    Notizen: Background : Mesalazine (5-aminosalicylic acid)-containing formulations, designed to optimize drug delivery to the ileo-caecal region, represent a cornerstone in the treatment of inflammatory bowel diseases.Aim : To test, by means of pharmaco-scintigraphy, whether novel mesalazine-containing pellets release 5-aminosalicylic acid in the same target region as mesalazine tablets (Salofalk).Methods : Fourteen healthy male volunteers received a single dose of either pellets or tablets containing 500 mg of mesalazine and 2 mg of 152Sm2O3 with a 1-week washout period. The gastrointestinal transit of 153Sm, incorporated into the formulations, was followed by gamma-scintigraphy. Mesalazine release was verified by assessing 5-aminosalicylic acid plasma pharmacokinetics.Results : The formulations reached the ileo-caecal target region almost at the same time (3.3 ± 1 and 3.8 ± 1 h for pellets and tablets, respectively). Plasma 5-aminosalicylic acid tmax values were comparable and corresponded to the time during which the formulations were located in the target region. Plasma AUC values were significantly lower for pellets, which might be explained by a more prolonged release of 5-aminosalicylic acid.Conclusions : Novel mesalazine pellets and Salofalk tablets release active 5-aminosalicylic acid in the same target region and pass through the gastrointestinal tract under fasting conditions in healthy volunteers in a comparable time. From a comparison of in vitro dissolution and plasma concentration data, a slower and more prolonged release of 5-aminosalicylic acid from pellets is suggested.

    Notizen: Background : Local treatment with foams in patients suffering from ulcerative proctitis or proctosigmoiditis is considered a rational treatment option.Aims : To investigate colonic spread, safety, tolerability and acceptance of a newly developed budesonide foam formulation.Methods : Twelve patients (four females, eight males) with acute proctosigmoiditis or left-sided ulcerative colitis were rectally administered a single dose of [99Tcm]-labelled budesonide foam (Budenofalk; Dr Falk Pharma GmbH, Freiburg, Germany) containing 2 mg budesonide in 20 mL foam after diagnostic colonoscopy. Thereafter, the colonic spread was assessed by means of γ-scintigraphy for 6 h. Serum samples were taken simultaneously.Results : Budesonide foam spread with a maximum between 11 and 40 cm, thus reaching the sigmoid colon in all patients. In some patients, the foam even extended into the distal third and the middle of the descending colon with maximum radioactivity at 4 h. Systemic budesonide absorption was rapid and pharmacokinetic data were comparable with published data on marketed budesonide enemas, with mean serum Cmax and AUC0-8h values of 0.8 ± 0.5 ng/mL and 3.7 ± 1.9 ng h/mL, respectively. The new formulation was well accepted by all patients, who could retain the foam for at least 4 h.Conclusions : In the majority of patients, budesonide foam effectively spread up to the left-sided colon and thus qualifies for the local treatment of proctosigmoiditis.

    Notizen: Background : Mesalazine (5-aminosalicylic acid, 5-ASA)-containing formulations represent a cornerstone in the treatment of inflammatory bowel diseases. Recently, a new formulation has been developed to provide selective and more homogeneous release of 5-ASA compared to traditional systems.Methods : In a first study, gastrointestinal transit was followed by gamma-scintigraphy after single-dose application of tablets containing 1200 mg mesalazine to 12 healthy male volunteers. 5-ASA release was verified by the assessment of plasma pharmacokinetics. In a second, 7-day, multiple-dose study, the steady state plasma pharmacokinetics, urinary excretion and safety profile were characterized after twice-daily tablet administration to 12 healthy volunteers.Results : Tablet erosion started after 6.9 ± 1.1 h in the ascending or transverse colon. Radioactivity spread homogeneously throughout the colon, indicating the sustained release of active 5-ASA. Plasma kinetics indicated an earlier initial absorption of 5-ASA, i.e. during transit of the small intestine and ileum. Mean Cmax values (350.6 ± 322.6 ng/mL) were observed during location in the ileo-caecal region. The mean relative absorption of 5-ASA was 19.9 ± 18.2% in the small intestine and ileum and 80.1 ± 18.2% in the colon.Conclusions : The administration of the new mesalazine formulation was well tolerated, and 5-ASA was continuously released along the whole colon, a favourable prerequisite for the therapy of distally located inflammatory bowel disease.

    Notizen: Summary Administration of high doses of131I-6β-iodocholesterol to rabbits is followed by marked size reduction of the adrenal glands with concomitant changes of the histological structure and with a decrease of serum cortisol. Stimulation of131I-6β-iodocholesterol uptake into the adrenals by additional administration of ACTH had only a minor additional effect on adrenal weight. However, with further accumulation of labeled cholesterol in the adrenals the radioactivity was reduced in blood, urine, and the ovaries.

