Kartagener's syndrome the following points must be borne in mind: ascertainment of the preoperative pulmonary status, antibiotic coverage, recognition of dextrocardia, necessity for aseptic techniques because of the possibility of abnormal neutrophil chemotaxis. Anticholinergic and antitussive medications are relatively contraindicated, as are nasal tubes. In the present case an intubation anaesthesia with thiopental, nitrous oxide, enflurane and succinylcholine was carried out; cefoxitin was administered for antibiotic treatment, and the patient made an uneventful recovery.
    Notes: Zusammenfassung. Eine 25jährige Patientin mit Kartagenersyndrom wurde bei primärer Ehesterilität laparoskopiert. Kartagener beschrieb die Symptomentrias Situs inversus, Bronchiektasen und chronische Sinusitis. Für den Anästhesisten sind insbesondere die respiratorischen und kardialen Befunde von Belang. Wichtige Gesichtspunkte sind die präoperative Abklärung der pulmonalen Mitbeteiligung, perioperative Antibiotikaprophylaxe, spiegelbildliche Ableitung des EKGs, weitestgehende Vermeidung nasaler Intubation sowie anticholinerger und hustendämpfender Medikation.

    Description / Table of Contents: Zusammenfassung Das Auftreten zweier Fibrinogenderivate mit höherem Molekulargewicht als dem des Fibrinogens im Nabelvenenplasma asphyktischer Neugeborener konnte in dieser Studie bestätigt werden. Nachdem aus Einzelproben ein Pool gebildet worden war, wurde dieser der Agarose-Gel-Filtration unterworfen, und anschließend wurden die eluierten Proben elektrophoretisch untersucht. Elektrophoresen in 5 Mol Harnstoff deuteten darauf hin, daß das 0,27 Derivat ein Komplex ist, der sich vorwiegend aus untereinander nicht kovalent gebundenen Komponenten zusammensetzt und der in 5 Mol Harnstoff dissoziiert. Möglicherweise entsteht dieser Komplex (Fibrinogen-Fibrinmonomer-Komplex) durch die Wirkung von Thrombin auf Fibrinogen. Das 0,17 Derivat setzt sich vorwiegend aus untereinander kovalent gebundenen Komponenten zusammen. Es könnte sich hierbei um ein Plasminderivat quervernetzten Fibrins (Defekt-Polymer) handeln.
    Notes: Summary The occurrence of two fibrinogen derivatives with a molecular weight higher than that of the parent molecule in cord vein blood of asphyctic newborns was confirmed in this study. Agarose gel filtration and electrophoresis of the eluted fractions were performed from pooled plasma. Electrophoretic studies in the presence of 5 M urea indicated that the 0.27 derivative is a complex formed of predominantly non covalently linked components, which dissociates in the presence of 5 M urea. It might be assumed that this derivative (fibrinogenfibrinmonomer complex) results from the action of thrombin on fibrinogen. The 0.17 derivative consists of predominantly covalently linked components. It might be originated from the action of plasmin on stabilized fibrin (defect polymer).

    Description / Table of Contents: Zusammenfassung Die Häufigkeit intravasculärer Mikrothromben bei perinatal verstorbenen Neugeborenen veranlaßte die folgende Untersuchung. In nativen Plasmaproben und nach Fällung des Plasmas mit 2,5 molarem β-Alanin wurden hochmolekulare Fibrinogenderivate durch PAA-Gel-Elektrophorese und Intra-Gel-Immunpräzipitation identifiziert. Zwei Komponenten, die langsamer als Fibrinogen wanderten, wurden beobachtet. Ihre relativen elektrophoretischen Beweglichkeiten betrugen 0,17 und 0,27 (Fibrinogen: 0,43)×10−5 cm2/V×sec. Aufgrund des Laufverhaltens in der zweidimensionalen Gel-Elektrophorese in Gelen verschiedener Porengröße kann für beide Komponenten ein höheres Molekulargewicht, als das des Fibrinogens angenommen werden. Die beschriebenen Derivate wurden im Nabelvenenblut asphyktischer Neugeborener beobachtet. Ihr Auftreten war mit einem positiven Äthanoltest, einem statistisch signifikanten Abfall der Thrombozyten und des Fibrinogens und mit Zeichen fetaler Asphyxie korreliert.
    Notes: Summary The high incidence of intravascular fibrin deposits in stillbirths and newborns dying in the perinatal period prompted us to undertake the following study. High molecular weight derivatives of fibrinogen were identified in native plasma and in plasma after 2.5 M β-alanine precipitation by PAA gel electrophoresis and intra-gel immunoprecipitation. Two components were observed which migrated more slowly than fibrinogen. Their relative electrophoretic mobilities were: 0.17 and 0.27 (fibrinogen: 0.43)×10−5 cm2/V×sec. Two-dimensional gel electrophoresis in gels of different pore size indicated a higher molecular weight than that of fibrinogen for both components. The derivatives described were found in cord vein plasma samples of asphyctic newborns. Their occurrence was accompanied by a positive ethanol test, by a statistically significant decline of platelets and fibrinogen and by signs of fetal distress.

    Notes: Abstract An optical tomography system that is capable of operating at frame rates of up to 5 kHz has been used to obtain spatially resolved cross-sectional temperature images of a heated round jet. These tomographic images show dynamic details in the evolving vortical flow structures found in the near field of the jet that are consistent with previous studies of low speed jet flow. Reconstructions produced by the system are quantitative temperature distributions of a planar cross section of the jet measuring temperature differences with a spatial resolution of 1.4 mm.
