    Notes: The lasing performance of a copper vapor laser with a 60 mm diameter and 2000 mm discharge length has been improved by adding Sc atoms. As a typical result, a 28% increase of output power and electrical-optical conversion efficiency has been obtained. The result was qualitatively but reasonably interpreted as indicating that the upper laser level was enhanced by the collisional energy transfer from a response of Sc atoms to a resonance-excited state of Cu atoms. © 1997 American Institute of Physics.

    Notes: Abstract A rare case of a superficial spreading carcinoma with esophageal stricture is described. A 68-year-old man with dysphagia was found to have an upper to middle esophageal stenosis on radiologic examination. Endoscopically, a reddish and slightly depressed mucosa was seen throughout the whole esophagus, and a localized ulcerative lesion was detected in the middle portion. The lesion was negative for iodine staining. A superficial spreading carcinoma of the esophagus was diagnosed. A total esophagectomy with wide lymph node dissection and posterior mediastinal esophagocolectomy were performed. Microscopically, the moderately differentiated squamous cell carcinoma with minute submucosa was seen continuously from the cervical to lower esophagus. The distance of the superficial spread was 15.5 cm.

    Description / Table of Contents: Résumé Trente six patients admis pour l'exérèse chirurgicale d'un léiomyosarcome gastro-intestinal dans l'unité de chirurgie II du Département de Chirurgie à Karrazawa University Hospital, Karrazawa, Japan, ont été sélectionés pour cette étude. Des renseignements concernant la survie était disponibles chez 32 patients. Les données clinico-pharmacologiques comprenant le site et la taille de la tumeur, la cellularité, l'index mitotique et le caractère diploÏde de l'ADN ont été analysés. Les résultats correlaient positivement avec le pronostic. Treize patients (41%) avaient des métastases à distance et/ou des récidives. Les métastases d'origine hématogène étaient très fréquentes. La récidive locale et la survenue de métastases à distance étaient correlées de faÇon significative avec la survie (p〈0.001), la localisation de la tumeur (p〈0.03), la taille de la tumeur (p〈 0.04) et le traitement chirurgical (p=0.05), mais pas avec le type de diploÏdie de l'ADN (p=0.06). Ni la cellularité ni l'index mitotique ne correlaient avec la survie de faÇon significative. De plus plusieurs patients ayant une récidive locale associée à des métastases à distance ont eu des survies prolongées après résection des récidives. Au vue de ces résultats, un traitement chirurgical agressif des récidives semble efficace.
    Abstract: Resumen Se seleccionaron 36 pacientes entre aquellos hospitalizados para resectión quirÚrgica de leiomiosarcomas del tracto gastrointestinal en el Departamento de Cirugía II, Hospital de la Universidad de Kanazawa, Japón, y los departamentos de círugía afiliados, para el presente estudio; la informatión relativa a sobre vida pudo ser obtenida en 32 de ellos. Las variables clinico-farmacológicas, taies como tamaño del tumor, ubicación, celularidad, indice mitótico y patrón ploidial de DNA fueron analizadas; los resultados aparecieron correlacionandose con el pronóstico. Trece (41%) de los pacientes se presentaron con metastasis distantes y/o recurrencias, las metastasis hematogénicas siendo el tipo predominante de recurrencia. Las recurrencias locales y/o las metástasis distantes aparecieron significativamente correlacionados con la sobrevida (p〈 0.001), así como con la ubicación del tumor (p〈0.03), el tamaño del tumor (p〈0.04), el tratamiento quirÚrgico (p= 0.05) y el patrón ploidial de DNA (p=0.06). Ni el índice mitótico ni la celularidad demostraron correlación con la sobrevida. Además, algunos pacientes con recurrencia local o con metastasis distantes sobrevivieron por largo periodo después de resección de tumores recurrentes. En vista de estos resultados, la resección quirurgica agresiva puede representar una modalidad eficaz de tratamiento de la recurrencia.
    Notes: Abstract Thirty-six patients who were admitted for surgical resection of leiomyosarcomas of the gastrointestinal tract to the Department of Surgery II, Kanazawa University Hospital, Kanazawa, Japan and its affiliates are included in the study. Follow-up data on survival is available for 32 patients. The clinico-pharmacologic variables, such as tumor site, tumor size, cellularity, mitotic index, and DNA ploidy pattern were analyzed and the results proved to correlate with the prognosis. Thirteen (41%) of the patients presented with distant metastases and/or recurrences, with hematogenous metastasis being the predominant type of recurrence. Local recurrences and/or distant metastases were significantly correlated with survival (p 〈0.001), as was tumor site ( p 〈0.03), tumor size ( p 〈 0.04), surgical treatment ( p =0.05), and DNA ploidy pattern ( p =0.06). Neither the mitotic index nor the cellularity proved to be significantly correlated with survival. Furthermore, some of the patients with local recurrences or distant metastases survived long after resection of recurrent tumors. In view of the results, aggressive surgical resection may be an efficient treatment of recurrences.

