Description / Table of Contents: Abstract. Posttraumatic cardiopulmonary resuscitation (CPR) is associated with a poor outcome. When evaluating the literature according to the Utstein method, there were only 2 survivors (0.18%) out of 1,135 CPR attempts after trauma (Table 1). Differences in the study populations and levels of prehospital trauma care led us to analyse the results of a physician-staffed prehospital trauma care system in Cologne. Methods. From January 1987 to December 1990, a total of 49,054 emergency calls were registered using a standardised protocol. Among 9,595 trauma-related calls, 636 patients were found to be pulseless on arrival of the emergency team, 412 of these were pronounced dead. CPR was initiated in the remaining 224 patients, who comprise the study population (defined as 100%). All patients who were admitted to a hospital were followed using a second protocol. Results. CPR in the field was successful in 68 (30.4%) patients, who were then admitted to a hospital; 42 of these died within the first 24 h. Four patients (1.8%) could be discharged from hospital alive and were still living 1 year later, 1 with a lasting neurological deficit (Fig. 1). In 156 (69.6%) cases resuscitative attempts were unsuccessful in the field. Conclusions. Even in a physician-staffed prehospital trauma care system, the chance of surviving a post-traumatic cardiac arrest is minimal. Survival has to be regarded as an individual fate; the overall results are discouraging. Even though this study analyses the largest population of posttraumatic CPR ever published, prognostic factors could not be identified due to the few survivors. Nevertheless, the result does not justify general omission of CPR after trauma as: (1) prognostic factors for survival have not been identified thus far; and (2) no significant additional costs arise from posttraumatic CPR.
Notes: Zusammenfassung.

Notes: Abstract The regional and laminar distributions of eight different transmitter-binding sites were measured in the marmoset hippocampus by means of quantitative in vitro receptor autoradiography. Receptors for 5-HT1, l-glutamate, N-methyl-d-aspartate (NMDA) and GABAA were similarly distributed. The highest concentrations of these receptors were found in the pyramidal layer of CA1 and the molecular layer of the dentate gyrus. The 5-HT2 receptors showed the highest concentrations in the oriens layer of CA2. The highest concentrations of muscarinic M1 receptors were seen in the pyramidal layer of CA1. Muscarinic M2 receptors were most densely concentrated in the pyramidal layers of CA1, CA2 and CA3. The noradrenergic α1 receptors were most densely packed in the radiatum-lacunosum-molecular layer of CA2 and the molecular layer of the dentate gyrus. Statistically significant co-distributions of serotoninergic, glutamatergic and GABAergic receptors point to possible interactions between these receptor systems in the same hippocampal regions and layers. Comparisons of marmoset distribution patterns for GABAA, NMDA, l-glutamate and 5-HT1 receptors with those in human hippocampi and those of other primates showed similarities between them. Clear differences in the patterns of α1, M1, M2 and 5-HT2 receptors could be seen between marmoset and human hippocampi, indicating a high degree of species specificity in a presumably “conservative” brain region. More similarities, however, could be found between marmoset and human hippocampi than between those of rat and human brains, especially in relation to 5-HT1 and GABAA receptors and l-glutamate-binding sites. In addition to the functional significance of receptor distribution patterns, such studies represent a valuable tool for the analysis of neurochemical aspects of brain evolution.

Notes: Summary NO and N2O release rates were measured in an acidic forest soil (pH 4.0) and a slightly alkaline agricultural soil (pH 7.8), which were incubated at different O2 concentrations (〈0.01 – 20% O2) and at different NO concentrations (40 – 1000 ppbv NO). The system allowed the determination of simultaneously operating NO production rates and NO uptake rate constants, and the calculation of a NO compensation concentration. Both NO production and NO consumption decreased with increasing O2. NO consumption decreased to a smaller extent than NO production, so that the NO compensation concentrations also decreased. However, the NO compensation concentrations were not low enough for the soils to become a net sink for atmospheric NO. The release of N2O increased relative to NO release when the gases were allowed to accumulate instead of being flushed out. The forest soil contained only denitrifying, but not nitrifying bacteria, whereas the agricultural soil contained both. Nevertheless, NO release rates were less sensitive to O2 in the forest soil compared to the agricultural soil.

