    Description / Table of Contents: Zusammenfassung Histologische und elektronemikroskopische Beschreibung eines klinisch typischen Falles einer subakuten Einschlußkörperencephalitis. Atypisch sind die laminären Nekrosen im Frontallappen und eine geringgradige Endarteriitis. Elektronenmikroskopisch wurden cytoplasmatische Partikel gesehen, die bis zu einem gewissen Grade Herpes-simplex-Viren ähneln. Auch wurden Kernveränderungen nachgewiesen, die ähnlich denen sind, die bei viralen Infektionen bisher beobachtet wurden.
    Notes: Summary The histological and electron microscopical aspects of a clinically typical case of subacute inclusion body encephalitis are described. Histologically a laminar necrosis in the frontal lobes and some degree of endarteritis were the only atypical features. In the electron microscope cytoplasmic particles were seen which resemble to some degree to Herpes simplex virus. There were also nuclear alterations similar to those observed in some viral infections.

    Notes: Background. Helicobacter pylori lipopolysaccharide (LPS) affects pepsinogen release by a nontoxic mechanism. We hypothesized that this effect was characteristic of the organism and related to the clinical status of the strain.Materials and methods. LPS was isolated from 11 H. pylori strains whose pathogenic profile was known and four other nongastric bacteria. The effects of luminal LPS on guinea pig gastric mucosal pepsinogen release was evaluated using the Ussing chamber technique. CCK-8 (10−9M) was used as a positive control.Results. H. pylori LPS dose-dependently stimulated pepsinogen release with a maximal stimulation at 250 µg/ml (~4500%; p 〈 .001 vs. control). LPS from other Helicobacter or Campylobacter species had no effect on pepsinogen release. ANOVA demonstrated significant differences in the efficacies of pepsinogen release between the 11 clinical H. pylori strains (p 〈 .0001) despite the fact that they were all cagA+ and 90% had the cytotoxic vacA subtype s1. Physical and chemical disruption of the LPS suggested that both the structure and the carbohydrate composition of this molecule may play a critical role in pepsinogen release. Polymyxin B partly (p 〈 .03) inhibited and dephosphorylation completely inhibited (p = .0002) LPS-stimulated pepsinogen release.Conclusion. Pepsinogen release is an innate property of all cagA+H. pylori LPS. The structure of the molecule and composition of side-chains are important in this response which appears to be partially lipid A driven.

    Notes: The relationship between Helicobacter pylori infection and gastric carcinoma remains controversial, especially in the African setting where infection is common, while gastric cancer is perceived to be uncommon, the basis of the so called ‘African enigma’. This discrepancy between infection and the development of disease is commonly attributed to differences in host, environment and bacterial factors. Interest in the bacterial factors has focused on heterogeneity in the so-called ‘virulence genes’.〈section xml:id="abs1-3"〉〈title type="main"〉Aim.The aim of this prospective, case-controlled study was to establish whether H. pylori infection is significantly associated with gastric cancer and to investigate whether gastric cancer is associated with genotypically distinct (as it relates to the candidate virulence genes) organisms in this population.〈section xml:id="abs1-4"〉〈title type="main"〉Methods.Patients with histologically confirmed gastric cancer were matched with nonulcer dyspeptic controls for age (within 5 years), gender and ethnicity. Helicobacter pylori status was determined by RUT, histology, culture and serology (locally validated and used as default determinant of H. pylori status). Tumors were classified according to the Lauren classification. The ‘virulence genotype’ of 17 paired culture samples was determined by previously described and validated molecular techniques (cagA presence, vacA alleles, structure of the cag pathogenicity island and analysis of the iceA alleles). Categorical variables were analysed by the χ2 test.〈section xml:id="abs1-5"〉〈title type="main"〉Results.Forty-eight patients (median age 59 years) could be adequately matched to controls. 39/48 (81%) cases and 43/48 (90%) controls were H. pylori positive (NS). Significant differences in the virulence genotypes of infecting strains were noted: vacAs2-controls 24%, cases 0%, p 〈 .00001; vacAs1 present – cases 100%, controls 76%, p 〈 .05; cagA-3′-length 〉 650 bp – cases 47%, controls 0%, p 〈 .002; cag pathogenicity island intact – cases 82%, controls 43%, p 〈 .04; iceA1 – cases 53%, controls 6%, p 〈 .005. cagA was found in all subjects.〈section xml:id="abs1-6"〉〈title type="main"〉Conclusion.This study indicates that, in this African population at least, there is no difference in the prevalence of H. pylori infection when comparing gastric cancer cases with matched controls. However, the findings suggest that gastric cancer may be associated with infection by organisms that are genotypically different from those not associated with disease.

    Description / Table of Contents: Zusammenfassung Histologische und elektronenoptische Befunde einer Hirnbiopsie eines 16jährigen Knaben werden mitgeteilt. Klinisch bot er eine progressive Demenz seit dem 10. sowie Grand Mal-Anfälle ab dem 13. Lebensjahr. Histologisch ergab sich das Bild einer neuronalen Lipoidspeichererkrankung. Elektronenoptisch wurde dieser Befund zwar bestätigt, doch ergaben sich keine für die bisher berichteten Formen von Lipoidspeichererkrankung charakteristische Veränderungen. Außer wenigen “Zebrakörpern” (Merkmale des Gargoylismus) enthielten die Neurone massenhaft Material (als “granuläre Vesikeln” bezeichnet), das an die Deposite in den Perizyten bei Gargoylismus erinnert, obwohl sie äußerst selten bei dieser Erkrankung in den Neuronen angetroffen werden. Da klinisch keine Hinweise auf einen Gargoylismus bestanden, war eine eindeutige Klassifizierung dieses Falles nicht möglich.
    Notes: Summary Histological and electron microscopical findings in a cerebral biopsy from a 16 year old boy are presented. He developed dementia from the age of 10 and grand mal from the age of 13. The histological picture was one of neuronal lipid storage disease, and the electron microscopy, while confirming this, was not clearly characteristic of any of the previously reported lipidoses. Apart from a few “zebra bodies” (characteristics of gargoylism) the neurons contained masses of material (here called “granular vesicles”) which resembled the pericyte deposits of gargoylism, though these are very rare in the neurones of that disease. It was therefore not possible to assign a presumptive classification to this case, as there was no clinical evidence of gargoylism.