    Description / Table of Contents: Résumé Cinquante-cinq cas de cancer gastrique avancé, à potentiel très malin (stade IV à l'imagerie préopératoire), ont été traités par une chimiothérapie PMUE comprenant une combinaison de CDDP, 75 mg/m2, MMC, 10 mg/Kg poids corporel, d'etopocide, 150 mg/Kg poids corporel et d'UFT, 400 mg/jour. Le CDDP et le MMC ont été donnés par voie intraveineuse à Jl, suivis d'etoposide, 50 mg/jour aux jours 3, 4 et 5. Tous les patients avaient des lésions qui ont pu être évaluées par la tomodensitométrie avant et après le traitement. Ces patients ont été randomisés en deux groupes: 29 ont eu une chimiothérapie néoadjuvante (CNA) par le PMUE en préopératoire, alors que les 26 autres ont d'abord été opérés, et ensuite ont reçu une chimiothérapie PMUE (groupe contrôle). Les caractéristiques des deux groupes ne différaient pas de façon significative. Le taux de réponse était plus haut dans le groupe CNA par rapport au groupe contrôle (62% contre 35%). Le taux de résecabilité était respectivement, de 79 et 88% dans les deux groupes. Le taux de cas potentiellement curables, cependant, était plus élevé dans le groupe CNA par rapport au groupe contrôle (38% contre 15%). Dans les cas de cancer gastrique non réséqués, le pronostic était extrêmement mauvais, quel que soit le groupe. Dans les cancers réséqués, la survie était significativement plus élevée dans le groupe CNA comparé au groupe contrôle. Ces résultats indiquent que chez les patients ayant un cancer gastrique avancé à potentiel très malin, une chimiothérapie néoadjuvante (d'emblée), suivie de chirurgie, peut être le meilleur choix thérapeutique.
    Abstract: Resumen Cincuenta y cinco casos de cáncer gástrico avanzado, en los cuales se había confirmado el Estado IV mediante imágenes diagnósticas preoperatorias, recibieron quimioterapia PMVE con el uso combinado de cisplatino (CDDP) 75 mg/m2, MMC 10 mg/cuerpo, Ectoposide 150 mg/cuerpo y VHF 400 mg/día. El CDDP y el MMC fueron administrados por vía intravenosa en el primer día, seguidos de Ectoposide 50 mg/día en los días 3, 4 y 5. Todos los pacientes exhibían lesiones medibles, las cuales fueron valoradas por escanografía computadorizada antes y después del tratamiento. Los pacientes fueron ubicados al azar en dos grupos; 29 quedaron en el grupo de la quimioterapia neoadyuvante (QNA) en el cual la quimioterapia PMVE fue practicada preoperatoriamente, y los 26 pacientes restantes fueron sometidos primero a operación y luego a quimioterapia PMVE, constituyendose en el grupo de control. Los antecedentes médicos no eran significativamente diferentes en los dos grupos. La respuesta fue mayor en el grupo de QNA en comparación con el grupo control (62% vs 35%). La tasa de resecabilidad fue de 79% y 88% en el grupo QNA y en el grupo de control, respectivamente. Sin embargo, la rata de casos potencialmente curables fue más alta en el grupo de QNA, en comparación con el grupo control (38% vs 15%). En los casos no resecados, sin embargo, la tasa de sobrevida fue significativamente superior en el grupo de QNA en comparación con el grupo control. Tales resulados pueden significar que en pacientes con cáncer gástrico de alto grado y en estado avanzado se debe considerar primero la quimioterapia como paso inicial (quimioterapia neodyuvante), y luego la cirugía.
    Notes: Abstract Fifty-five patients with high-grade advanced gastric cancer in whom the presence of stage IV was confirmed by preoperative diagnostic imaging were treated with PMUE therapy by a combined use of cisplatin (CDDP) 75 mg/m2, mitomycin C (MMC) 10 mg/body, etoposide 150 mg/body, and UFT (a combination of 1-(2-tetrahydrofuryl)-5-fluorouracil and uracil in a molar ratio of 1∶4) 400 mg/day. CDDP and MMC was administered intravenously on the first day, followed by etoposide 50 mg/day on the 3rd, 4th, and 5th days. All the patients had measurable lesions that were evaluated by computed tomography scanning before and after the treatments. These patients were allocated randomly to two groups. Of these cases, 29 belonged to the neoadjuvant chemotherapy (NAC) group to whom PMUE therapy was given preoperatively; the remaining 26 patients underwent operation first and received PMUE thereafter (control group). Background factors did not differ significantly between the two groups. The response rate was higher in the NAC group than in the control group (62% in the former versus 35% in the latter). The resectability rates were 79% and 88% in the NAC and control groups, respectively. However, the rate of potentially curable cases was higher in the NAC group than in the control group (38% in the former versus 15% in the latter). Among the nonresection cases, the prognosis was highly unfavorable in both groups. In the resection cases, however, the survival rate was significantly better in the NAC group than in the control group. These results may indicate that in patients with high-grade, advanced gastric cancer initial chemotherapy (neoadjuvant chemotherapy) and then surgery should be considered.

