Notes: A hypertrophic hemangioma of a 6-month-old girl was studied electron-microscopically. Hypertrophic hemangioma is characterized by few patent vessels, high cellularity, local aggressiveness, occasional mitotic figures and frequent recurrence alter operation. The ultrastructural examination showed the tumor to be composed of three types of cells: endothelial cells, pericytes and phagocytic fibroblasts.

Notes: In an uncontrolled study of twelve patients with miliary lupus of the face, dapsone gave good therapeutic results.Miliary lupus of the face (MLF) is an asymptomatic papular eruption affecting the central area of the face, which runs a chronic course and involutes spontaneously with scarring. Histopathologically, the papules usually show scattered masses of tuberculoid granulomata composed of epithelioid cells, giant cells, and an encircling rim of small round mononuclear cells in the dermis. The actiology and pathogenesis of this condition are still unknown. The treatment of MLF has not been satisfactory despite the use of many topical and systemic agents. In this paper we report that dapsone has been found to be beneficial in the treatment of MLF and has accelerated its resolution in twelve patients.

Notes: Nitric oxide (NO) is known to play an important role in neurally mediated relaxation of the sphincter of Oddi.〈section xml:id="abs1-2"〉〈title type="main"〉Aim:We investigated whether NO donors, such as nitroglycerin or zwitterionic polyamine/NO, applied into the common bile duct or intravenously, may induce the relaxation of the sphincter of Oddi by producing NO in the anaesthetized dog.〈section xml:id="abs1-3"〉〈title type="main"〉Methods:NO production in the sphincter of Oddi was measured by detecting NO oxidation products (NO2– and NO3–) using micro-dialysis methods.〈section xml:id="abs1-4"〉〈title type="main"〉Results:Zwitterionic polyamine/NO and nitroglycerin applied into the common bile duct induced a marked increase in NO2– but not NO3–, in the sphincter of Oddi. Intravenous infusion of zwitterionic polyamine/NO and nitroglycerin induced little or no increase in NO2– formation. Nitroglycerin infused into either the common bile duct or intravenously administered produced relaxation of the sphincter of Oddi, but zwitterionic polyamine/NO had no effect on the sphincter of Oddi in spite of the increase in NO2– levels.〈section xml:id="abs1-5"〉〈title type="main"〉Conclusions:Locally or systemically applied NO donors induce relaxation of the sphincter of Oddi by producing NO, although their mode of action differs in different analogues.

Notes: Abstract To evaluate reduction methods using two of the several types of Cotrel-Dubousset instrumentation (CDI), 39 patients with low-grade (Meyerding grades I and II [12]) lumbar spondylolisthesis were followed clinically and radiologically for a minimum of 6 months. Our surgical methods were classified into three types as follow: reduction by 2-level assembly using double-threaded screws (type 1); reduction by single-level assembly using the double-threaded screws (type II); and reduction by single-level assembly using tulip screws (type III). We found that satisfactory reduction could be achieved by single-level assembly using either the double-threaded or tulip screw without resection of disc material or the posterior leverage method. We found that the tulip screw was superior in obtaining stable reduction. We discuss the mechanism of reducing spondylolisthesis in comparison with that of previously reported methods.

Notes: Summary Cervical disk injuries are defined as a cervical injury associated with neurological deficits, radicular symptoms, or radiological evidence of disk degeneration, but not with a fracture or a dislocation of the cervical spine. Thirty cases covering the period from July 1982 to June 1984 were analyzed, and the following findings are presented. Fifty percent of the injuries were sustained in American football. Sixty percent of radicular signs and symptoms were from the fourth and fifth cervical root. The roentgenographic changes were most common at the fourth and fifth intervertebral disk spaces. Most of the cases responded satisfactorily to a simple cervical collar and cervical traction. The athletes who presented radicular signs and symptoms required 5 months to return to full sports activities, and 60% of these had some residual symptoms after completion of treatment. The athletes who had a block vertebra of the cervical spine were not suited for contact sports. Recognition of the seriousness of cervical disk injuries in athletes is emphasized.

Notes: Summary We investigated the preoperative diagnostic value of MRI, myelography, and CT myelography (CTM) in a retrospective analysis of 17 patients (15 men and two women) who were surgically treated for cervical disk herniation. Their ages ranged from 29 to 63 years, with a mean of 49 years. Anterior interbody fusion using iliac bone grafts was performed following microscopic diskectomy in all patients. MRI, myelography, and CTM were performed in 13 patients, while four patients were operated on after MRI alone. Neurological improvement was observed in all 17 patients after the surgery. MRI proved useful for determining the level of symptomatic cervical disk herniation. Surgery can be carried out on the basis of MRI findings alone when there is disk herniation at a single level which corresponds with the patient's symptoms and signs.

Notes: Summary Fifteen patients with malignant spinal metastases underwent fixation of the spine using metallic implants and bone cement. The operation provided considerable relief of pain in all 15 cases and a significant improvement in mobility in 12 cases. These results compared favourably with 77 patients with fatal metastatic cancer of the spine who were treated without spinal surgery. The technique seems to be best indicated for use in patients with breast and thyroid cancer with a relatively prolonged terminal course.
