    Notes: Summary In an attempt to determine to what extent the measurement of cerebrospinal fluid (CSF) outflow resistance (Rout) can distinguish patients with normal-pressure hydrocephalus (NPH) who respond to shunt surgery from patients who fail to do so, the authors undertook a controlled trial. Eighteen patients with the diagnosis of NPH entered the study. In connection with the shunt operation all patients who entered the study underwent Rout measurement by means of the pressure-volume index (PVI) method of bolus manipulation (Marmarouet al.). As a safety measure patients above 70 of age and patients with dementia as the only clinical symptom underwent continuous intraventricular pressure measurement before they entered the final study. As a result a total of fourteen patients underwent PVI test and shunt surgery during the same procedure. Patients with normal Rout thus served as controls, since all patients underwent shunt surgery irrespective of the result of the Rout measurement. Eight patients improved following surgery and the outcome did not correlate with result of PVI nor Rout determinations.

    Notes: Summary Twenty-nine patients consecutively admitted for consideration of CSF diversion surgery for suspected communicating (“normal pressure”) hydrocephalus underwent cranial computerized tomography (CT) and study of cerebrospinal fluid (CSF) absorption, the latter determined as resistance to outflow of CSF (Ro). From the CT the size of the ventricular system was determined by various linear measurements and ratios and the presence of periventricular lucencies (PVL) and the size of cortical sulci was noted. Except for absence of cortical sulci greater than 3 mm no CT feature was suggestive of compromised CSF absorption. On the other hand, cortical atrophy did not rule out increased resistance to CSF-outflow. Features thought of as characteristic for normal pressure hydrocephalus on CT: large ventricles as compared with the amount of cortical atrophy and presence of PVL, correlated poorly with measured high resistance values. There was no correlation between the size of the ventricular system and resistance to CSF outflow. In the diagnostic distinction between communicating (“normal pressure”) hydrocephalus and cerebral atrophy (hydrocephalus ex vacuo) static CT cannot replace an actual determination of CSF outflow resistance.

    Notes: Summary One hundred patients with symptoms of lumbo-sacral root compression were prospectively and consecutively assigned to operation based alone on clinical findings. A preoperative myelogram was performed in all patients and described without a knowledge of the clinical features. All patients were explored for the clinically and myelographically relevant disc. When the myelogram was normal (16 patients) both lower lumbar interspaces were exposed. In 58 patients a herniated disc was revealed at surgery. Only “myelographic herniation” with indentation of the contrast column was accompanied by a high frequency of disc herniation at surgery (73–87%). In cases with normal myelograms only 5% had a disc herniation. The severity of the myelographic finding was clearly correlated to the frequency of positive surgical findings and good outcomes. The preoperative radiculogram gives a high degree of certainty in the preoperative evaluation whether a surgical lesion is present or not and reveals a precise prediction of the outcome of surgery.

    Notes: Summary The Plastimed® epidural pressure sensor was evaluated in 35 patients, twenty-eight of whom were suffering from head injury. In seven patients simultaneous intraventricular pressure measurements were obtained. The epidural pressure sensor was only functioning satisfactorily in approximately 2/3 of the patients, while it was malfunctioning or not functioning in the remainder. In seven comparable IVP/EDP studies significant differences up to 25 mm Hg were noted. In three patients IVP was greater than EDP. In two patients the opposite was true. No significant complications were observed. These unsatisfactory results have made us abandon the technique and resort to intraventricular or subarachnoidal pressure measurements.

    Notes: Summary In a prospective, consecutive study of patients with lumbar back pain and sciatica, various clinical features and surgical findings were evaluated in order to analyse the predictive value regarding (1) level of diseased interspace (2) presence and type of lesion responsible for root compression (3) outcome after surgery. One hundred patients underwent surgery solely on clinical grounds. Fifty-eight had disc herniation. The level of disc herniation was correctly predicted in three quarters of patients with aprolapsed disc. The outcome after surgery was good in 77 patients. Only few clinical features, namely male sex and scoliosis were predictors of a good outcome. Lasegue's sign was indicative of root compression in 90%, but only two-thirds had disc herniation. Conversely one-third had disc herniation in spite of a “negative” test. Lasegue's sign was not superior to other clinical tests in predicting outcome. The most important indicator of a good outcome was the presence of disc herniation at surgery. Patients with disc pathology other than true disc herniation fared equally with patients, who had normal discs disclosed at surgery. Myelography was undertaken in all patients prior to surgery, the results of which are analysed in the following paper.     Notes: Summary During a period of 6 years 256 patients underwent ventriculostomy as their first operative procedure. Twenty-seven patients (11 %) developed CSF-infections, in almost 90% caused by Gram-positive cocci compatible with normal skin flora. The incidence of infection was distributed evenly in the postoperative period. All patients received intravenous and intrathecal antibiotic treatment simultaneously according to the sensitivity test. The treatment was highly effective, and without serious adverse effects.

    Notes: Summary 44 normal adults were examined by computer tomography (CT) with the fine matrix EMI scanner. Measurements of the width of the hemispheric sulci were possible in all but one case. An average value of 3.4 mm was found, but no sexual difference or influence of age could be shown statistically. The necessity of larger investigations of the normal population is stressed.

    Abstract: Zusammenfassung Untersuchungen an 44 normalen Personen mit dem EMI-Scanner (160/160 Matrix). Die Messungen wurden an verschiedene Teilen des Ventrikelsytems durchgeführt. Es fanden sich Geschlechtsunterschiede und ein Größenanstieg mit zunehmendem Alter. Dabei zeigte sich eine gute Korrelation zwischen den Untersuchungen mit der Computer-Tomographie und der Pneumoencephalographie.

Measurements of various parts of the ventricular system showed a sexual difference and an increase with age. Absolute measurements are preferred to indices or ratios. Comparison with the same parameters in pneumoencephalography and ventricular casts showed good correlation. The need for larger investigations of the normal population is stressed.