    Notes: Abstract We treated a patient with a complete invagination of the cecum which contained a mucocele of the appendix secondary to an obstruction by endometriosis. Preoperatively, a barium enema showed a crab's claw-like area without filling in the oral side of the transverse colon. An emergency laparotomy was performed and revealed a mucocele of the appendix to have induced appendicecal invagination; however, no colonic invagination was observed. An appendicecal resection was thus done. Pathologically, the resected specimen was a mucosal hyperplasia with mucin-secreting lesions of the appendix. The theories regarding the pathogenesis of appendicecal mucocele are reviewed and discussed.

    Notes: Summary The expression of the proliferation-associated nuclear antigen p105 in 43 patients with advanced gastric carcinoma was studied by multiparameter flow cytometry. The p105 immunofluorescence intensity of tumor cells in the G0G1 phase was 1.8–14.5 times that in stromal cells, and the stromal cells could be separated by flow cytometry. The p105 intensity, which is the ratio of the mean fluorescence intensity of cancer cells to that of stromal cells in the G0G1 phase, was not correlated with the DNA index, the size of the S-phase fraction, or the histological type of the tumor. However, the intensity was significantly greater in patients with hepatic metastasis and significantly lower in patients with peritoneal metastasis. The patients with a p105 intensity of 4.0 or more had a significantly higher incidence of hepatic metastasis (8/22; 36%). The patients with a p105 intensity of less than 4.0 had a significantly higher incidence of peritoneal metastasis (8/21; 38%). These results suggest that the p105 intensity is a marker of metastasis useful in the treatment and follow-up of patients.

    Notes: Abstract Candidiasis of the esophagus progressing to stricture formation in a 74-year-old male is reported. Esophageal candidiasis develops in the presence of various predisposing conditions such as long-standing administration of antibiotics or corticosteroids, and malignancy. The first endoscopic examination of this patient revealed unusual multiple black plaques in the esophagus. Despite intensive exploration, no predisposing factors were found. The stricture was progressive, despite the administration of adequate antifungal therapy, and its presence necessitated several attempts at dilatation. Case reports of esophageal candidiasis without underlying disease are very rate.

    Notes: Abstract This paper describes an unusual case of an 80-year-old man followed up for multifocal gastric cancers. There were three separate polypoid carcinomas and one adenomatous polyp with no sign of malignancy. We measured the DNA content of the gastric cancer and adenomatous cells obtained from endoscopically biopsied specimens. The adenomatous polyp and one of the cancerous lesions showed DNA diploidy. The other two cancerous lesions showed DNA aneuploidy, with different DNA index (DI) values (1.12 and 1.64, respectively). It is considered that the three cancers arose from different stem lines. However, an operation was not performed because the patient refused gastrectomy, and therefore only conservative follow up has been continued. Presentation of this case is followed by a detailed discussion focusing on the possible development of carcinoma in gastric adenomatous polyps in view of the data from the literature.

    Notes: Abstract We report herein a rare case of spontaneously perforated pyometra found in a 72-year-old woman who was admitted to our hospital with abdominal pain and vomiting. A distended abdomen with muscular rigidity, a positive Blumberg sign, and a WBC count of 11,900/mm3 indicated diffuse peritonitis, although a plain abdominal X-ray film revealed no free air in the peritoneal cavity. An emergency laparotomy was performed, which revealed a lot of pus, and perforation in the fundus of a distended uterus. The patient was therefore diagnosed as having suffered uterine perforation associating with a pyometra, and a total hysterectomy with bilateral salpingo-oophorectomy was carried out. Histological examination revealed a pyometra with inflammation and destruction of the endometrium and myometrium, and cervical occlusion with no evidence of malignancy. Postoperatively, the patient developed a subcutaneous abscess and pneumonia, but recovered and was discharged on the 74th day after her operation. Thus, although rare, spontaneously perforated pyometra should be considered when elderly women present with acute abdominal symptoms.