Notes: Abstract: Used in the treatment of gross circumferential hemorrhoidal prolapse. Whitehead hemorrhoidectomy is technically more demanding and has slightly more complications than four-quadrant hemorrhoidectomy. The former, however, offers a radical cure and is the better operation whereas the latter is often associated with considerable remaining skin tags and areas of residual hemorrhoidal tissue. Following the Whitehead operation, we have observed fewer complications in older patients compared to younger patients. Case notes and follow-up data of all patients who underwent a Whitehead hemorrhoidectomy in the period October 1995 to November 1999 were reviewed, and clinical presentation and outcome were analyzed. The study group consisted of 11 patients (8 males, 3 females), of median age 58 years (range, 41–,75 years). The median operation time was 30 min (range, 15–40 min). The median hospital stay was 3 days (range, 2–15 days). There was only early and one late complication, both with minimal related morbidity. In both these patients, appropriate treatment resulted in complete recovery. All othera patients had an uneventful course with no residual symptoms at the last visit. Elderly patients may be less prone to developing significant sphicter spasm due to a generally more lax sphincter tone. Therefore, they may be at less risk of suture line ischemia, breakdown and stenosis.

Notes: Abstract: The use of staplers for ileostomy closure may facilitate the procedure and improve outcome by creating a wide, functional end-to-end anastomosis. We retrospectively compared 136 sutured ileostomy closures with 152 stapled closures. Perioperative data and outcome parameters were analyzed. Patient characteristics were similar in both groups. Highly significant reduction was found in operating time (p 〈 0.001) and hospitalization (p = 0.001) in the stapled group compared to the sutured group. Recovery of intestinal function and complication rates showed no differences. Our results show that both techniques are comparably safe but stapling is considerably faster to perform. The higher expenses of using a stapling device may be economically compensated by the shorter operation time under certain circumstances.

Notes: Abstract Results are presented from the first experiments on interferometric enhanced scattering (IES) in a plasma in a nonuniform magnetic field. Measurements of the lower hybrid fluctuations of the plasma density by the IES and rf probe methods in an experimental geometry that models a tokamak configuration are found to be in satisfactory agreement.

Notes: The effect of the density gradient on the dispersion of the m=+1 and m=−1 helicon modes is investigated both theoretically as well as numerically. In particular, attention is focussed on the helicon wave damping that is closely related to the rf power absorption and the Poynting flux. It is shown that the propagation characteristics of the m=−1 mode changes drastically when the density gradient is sufficiently strong. This mode exhibits a cutoff that depends on the gradient scale length and the plasma parameters. In the regime where both helicon modes propagate, the excitation of the m=−1 mode is unlikely as it is strongly damped. The results give evidence that the density gradient effect is decisive for the formation of helicon discharges, which are predominantly sustained by the m=+1 mode. © 1999 American Institute of Physics.

Notes: A high-density helicon discharge (ne≤1020 m−3) produced through a new m=1 helical antenna is investigated. Various diagnostics are applied to measure the discharge parameters and the radio frequency (rf) quantities like the plasma resistance and the rf field distribution. Special attention is paid to the axial asymmetry of the discharge, which is characteristic for helicon devices with helical antennas. The axial profiles of the rf wave fields, as well as the energy deposition profiles, reveal that the rf power is mainly transferred and absorbed via the m=+1 helicon mode traveling in the positive magnetic-field direction. The experimental findings are compared with numerical results obtained from a fully electromagnetic model, which takes into account the rf current distribution of the launching antenna, as well as the finite size of the plasma column. The antenna–plasma coupling, as well as the total rf power deposited in the plasma, can be explained satisfactorily if the measured profiles are taken in the computations. In particular, the axial asymmetry of the helicon discharge can be understood in terms of the radial inhomogeneity of the plasma column. Furthermore, the calculations show that the small-scale Trivelpiece–Gould waves may be excited near the plasma edge. These waves would carry a considerable fraction of the absorbed rf power and may thus be important for the rf power coupling. © 2000 American Institute of Physics.

Notes: When bone marrow transplantation recipients undergo standard pre-transplant immunosuppressive therapy, engraftment failures are significantly more frequent with the use of T-depleted allogeneic donor bone marrow cells than with T cell-containing allogeneic donor bone marrow cells. The relative importance of T versus natural killer (NK) cells in the rejection process of T-depleted donor bone marrow cells remains debatable. Here, NK- and T-deficient mouse mutants were transplanted across the same major histocompatibility complex (MHC) differences with homozygous or heterozygous T-depleted bone marrow cells. Results show that under the experimental conditions described, residual host NK cells are almost exclusively responsible for the increased rejection rate.
