    Notes: Abstract The composition of 15 V Tβ gene subfamilies has been examined by Southern hybridization among a broad spectrum of colony bred rat and mouse species extending phylogenetically from Rattus to Mus musculus domesticus. Most mouse species contain a similar content of V Tβ genes as determined by the number of hybridizing restriction fragment (RF) bands. Furthermore, the extent of restriction fragment length polymorphism (RFLP) appears to be limited. Some V Tβ gene families, however, are missing from Rattus (VTβ7, V Tβ12) and M. shortridgei (V Tβ9, V Tβ16). Extension of the V Tβ survey to a panel of 38 wild-caught mice reveals that nearly a third lack specific hybridization to the V Tβ5 probe. Previous reports have established that the mouse inbred strains SJL, C57BR, C57L, and SWR lack 50% of their V Tβ repertoire, including V Tβ5 (Behlke et al. 1985). This study demonstrates that natural populations of mice also carry a significantly reduced V Tβ gene repertoire.

    Notes: DURU, F., et al.: Clinical Evaluation of a Pacemaker Algorithm That Adjusts the Pacing Rate During Sleep Using Activity Variance. Even though rate responsive pacemakers are able to regulate pacing rates based on sensor activity, they are set with a minimum rate that is not adjusted to provide rate decreases during sleep. The aim of this study was to evaluate the performance of the “Sleep Rate” feature, as compared to patient diaries and a validated method that identifies sleep from wrist actigraphy. In 19 patients (15 men; age 69 ± 8 years) with Pacesetter Trilogy DR+ pacemakers, the base rate and the sleep rate were set to 80 and 50 ppm, respectively. When the patients returned 2 days later, data recorded by the pacemaker and wrist actigraph were analyzed to find the agreement in corresponding sleep/wake periods. In 17 (89%) patients, the pacemaker went into the sleep mode. The total sleep time derived from actigraphy significantly exceeded the time during which the pacemaker was in sleep mode (1156.8 ± 83.4 vs 307.3 ± 77.2 minutes). Frequent reversions out of the sleep mode limited the total sleep time derived from the pacemaker. Cumulative analysis of the pacemaker data showed that the maximum time in the sleep mode was 78 minutes, and exceeded 1 hour in six instances, 30 minutes in 32 instances, and 15 minutes in 83 instances. Epoch by epoch comparisons revealed a good agreement (93.6 ± 1.8%) during wakefulness between the corresponding actigraph and pacemaker epochs. However, only 24.6 ± 3.7% of the corresponding epochs during sleep were identical, and the overall agreement was 54.4 ± 3.7%. Except for one patient who reported palpitations, patients did not suffer from a pacemaker rate change. The Sleep Rate feature provides rate reduction during sleep, while assuring rapid frequency response during physical activity. However, the current algorithm does not allow long periods of slow pacing rate during continuous sleep, possibly due to its conservative design and the presence of movement arousals, which has to be improved in future generation algorithms.

    Notes: Summary A starlike arrangement of cells and fibers, the “storiform pattern”, was found to be a typical, but not obligatory, histological feature of benign and malignant fibrous histiocytomas. In 155 benign fibrous histiocytomas storiform structures were missing in 29 cases, chiefly of the fibroblastic type comparable with classical “dermatofibroma”. 12 of 70 malignant fibrous histiocytomas did not reveal storiform structures, especially the cellular pleomorphic variant, i.e. the classical pleomorphic sarcoma. Storiform structures were either small and highly cellular with few fibers (collagen type III), or larger, less cellular, but with abundant fibers (collagen type I). There was no sharp demarcation between these two extremes, but many transitional structures or patterns were seen. The histiocytic nature of the cells was demonstrated in both variants of storiform structures by immunhistochemical methods on paraffin embedded material. Alpha1-anti-chymotrypsin was especially valuable in this respect.

