    Notes: A characteristic feature of the kinetics of nicotinic acetylcholine receptor (nAChR) channels is fast and nearly complete desensitization with a time course between 10 and 100 ms and recovery from desensitization in the range of some hundred ms. In the present study we used a piezo-driven system for ultra-fast solution exchange, analysed the recovery from the fast desensitized state of mouse recombinant adult-type nAChR channels and found no difference to that of embryonic-type channels. By double pulse experiments with application of pulses with a saturating concentration of 1 mm acetylcholine (ACh) with increasing duration of the first pulse and a constant interval between pulses we detected a second slow desensitized state which was entered with a time constant of 2835 ms. Recovery from the slow desensitized state proceeded with a single exponential with a time constant of 16134 ms. The experimental data were interpreted by the addition of a transition from the desensitized state with two bound ACh molecules to a slow desensitized state to the well known circular kinetic scheme of activation and desensitization of nAChR channels. This slow desensitized state might play a role in muscle fatigue or in pathological states like myasthenic syndromes.

    Notes: α-Amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid (AMPA)-type glutamate receptor channels of rat caudate-putamen neurones were studied by ultrafast application of agonists to outside-out vesicles taken from medium-sized spiny neurones in thin slices. Upon application of 10 m m glutamate for 50 ms, fast rising and desensitizing currents were activated. Ten to 90% rise time values were ≈ 0.5 ms. Dose–response studies revealed an EC50 of 0.63 mm glutamate. In double logarithmic coordinates, the curve had a maximal slope between 1.33 and 1.85 at low concentrations, indicating at least two binding sites for glutamate. Rise time increased with low agonist concentrations, whereas desensitization kinetics showed only a weak dependence on concentration. The time constant of desensitization was fitted with one exponential and ranged between 2 and 11 ms, with a mean of 6.19 ± 2.31 ms (n = 239). Following brief glutamate pulses (1 ms) currents decayed with time constants of 2.7 ± 0.23 ms (n = 12). Recovery from desensitization was investigated by double-pulse experiments. Recovery time constants fell in two subgroups with respective mean values of 110.6 ± 14.2 ms (n = 8) and 288.6 ± 33.2 ms (n = 8). By adding low glutamate concentrations to the bath solution, predesensitization of AMPA-type receptors without channel opening could be shown. A 50% reduction in control amplitude was achieved with 5.2 ± 2.1 μm (n = 22) glutamate in the background. We hypothesize a circular reaction scheme with at least two binding sites for glutamate to describe activation, desensitization and recovery from desensitization in rat caudate-putamen neurones.

    Notes: Political context may play a large role in influencing the efficiency of environmental and health regulations. This case study uses data from a 1989 update of the Occupational Safety and Health Administration (OSHA) Permissible Exposure Limits (PELS) program to determine the relative effects of legislative mandates, costly acquisition of information by the agency, and pressure applied by special interest groups upon exposure standards. The empirical analysis suggests that federal agencies successfully thwart legislative attempts to limit agency discretion, and that agencies exercise bounded rationality by placing greater emphasis on more easily obtained information. The 1989 PEL were less significantly related to more costly information, contained “safety factors” for chemicals presenting relatively more ambiguous risks, and the proposed standard stringencies showed evidence of beinginfluenced by vying industry and labor interests.

    Notes: Although relatively little attention has been paid to the question how acute alcohol withdrawal might affect cognitive functions, this factor remains of particular interest because it influences psychotherapeutic treatment during detoxification. The clinical outcome and neuropsychological state of 37 inpatients with alcohol withdrawal was investigated in a randomized single-blind approach. Two different medical strategies [chlormethiazole (CMZ) vs. carbamazepine (CBZ)] in the treatment of inpatients with alcohol withdrawal syndrome were compared. Among comparable groups (related to gender, age, initial alcohol level, severity of abuses, severity of initial withdrawal symptoms such as tremor, perspiration, psychomotor agitation, hallucinations, orientation, intelligence, patient demographics), CBZ is just as potent as CMZ in therapy of withdrawal symptoms (circulatory function, vegetative function, psychomotor activity). Patients in both groups showed initial impairments in some neuropsychological tests (d2, Zahlen-Verbundings test, Beck Depression Inventory, Anxiety Sensitivity Index) with significant improvement during detoxification. Additionally, CBZ-treated patients showed significantly better verbal memory performance during the first days of treatment. Without any addictive potential, CBZ therapy could be very supportive in alcohol detoxification. In addition a higher verbal memory performance state could be favourable for a psychotherapeutic approach.

