    Notes: Abstract We studied 13 patients with Wilson’s disease (WD) using localized magnetic resonance proton spectroscopy to test whether hepatic encephalopathy or impaired energy metabolism contributes to neurological dysfunction. Levels of myoinositol (MI), choline-containing compounds (Cho), creatine (Cr), N-acetyl-aspartate (NAA), Glx (unresolved resonances of glutamate, glutamine, and γ-aminobutyric acid) and lactate were measured using a relative quantitative approach. Results were compared with those from 12 healthy controls. In one patient with de novo WD and acute hepatic disease but no neurological symptoms we found a marked decrease in the Cho/Cr and MI/Cr ratios. However, proton spectroscopy in the white matter, gray matter, and putamen of patients with treated WD showed no significant differences compared to healthy controls. In none of the subjects studied was the lactate/Cr ratio elevated. The spectroscopic findings were compatible with subclinical hepatic encephalopathy in the one patient with de novo WD and acute hepatic disease, but this does not play a major role in brain dysfunction in patients with treated WD. Additionally, there was no evidence of increased lactate concentration, indicating that cerebral energy metabolism was not grossly impaired.

    Notes: Zusammenfassung Die Beziehung zwischen dem Organgewicht und dem Leberzellvolumen von Lebern gesunder, weißer Ratten wurde an histologischen Schnitten mittels des Meßquadrates und desLeitzschen Integrationstisches untersucht. Hierbei ergab sich, daß zuerst bei ganz jungen Ratten die Lebern durch numerische Vermehrung ihrer Zellen wachsen, dann — etwa von der 2.–3. Woche nach der Geburt ab — durch vorwiegendes Volumenwachstum der einzelnen Zellen bei weitgehend konstanter Leberzellzahl und schließlich wieder vom sog. „kritischen Lebergewicht“ an, das wir bei der Ratte bei 6000–8000 mg fanden, durch numerische Vermehrung der Zellen. Die Leberzellzahl der Ratte und die Kern-Plasmarelation wurden im zellkonstanten Bereich ermittelt. Ferner wurde geprüft, inwieweit sich derLeitzsche Integrationstisch für histologische Untersuchungen eignet.

    Notes: Abstract A reduction in signal in the basal ganglia on T2-weighted images is said to correlate with a poor response to L-DOPA and may help to identify patients with nonidiopathic parkinsonism. Our aim in this prospective study was to use the contrast-to-noise ratio of the MRI signal on T2-weighted images in various parts of the basal ganglia in 43 patients with de novo parkinsonism. Signal intensity measurements were compared to the response to the dopamine agonist apomorphine and dopamine-D2 receptor binding obtained by 3-iodo-6-methoxybenzamine single-photon emission computed tomography (IBZM-SPECT). A reduced contrast-to-noise ratio in the putamen correlated significantly with a negative response to apomorphine and reduced striatal IBZM binding. No additional signal intensity measurement correlated with response to apomorphine or specific IBZM binding. However, there was a considerable overlap of contrast-to-noise ratios between patients with a positive or negative response to apomorphine or normal and reduced IBZM binding. We suggest that semiquantitative assessment of signal intensity in the putamen shows a significant reduction in patients with probably nonidiopathic parkinsonism compared with patients with probably idiopathic parkinsonism. However, this method does not exclude idiopathic parkinsonism in a given patient.

    Description / Table of Contents: Zusammenfassung Von 1970 bis 1984 wurden 480 polytraumatisierte Patienten behandelt, von denen 301 (62,7%) ein Rumpftrauma erlitten hatten. Dieses Patientenkontingent war zu 85% belastet durch Begleitverletzungen am Schädel (69,1%) und am Bewegungsapparat (66,1 %). Die Rate der Thoraxverletzungen lag bei 81,4%, die der Abdominalverletzungen bei 52,5%. Notfallmäßige operative Eingriffe erfolgten am Thorax bei 34,7%, am Abdomen bei 50% bezüglich der Verletzungsrate. Bei Operationsindikation lag die Letalität der Thoraxverletzung bei 70,6%, die der Abdominalverletzung bei 40,5%. Die Todesursachen waren zu 2/3 durch Verletzungskombinationen, zu 1/3 durch isolierte Läsionen bedingt. Von 245 Thoraxverletzungen verstarben 33,1%, von 158 Abdominalverletzten 42,4%. Das multiple Organversagen stellt bei schweren Verletzungskombinationen das entscheidende Kriterium für die voraussichtliche Prognose dar. In der rechtzeitigen Indikationsstellung nach Behandlungsprioritäten liegt der Schlüssel zum Erfolg oder Mißerfolg.
    Notes: Summary A total of 301 patients with thoracoabdominal lesions had concomitant craniocerebral lesions (69.1%) and injuries of the locomotor system (66.1%). In 65.4% operative intervention was necessary, while 34.6% could be primarily treated conservatively. Operative emergency therapy was required in the thorax in 34.7%, and in the abdomen in 50%. Under these conditions, the mortality for the thorax was 70.6% and for the abdomen 40.5%. As a result of mostly combined causes (66%), 101 patients died (33.6%). Multiple organ failure was in the high range (80%), while the most frequent single cause of death was craniocerebral trauma, ranking before thoracic and abdominal traumata.