    Description / Table of Contents: Zusammenfassung Die Beurteilung der Ausdehnung und des topographischen Niveaus der FascienverÄnderungen bei 100 FÄllen mit nodulÄrer Fasciitis ergab z.T. nur intrafasciale Zellproliferation mit spindeliger Auftreibung der Fascie. HÄufiger fand sich jedoch einseitig von der Fascie ausgehendes infiltratives Wachstum, am hÄufigsten eine vollstÄndige Unterbrechung der ursprünglichen Fascienstruktur mit beidseitiger Infiltration der Umgebung. Die nodulÄre Fasciitis befiel nicht nur die subcutane Fascie und das umgebende Fettgewebe. VerÄnderungen der oberflÄchlichen Fascie führen zu einer Mitbeteiligung der Cutis. Bei tief gelegener nodulÄrer Fasciitis greifen die VerÄnderungen auf die Muskulatur über. Es finden sich entweder übergangsformen zur proliferativen Myositis bei Befall der supramuskulÄren Fascie oder bei rein intramuskulÄren VerÄnderungen das typische Bild des proliferativen Myositis, die demnach als tiefsitzende Variante der nodulÄren Fasciitis mit Muskelbeteiligung aufgefa\t werden kann. IntramuskulÄre Myxome, die ebenfalls z.T. übergangsformen zur typischen nodulÄren Fasciitis erkennen lie\en, scheinen eine intramuskulÄre mucoide Variante der nodulÄren Fasciitis darzustellen. Insgesamt fand sich am hÄufigsten eine bilaterale Infiltration in allen Lagen der nodulÄren Fasciitis. In FÄllen mit Muskelbeteiligung fand sich ausschlie\lich eine Zellproliferation mit Zerstörungen der Fascie und bilateraler Infiltration der Umgebung.
    Notes: Summary Nodular fasciitis (NF) shows a cellular proliferation which leads to widening of the fascia. Frequently unilateral or more often bilateral disruption of the fascia, with an infiltrative pattern is present. Subcutaneous fascia and surrounding fat are involved. Superficially the cellular proliferation may extend into dermal connective tissue. Deeper muscular tissue may be involved, with transitional forms or purely intramuscular changes, compatible with proliferative myositis. Proliferative myositis is considered to be a deep-seated variant of NF with muscular involvement. Intramuscular myxoma may be thought of as an intramuscular and mucoid variant of NF. A bilateral infiltrative pattern was most frequently found at all levels; in cases with muscular involvement it was always present.

    Description / Table of Contents: Zusammenfassung Die Weichgewebstumoren machten ca. 2,8 % unseres gesamten chirurgischen Einsendungsmaterials aus. Am häufigsten fanden sich dabei Tumoren des Fett- und Bindegewebes. Bei den letzteren sind die agrressive Fibromatose und die noduläre Fasciitis von besonderem klinisch-pathologischem Interesse. Durch eine kombinierte Beurteilung der histologischen Graduierung und der histogenetischen Klassifizierung kann das TNM/G-System angewandt werden, woraus sich auch eine Stadieneinteilung ergibt. Die Unterscheidung von geringem und hohem Malignitätsgrad ist die Voraussetzung einer optimalen Therapie von Sarkomen.
    Notes: Summary Soft tissue tumors showed an overall incidence of 2.8% in our surgical material. Most frequently found were tumors of fatty or connective tissues. In the latter, aggressive fibromatosis and nodular fasciitis are of special clinical-pathological interest. By combined consideration of a histological grading and histogenetic classification the TNM/G-system can be applied with consecutive staging. Distinction between low- or highgrade malignancy is a prerequisite for an optimal therapy.

We tested the practicability of dipyridamole myocardial nitrogen-13 ammonia positron emission tomography (dipyridamole 13NH3 PET) for the perioperative risk assessment of coronary artery disease (CAD) in a cohort of patients with severe chronic obstructive pulmonary disease (COPD) undergoing lung volume reduction surgery (LVRS). Twenty consecutive LVRS candidates, 13 men and 7 women (mean age 57±2 years), without symptoms of CAD were prospectively studied by dipyridamole 13NH3 PET. Side-effects and overall tolerance were assessed by a questionnaire and visual analogue scale. Repeated pulmonary function tests were performed before and 4, 12, 16 and 30 minutes after dipyridamole injection. All dipyridamole 13NH3 PET studies were negative for CAD. Seventeen patients underwent LVRS without cardiac complications; three patients did not undergo LVRS for other reasons. Nine patients suffered intolerable dyspnoea requiring i.v. aminophylline. Mean FEV1 decreased significantly after dipyridamole infusion: in nine patients the reduction in FEV1exceeded 15% from baseline. We found that dipyridamole is not well tolerated and causes significant bronchoconstriction in patients with severe COPD. Although all dipyridamole-induced side effects can be promptly reversed by aminophylline, dipyridamole cannot be recommended as a pharmacological stress in this setting.

    Notes: Summary A storiform pattern is an important structural feature of fibrous histiocytoma (FH). In this analysis reconstructions of histological patterns were carried out from drawings. Highly cellular FH with only a suggestion of storiform pattern and small star-formations were seen, there were other lesions with more pronounced fiber formation which showed more distinct and larger storiform stars. These structures can frequently be followed for only 3 sections of 5 micron thickness. In contrast to leiomyomas or meningioma, no regular or consistent orientation of these structures with respect to vessels is evident. Storiform structures apparently develop at the periphery of adjacent proliferating cells groups. They show a typical and diagnostically significant histological pattern, which was found to some degree in all FH examined.