    Notes: Summary To evaluate the reactogenicity and immunogenicity of aHaemophilus influenzae type b conjugate vaccine (HbOC) and of a tricomponent acellular pertussis vaccine (DTaP) when injected simultaneously into either contralateral arms or into contralateral thighs, 110 infants were enrolled to receive three doses of DTaP at 3, 4, and 5 months and two HbOC doses at 3 and 5 months of age. Administration of either of the two vaccines into arms was associated with significantly more local side effects than administration into thighs. There was no difference in geometric mean concentration (GMC) values for any of the four vaccine antigens between subjects who had been vaccinated into arms or thighs. After immunization, all children had protective antibody titers to diphtheria toxin. While post vaccination the mean anti-tetanus toxoid GMC was ≥ 1.25 IU/ml, there was no significant rise as compared to the GMC before vaccination. GMCs of antibodies against the various pertussis antigens were similar to those observed before with the same DTaP vaccine. The simultaneous administration of DTaP and HbOC was safe and immunogenic irrespective of the site of vaccine administration, but significantly more local reactions occurred when vaccines were injected into arms.

    Description / Table of Contents: Summary Thrombembolic events as paraneoplastic complications in malignant disease account for severe morbidity and mortality in these patients. In some cases disturbance in hemostasis is the first manifestation of a neoplastic process. We report the cases of two patients with cerebral and extracerebral vessel occlusions, in whom epithelial tumors of low malignant potential (borderline-tumors) of ovar and peritoneum were diagnosed later on. In one case the removal of the tumor stopped the coagulation problems. The second patient died a few days after the first symptoms with devastating multiple vessel occlusion. In stroke of unknown aetiology a paraneoplastic process should be kept in mind. The diagnosis is more probable with recurrent thrombembolism in different body regions and when warfarin therapie was ineffective. Thoughtful coagulation studies and a tumor search program is recommended in these cases.
    Notes: Zusammenfassung Thrombembolische Komplikationen treten paraneoplastisch bei malignen Erkrankungen auf und tragen zu Morbidität und Mortalität bei. In einigen Fällen sind Hämostasestörungen die erste Manifestation eines neoplastischen Prozesses. Wir beschreiben die Fälle von zwei Patientinnen mit zerebralen und extrazerebralen Gefäßverschlüssen, bei denen nach der Gefäßmanifestation epitheliale Tumoren mit niedrigem Malignitätspotential (Borderline-Tumoren) an Ovar und Peritoneum diagnostiziert wurden. Paraneoplastische Hämostasestörungen sind bei diesen Tumoren mit per se guter Prognose bisher nicht beschrieben worden. In einem Fall führte die Tumorentfernung zum Sistieren der Gerinnungsstörung. Die zweite Patientin starb wenige Tage nach den ersten Symptomen an den Folgen multipler Gefäßverschlüsse. Bei Schlaganfällen unklarer Ätiologie sollte auch an eine paraneoplastische Genese gedacht werden. Besonders wahrscheinlich ist die Diagnose bei rezidivierenden venösen und arteriellen Gefäßverschlüssen in verschiedenen Körperregionen und bei der Resistenz auf die Therapie mit Vitamin-K-Antagonisten. Eine sorgfältige Gerinnungsdiagnostik und ein Tumorsuchprogramm sind in diesen Fällen zu empfehlen.
