Zusammenfassung Seit 1977 wurden am Radiologischen Institut 79 PTDs bei Verschlußikterus durchgeführt, 45 Patienten wurden in der Chirurgischen Klinik weiterbehandelt. 1. PTD als vorbereitende Maßnahme vor Operation: 29 Fälle. Binnen 10–14 Tagen fiel das Serumbilirubin im Mittel von 15 auf 4 mg · % ab. Damit verringert sich das allgemeine Operationsrisiko. 22mal lag ein maligner Verschluß, 6mal Steinverschluß, 1mal eine iatrogene Choledochusstenose zugrunde. Bei malignem Verschluß wurde 11mal palliativ (biliodigestive Anastomose, Choledochus-Endoprothese) und 4mal kurativ (partielle Pankreatoduodenektomie) operiert. 7mal bestand Inoperabilität. In diesen 7 Fällen wäre den Patienten die explorative Laparotomie erspart geblieben, wäre der inoperable Befund prdoperativ bekannt gewesen. — 2. PTD als alleinige therapeutische Maßnahme: 14mal maligner Verschluß. Kriterien der Inoperabilität waren Re-Verschluß einer bestehenden biliodigestiven Anastomose durch Tumorrezidiv, Tumorkompression der intrahepatischen Gallengänge, extrem schlechter AZ, Operationsablehnung. Benigne Stenosen: 1mal (82 J. ♀) Papillensklerose durch PTD-Katheter aufbougiert; 1mal narbig stenosierte Hepaticojejunostomie pneumatisch aufgedehnt, 2 Jahre rezidivfrei.

Summary Since 1977, 79 PTD were performed in the Department of Radiology; 45 patients were further treated in the Department of Surgery. (1) PTD for preoperative conditioning of patients: 29 cases. Within 10—14 days serum bilirubin levels fell from 15 to 4 mg % mean, thereby lowering the general surgical risk. A malignant obstruction was observed in 22, 6 had common duct stones, and 1 iatrogenic common duct stenosis. The malignancies were treated by palliative (10 biliodigestive anastomoses, 1 common duct endoprosthesis) or curative procedures (4 partial pancreatoduodenectomies). Seven cases turned out to be technically inoperable. In these seven cases, an explorative laparotomy would have been unnecessary, retrospectively, if the inoperable situation could have been assessed preoperatively. — (2) PTD as single therapeutic approach: 14 cases with malignant obstruction. Inoperability was assessed in malignant reocclusion of a biliodigestive anastomosis, tumor compression of intrahepatic bile ducts, extremely poor general condition, or refusal of surgery. Benign obstruction: in a 82-year-old female, the sclerotic papilla was bougied with the PTD catheter; in another case a stenotic hapaticojejunostomy was pneumatically dilated and remains oen after 2 years.