    Notizen: Summary Marked weight reduction of rat adrenals was observed after131I-6β-Iodocholesterol administration whether or not the animals were treated with metapyralone. Damage or even histologic changes of the adrenals have not been found. Though the radiation dose is high with the applied dose of131I-6β-Iodocholesterol and might limit or even prevent its application in the treatment of human adrenal disease, the influence of endogenous irradiation on rat adrenals is obvious and justifies further investigations.

    Beschreibung / Inhaltsverzeichnis: Summary Clinical PET is now primarily used in oncological indications. Fluorine-18-fluorodeoxyglucose (FDG) shows a high uptake in a variety of malignancies and accounts for approximately 95% of the oncological PET studies. The first part of this review deals with theoretical and methodological aspects of FDG-PET. Possible sources of errors for the method are discussed briefly. Oncological indications for FDG-PET are highlighted according to the results of consensus meetings of experts from nuclear medicine and oncology, organized by the German Society of Nuclear Medicine. FDG-PET is mainly used in staging and restaging, demonstrating of tumour recurrence and therapy control. Main benefits of the method are functional imaging, better spatial resolution than conventional scintigraphy and the possibility to perform whole-body imaging easily. A brief outlook on future prospects for the application of PET in oncology is given.
    Notizen: Zusammenfassung Der klinische Einsatz der PET betrifft heute vorwiegend onkologische Fragestellungen. F18-Fluorodesoxyglucose (FDG) zeigt eine sehr hohe Anreicherung in einer Vielzahl von malignen Tumoren und wird in etwa 95% der Fälle als Radiopharmakon für die Tumordarstellung mit PET eingesetzt. Dieser Übersichtsartikel beschreibt die theoretischen Grundlagen und methodische Gesichtspunkte der FDG-PET. Mögliche Fehlereinflüsse auf die Methode werden diskutiert. Die Hauptindikationen für FDG-PET bei onkologischen Fragestellungen werden im wesentlichen entsprechend den in Konsensuskonferenzen zwischen Nuklearmedizinern und Onkologen getroffenen Vereinbarungen der Deutschen Gesellschaft für Nuklearmedizin besprochen. FDG-PET findet insbesondere beim Staging, beim Rezidivnachweis und bei der Therapiekontrolle Anwendung. Funktionelle Bildgebung, im Vergleich zur konventionellen Szintigraphie verbesserte Ortsauflösung und die leichte Anfertigung von Ganzkörperaufnahmen sind besondere Vorzüge der Methode. Zukünftige Möglichkeiten und Trends der Anwendung von PET in der Onkologie werden kurz beschrieben.

    Notizen: Abstract Peroral small bowel biopsy is a decisive step in the diagnosis of coeliac disease and is performed frequently. The patient's exposure to ionizing radiation during fluoroscopic control therefore deserves consideration. Compared to data in the literature, the biopsy method employed at the University Children's Hospital in Vienna achieves a markedly shorter duration of fluoroscopy. These values were registered during 1039 biopsies. In 439 examinations the exposure area products were also documented. In 10 consecutive biopsies additional measurements were carried out by lithium fluoride thermoluminescence dosimeters attached to the patient. Expert biopsy-technique with precise and thoughtful use of modern fluoroscopes on the one hand, and critical self-control by routine documentation of the exposure area product and duration of fluoroscopy on the other can markedly reduce the patient's exposure to X-rays.

    Notizen: Abstract Background: On the basis of promising data on the use of fluorine-18-2-fluoro-2-deoxy-D-glucose (F18-FDG) whole body positron emission tomography (WB-FDG-PET) in the staging of patients with lymphoma, we initiated a pilot series to evaluate the role of WB-FDG-PET in the staging of extranodal B-cell lymphoma of the mucosa-associated lymphoid tissue (MALT) type. Patients and methods: We examined ten consecutive patients with histologically-verified MALT-type lymphomas of various origin before initiation of therapy. Nine patients had low-grade lymphomas (five cases of gastric lymphoma, two patients with lymphoma arising in the lung, one parotid and one lacrimal gland lymphoma), while one patient had a high-grade gastric lymphoma arising from a low-grade background. Two patients had stage EI, seven had stage EII disease, and one presented with stage EIII. WB-FDG-PET scans were performed 40 min following the injection of 300–380 MBq of F18-FDG. The PET scans were correlated with extensive conventional staging including ophthalmologic investigation, otolaryngologic examination, gastroscopy, endosonography, enteroclysis, colonoscopy, CT of thorax and abdomen, and bone marrow biopsy. Results: WB-FDG-PET documented no lymphoma in any of the 10 patients studied, as no focal tracer uptake was demonstrated in either gastric or extragastric lesions or in involved lymph nodes, irrespective of histologic grading. In three patients the scan showed a false negative result with respect to the MALT lesions but showed focal tracer uptake indicating tumor spread, which, however, was ruled out by further follow-up and biopsy, respectively, and was thus rated false positive. Due to these results, the study was discontinued prematurely after the first ten patients. Conclusions: These discouraging results indicated that WB-FDG-PET is not useful for staging and follow-up of MALT-type lymphoma, and should therefore not be included in the clinical decision making process.
